    I've been struggling for years with leg tension, severe back pain, and other lower body issues. The medical profession makes zero effort to deal with about 50% of physical problems, from my experience. I just did these tests and for the first time I have a least a partial key to some of my issues. I had problems with all of these tests indicating that this is something I need to work on. Thank you for helping me understand somehting that multiple doctors, surgeries, and other practitioners could not identify!

    Really helpful information. I did the tests and my left psoas is definitely tight. I recently had pain in my left hip/upper thigh, particularly climbing stairs after a walking tour of 20,000 steps. I walk a lot so the number of steps was not unusual though I average about 10,000 per day. looking on UA-cam to figure out what is causing the pain is tricky but Sam's experience and knowledge give me more confidence to experiment with the exercises. Thank you, Sam.

    Your videos are excellent, with clear concise explanations and demonstrations. My right hip is to tight I can't bring it in anywhere close to my body. Doing the lying down test, my knee adducts...can't even imagine having it rotate externally! When I did the standing hip flexor and tucked my butt under, it was instant relief. I'll be doing that one regularly! I'll also look for other stretches that might help. Thank you so much!

    Dear Sam, thank you so much, loved this video! But when I was doing test number two in static as a stretch I had strong pain in my hanging knee, more on the outside, so I shouldn't do this stretch anymore? And what that passion could be coming from? Thank you so much!

    Insightful vid thanks sam. I'm a 19 year old guy with no family history of scoliosis and I can't even sit properly anymore because my lower back/lumbar feels bent and very tight on the left side, extremely uncomfortable to sit without leaning to one side. No injuries or trauma to the back. I believe it started from slouching/leaning to one side while sitting for prolonged periods. Any idea what it may be? And how can I address this with exercises? I experience intense lower back pain when lying down on my back but no leg pain

    Thank you, really appreciate you spending time on this..
    I’m 38 had a hip resurfacing at 30 metal on metal, ended up with metalosis. Had a revision 3ish years later minus the acetabular cup, a dislocation within 2 days, then he replaced the acetabular cup and the femoral stem again. I’ve had terrