Thank u dr. how about the patients that are taking NOAK? like Xarelto, Eliquis? should they have standard pre and post-dialysis heparin doses?

lower the dose and adjust dose of heparin according to trial and error even stop if no clotting

لو سمحت يا دكتور ازاي hirudin يكون dialysable ويستخدم في HDX ؟

even if dialysable we use it. MW of UFH is 5000-35000 , MW of LMWH is 3000-30000 (average 5000) and MW of Hirudin is 7000 and we still use them. so use hirudin in a high bolus dose or continous infusion.

يعطيك العافية ...اذا في عندي HIT ينفع اعطي المريض Mrcumar واخلي INR فوق ٣ وبستغني عن الهيبارين بهالحالة

if you mean warfarin, it is not recommended if there is no other associated indication to use it (eg AF). use Hirudin or Argatropan if available.

type 1 is not immunologic but type 2 is.
so antibodies for HIT will be positive only with type 2. In type 1 rarely platelets will decrese below 100 and resolve spontaneously even if you continued on heparin, but in type 2 you should discontinue heparin and not to use it again.

@@sarasoliman8858 HIT type 2 will take in average one week to develop thrombocytopenia (for ab formation). if decreased below 100,000 ask for the test and hold heparin
