    Been there and had that pain for the first few months after a TKR. Almost 6 months post op now and it’s gone as everything settles down and gets used to its new life. Knee is 95% pain free and feeling great. Shame about the rest of me😊

      Hi Tony I'm almost 3weeks Monday still in pain special in the night can find the right position to sleep I take Advil and tinenol don't help i need sleep I sleep 4 hour walk up every hour

      @@denisewade2512 My 9 month old knee is giving me pain and stiffness in that area. But my 5 month knee is pretty close to being perfect. You just never know.

      Day 8 of post op acl/meniscus repair and the pain is so unbearable. I pray at night but I have neglected my god when I am crying and in pain while doing the exercises. I asked for god for the strength to get me threw this as I said my nightly prayers and something told me to look at this video. Your comment was the first one I saw and resonated with me like it was a sign.. thank you so much for this, I really needed to be reminded to keep praying especially in the hardest moments.

      I never had pain . took painkiller while in clinic 5 days and never again no pain at all.

    Thank you Tony. This makes sense. I was told by my surgeon that it was sciatica. The pain runs all the way done the side of my leg and also hurts when i’m lying on my side and lift my leg to turn to the other side.

    Great video and explanation. Had exactly this problem which caused excruciating pain, including lightning bolt flashes of pain down my leg in the night. My physio did exactly the manipulation you described and the results were almost immediate. I have continued to work on this area with my thumbs and am now back on track to a good recovery after 10 weeks.

      I hope that’s all it is with me. Those lightening bolt flashes of pain are the worst! I scared my daughter who was driving at the time, when I yelled out in pain from one. Can you recommend a video of the instructions you’ve been using to alleviate the pain?

      I’m heading to my physical therapist- this sounds like my problem. Played golf today and stepped off the green to a lightning bolt of pain. I have been feeling this walking downstairs as the knee flexes.

    I had a tibial plateau fracture about six months ago. Your postings on total knee replacement therapy also pertains beautifully to tibial plateau fractures. The sleeping difficulties, range of motion issues, etc., are exactly the same issues one experiences with this fracture. I would love to see these posting cross referenced, to help others with this type of fracture. Your postings on listening to your body’s response to over exercising is so important too. Thank you so much for your outstanding videos. They have helped so much.

      Hi Susan sorry about your injury, I did TKR about 5 weeks ago, whilst the surgeons was doing the surgrey, my Tibial Plateau got broken and they keep telling me don't put weight on my foot, don't walk on that foot, it's can be frustrating, but how is your foot now, is it OK, thanks

    Another great video, T! Your patients are so blessed to have you their therapist😁I'm 6 months and did my first, trott, lol. Not jog🤣 My leg was definitely bowed. Now I look knock kneed...Or maybe it's still adjusting..Question, how long does it take muscles to rebuild? My legs used to be really pretty and nicely toned. Thank you for being here for us😁

    thank you so much for this. this is exactly the pain that makes it hard for me to walk any distance at all and what makes even more sense to me was when you mentioned about bowed legs being straightened...that is exactly my issue. I've always had bowed legs and of course with this surgery, it's been made straight. This pain is excruciating to say the least.
    I will certainly research the AP & PA glides and rotation exercises you mentioned.

    Wow Tony, this is exactly the issue for my ltkr after 5 months. I have mild pain at the top of the fibula and that is where the clicking and clunking comes from. I have mild pain lower down between the fibula and tibia plus a bit on the front of the knee at the top of the tibia. No physio backup still in the UK. I have rubbed ibuprofen cream into the joint (not advised anywhere) but it does genuinely seem to make a big difference. Thanks again for sharing your expertise, very much appreciated.

    Thanks so much for these explanations. I had right TKR on Oct 8th and things are going well. I had left TKR on Nov 19th and 1 1/2 weeks in, I had and still have the most intense pain possible when I get in and out of a seating position. There is NO pain when walking or sitting with legs out straight. I basically cannot bend the leg. My PT was able to eventually get the leg to bend last Thurs., but reverted back as soon as I got home. I see my surgeon tomorrow and we’ll see what he says. Sounds like it is one of the things you describe here. Thanks again!

      @@barbarasiegel3118 that might be from sitting a lot after surgery. I had hip pain as well but that has resolved after periodic walking and PT. I was also told I probably had bursitis there a few years ago.

    Love your videos. This is exactly my issue. I was extremely vagus. I am 3 months out of rtkn with great range of motion but much stiffness on.the outside of my knee area.

      @@totaltherapysolutions ...no, what is the name of your group? My knee is so great and straight now. So so happy about that. I can walk at a good pace for 45 min, however when I stop I get quite stiff. Icing and exercising it and some days are worse than others. Thks for all your videos, love them

    I experienced IT band pain about 9 mo post-op. Was doing great until I tripped over a parking lot berm and hit the outside of the knee on the pavement. Luckily, no breakage. Sent back to PT and dry needling. My PT said that dry needling needed to be done higher up at the glute and hip level where IT band goes. Totally worked. Clamshells are a lifesaver!

    Thank you Tony your explanations are always so clear and concise. I’m a week postop and that sounds just like what I’m experiencing when I do the flexion exercise on the bed, on the chair it’s less obvious so I guess has to do with loading.
    I’d be very keen to see a video on the sort of moves to help free it up if you had time.

    Totally makes sense! It is annoying! Down to the lateral ankle! Especially at night. I'm very happy with new knees overall the grinding pain gone. Had same discomfort on left side took 5 months to stop.

    I am getting lot of pain on the sides of my knees just where you mentioned. I have had two surgeries and it’s been almost 9 months and I am still not better. How will this pain go away ? Will it ever go away? I thought after knee replacement I can do all my normal activities and I will be pain free. I feel very depressed and this is affecting my mental well being as well.

    Hi Tony exactly one of my issues, especially when practicing going up stairs. It may be a loading issue as you suggest with the new knee now being straighter I suppose the body is adjusting. I have noticed on stair climb I’m loading flat footed rather than ball of my foot. That said I am making progress slowly. Your videos are really helpful. Thank you 🙏

    This is exactly where my pain is coming from especially going upstairs before I have the tkr in about a month. Have bone on bone but no pain in mid knee just problem getting up and down motions and grinding sounds that lurch me forward when it catches to sometimes being off balance. Sounds like having the tkr will not eliminate this problem. Currently having peroneal nerve issues mid calf and lateral side of foot.

    1. Get up and move around frequently. Don't sit in chairs, couches etc with knees bent at 90 degrees for longer than 30min at a time
    2. Avoid >90 degree flexion of the knee to prevent tearing up the meniscus (avoid child's pose in yoga, deep squats, take care gardening or working in the shop, etc)
    3. Buy a sit/stand desk or desk converter if working from home
    Thanks for the fibula mention! It does seem likely that surgery anywhere around the tibial head might also impact the fibular head in ways which could take extra time for the patient to adapt and even affect movement and loading patterns on the opposite leg. I shared above my three simple tips for knee pain for folks who are in a comparable situation to me. My background is that I was a gym rat into heavy lifting but since covid I became a little sedentary and was sitting 12+ hours a day in a sit only desk and then going on long runs late at night. I had a lateral meniscus repair about 6 months ago and now have some lateral knee soreness that clears up when I do the things above.

    Great video. I am now five months after TKR surgery on my left leg. I have a slight pain on the lower inner knee. What will be the most possible cause for this. Other than this pain I am fine. Totally recovered, feeling thousand percent better than before. I walk two miles a day etc

    This is exactly where most of my pain is coming from. The pain increases upon flexion and feels better on extension in this area. Thank you for info on the possible causes. I believe I had a bone spur in this area that was removed during TKR surgery. The leg had become knock knee'd over the years and is now straight. I'm hoping over time the IT band an ligaments will adjust. If there is still arthritis there I hope it doesn't increase. Your videos are so helpful and I joined the FB group recently!

    This is such good information. I am almost year out from my double knee replacement and I have never received such a clear explanation of why I still have residual pain. This makes so much sense. Thank you for this video! (I would still do the surgery all over again, I just wish this had been explained to me up front so I could set my own expectations much better.)

    Great explanation. The lateral collateral ligament? I know the feeling of pain from the ITB which was repaired on my other knee in 2016. My TKRs are 21years old and I am getting a lot of pain on the lateral side, opposite of leg which had the ITB repair in 2016. This pain has been manageable up to the last six months. I don't think it's the fibula head.
    Originally I was having issues with the Bakers cyst which I felt that pain further behind the knee and in the top of calf, in soft tissue, I treated that with deep massage. This pain is starting to extend up and down on the lateral side and is now starting to catch. Best that I can explain it.

    Tony, I am 6 weeks post op as of today. I am having a lot of lateral pain both in the fibular head area and extemely tight IT band. My surgeon looks at me like I’m nuts and just says thats not where people complain of pain. Can you give any examples of what to do for that pain? Your video is the only place I’ve found mention of this immedialtly post op and not years later.

      Does your IT band snap As it glides across your knee during flexion? I have had this issue since the day after my surgery and when it snaps the pain is excruciating. It has not improved and I am Hoping it was caused by the swelling.

    ITBand and lateral collateral ligament.... I used the graston message ... problem gone. My problem as you mentioned ... I had bowed leg which destroyed my meniscus on the medial side. Then I took some Meloxicam for about 3 weeks and that pretty much took care of the inflammation... doing great now. Worked for me.

      Been almost 7 months since my ACL surgery and meniscus repair. I still have the pain. Like you've mentioned, I have bowed legs too. I guess that's part of the problem. Did you add any special exercises to your rehab to get over it?

    Finally a video helping me 🙁but i still have the pain. This my second TKR in 8 months on the same knee. 😢 they had to redo it and put in a smaller one and that stabbing on the outside is still there. Ugh it's only been 5 days postop so 🤷🏽‍♀️

    I had total knee replacement 11 months ago. I have great range of motion. My problem is pain & swelling after walking even couple of blocks. Tired of taking anti inflammatory meds. Surgeon says, structurally ther is nothing wrong. Pain seems to be in the tissue.

    I had continued lateral knee pain after TKR, especially with any twisting movement. Did my own research, that led me to suspect problems with my popliteus tendon. A cortisone injection into the tendon done under ultrasound ended months of pain. But my surgeon never suggested this tendon as the source of my pain; I had to bring my research to him.

    Love all your info. I had tkr 6 months ago. Couldn't take anti inflamitory meds. Still having pain, lots of swelling. Nothing seems to help. Still doing all excercises. Scared to just stop. Now having burning on the outside and part of the top. Help!

    Currently a month out and recently started with the lateral knee pain. I have also had a lot of pain along my shin. Pain is terrible at night which makes sleeping limited. What blows my mind is that no medication relieves the pain and the only thing i took before surgery was ibuprofen once a day. I always credited myself with a high pain tolerance but this is miserable.

      Me too have found just exercise all the time best then elevating icing. Happy with TKR think we just have to not give up 🙏 be patient all settles sooner than later. Good luck in your recovery.

    I had total knee replacement ln 2021 went well went back for check up got told I have cement loss in my knee told the surgeon I have not had fall went for scan haven't heard nothing since

    I've had constant pain from day 1 of my knee replacement since 2015. On the outer right side of my knee about the size of a quarter. Numb and burning . Dr did 3 xrays within about 9 weeks. Each time he said no problem with the knee. I'm still in pain daily. I was in rehab for a month from 2nd day of surgery. Trying to get my knee to straighten. Then more therapy after going home. I don't know what to do. It's my right leg.

    I’m ten weeks po L tkr I have been doing wonderful till last week when this aching annoying pain developed and I could hardly bend my knee to go up the stairs or down. I was bowl legged so all this makes sense thanks

    It’s been 4 months since my total knee replacement and my pain is unbearable I explain to my Dr wheee my pain is and he is at a loss for words because mri and X-rays show nothing wrong in this area but I can barely walk most days. If I sit to long and go to get up it is very difficult and when walking it feels as if it is going to buckle. My Dr has scheduled me for another surgery but plans on resurfacing my knee cap which is know where close to where my pain is at. Also to relieve any pressure around my it band. He has not mentioned anything about this area even though I point it out to him all the time. I am nervous about the surgery because I feel he will not find anything wrong especially if he is not looking at the area I am explaining to him the pain is at. This video is the best explanation of where my pain resides at. And I am torn whether or not I should proceed with the surgery even though I know something is clearly wrong in this area or should I delay it and wait it longer.

      the same for me also.i cant straight. my leg out so the surgeon says he wont give me.another surgery. so im trying to go to another surgeon. i cant walk hardly my leg buckles the pain i cant describe. and my leg isnt straight wtf i can bend my knee a bit .what has helped u

      @@totaltherapysolutions Hi Tony I recently had my knee replaced I am about 2 months out. I do have pain in that area that you are pointing to in this video. Hopefully it will subside somewhat. My question is is it normal to have clinking in the knee almost like., Grinding sounds. I did not see my doctor but I saw a nurse on my first visit after the operation. She said it's not normal to have too much of that noise grinding. So I'm going back to the original surgeon in two more months to see how I progress. What can cause the grinding and clunking sounds?

    5 weeks into my TKR (R). Still can't walk freely swollen and pain. Dr. said recently I'm 85% ok & bal. 15% will be good in 5-6 weeks. Not easy though.

    I had a right total knee replacement 2 months ago and my range is great but I have really bad pain when getting up from sitting. It’s very painful. Is that normal or something like you are describing in the video. My therapist says it’s normal at 12 weeks. It’s actually very similar to pain I was having before surgery.

    Hi Tony it's two years since I had a left knee replacement on the 29th October 2019and and am still having issues with the said Doctor recommended scar tissue removal which I did in August this year.The knee is bending a bit better but am experiencing to much pain especially on the lateral side of the knee. Am tired using icepak and taking pain killers. Please tell me how to treat this issue please

    Can you do a video on septic arthritis of the knee? I'm now going to have MUA on Feb 8th because of scar tissue build up. The infection was staph aureus and was severe. I was septic and I had emergent surgery 6 days after symptoms started.

    Interesting about the fibular head. This seems to be exactly where I get a weird sensation after trying to go upstairs. Then I try to massage it out and eventually will go back to normal. I had knee replacement 6 months ago

    I had by lateral TKR 12/10/21. Almost 6 mo. later I still have swelling and pain in both knees. I still don't have total full flexion and straighten. Im still in PT and the pain is still almost unbearable. Should I still work through the pain

    wow, I have not even hand my replacement yet and I am experiencing that pain in the knee that is not going to be done first, but will also be done. I am in so much pain, I am in bed, trying to do excercises to keep my muscles strong but all this information is really freaking me out. I have no choice, I now have RA and the pain causes me to scream and my knees not to lock when I stand. My poor grandsons run to me when I can seem to stand up and they push on my knees to force them back. What can I do prior to surgery to make my surgery as successful as possible. As it is I am going to be in a wheel chair soon as I can barely take a few steps at a time and this is with a very supportive cane or a walker. I keep them under a heating blanket so when I stand from the bed it is not so bad, but it is not without a lot of pain. I am already on pain meds, for a very long time due to a devastating accident that put me into traction for 7 years, every night, so I could function. I am having surgery in Jan and although I push to stand, there are times I just cant. I do not know what to do at this point. I am afraid my muscles will not be good enough to support the replacement. Any suggestions?

    Thanks for sharing this video. I'd my knee replacement 5 weeks ago. I'm healing but pain is insane however it's getting better. I just want to ask a question is there any suggestion to relax the leg during the night time as I've to move & regulate my leg every hour. I'm desperate for 1 good night sleep 😴

      Hi I’m now 10 weeks had the same issue up to a couple of weeks ago alongside an awful clunking sensation anytime I tried to turn with restless legs, the pain gradually improved by week 6 but the pain meds were pretty hopeless anyway bar the recent introduction of cocodamol 30/500 at night, which has helped me sleep a bit better, alongside a comfortable pillow to rest between knees. Hang in there it will get better!

      I'm on the same dose of cocodamol aswell but no relief in pain yet. I'll try pillow at night time. I did few weeks ago it wasn't comfortable for me.
      I did have some sleep last night but again with every hour of regulating & moving from one side to another. Hopping it'll get better in coming weeks. 😊

    Hi Doc, thank for your videos, I had a TKR done 5 weeks ago, during the surgery the surgeons as they say, mistakenly broke my Tibial Plauteau and all they keeps telling me don't walk on that foot, don't put weight on it because it can get worst, how such a thing happen, this can be so frustrating, I'm in my 60s, so how do I move about without using my feet, I'm dependent on myself, will like to hear from u please, every now and then I get a lancing throbbing pain, on top or bottom, where d new knee replacement is

    My pain is lateral but higher up, more in line with the knee cap. Pain is always worse if you're scared. What are your views on TENS machines? I am 5 weeks post TKR with the beginnings of an ulcer thx to high does of pain killers. Trying to wean myself off them atm🤔🤨🤔

    Have any of your patients experienced popping behind the knee, it feels like something is getting caught and then a pop when the release of whatever is caught let’s go, and they have bruising on the back of the knee.

    I had/have EXTREME pain in lateral side of both knees. My left TKR helped, but it still hurts. I had that knee done at 30. Getting my other knee done in couple of months. My orthopedic told me the only way he could make me pain free was to fuse my knee, no thanks.

    I’m 8 1/2 weeks post op & I do have pain on the inside of my knee…I don’t think it’s fibula related …isn’t that the opposite side!? Could it b from detaching my patella & re attaching it? It has to b moved out of the way…it’s been problematic since the beginning

      I am a little further than you and had to have a manual manipulation due to my knee freezing, but now out of that I have pain where you are describing. I feel that it has something to do with the knee cap not moving like it should. I feel PT is part of this....just being forceful with the exercise not at my rate....

    Help!! I had a second surgery because I was infected by the superglue or had a reaction to it. Second surgery “debreeded “ the infected area and used staples instead. I was 6 weeks ahead of pt schedule (per therapist) at time of second surgery. After second surgery this pain began. PA told me they stretched skin over fibula to complete second surgery and fix the problem with the suture. Since then clicking in the knee and continuous pain where you are describing in this video. My pt would massage it to release the nerve and muscle. Lost my pt due to insurance change over. Any thoughts!?

    Hi! I do have that pain, (after the MUA) i am taking pt, and anti inflammatory medicine. But I can’t walk or going up and down the stair. Is painful.

    Hey Tony my question is two weeks after surgery is a physical therapist supposed to try to move my knee backwards I went to see the physical therapist first time after two weeks I was getting in-home physical therapy and I was doing just fine I had to go to physical therapy at the doctor's office he got me up on the cot and put small pillow under my ankle nothing else in between and he pushed down on my knee and ever since then I have had problems could that be because he was pushing down on my knee

      Sorry to hear that Judy. I will say "patellar mobs" are a common part of the physical therapy treatment program, and I do them in the clinic, but I have had several patients who reported a negative response afterward. To answer your question, yes it is normal, but if you don't like you are well within your rights to ask them not to do it.

    What about pain on inside of knee. I have 133 flexion. I was laying on stomach and using rope to pull my leg towards bottom like I did in therapy. This was 2 or 3 weeks ago, next day my calf and inside of knee swelled and was very tinder to the touch, knee got tight also, it has gotten better, but still feels off, could I have damaged medial collateral ligament. I can walk fine, but when sitting it hurts to straighten out leg and getting out of chair. Need info thanks.

    I wish I did a whole lot more research before I had what is now (3) yes three surgeries in two years on my right knee😢😢😢
    Needles to say my pain level is way more than ever 😢😢😢
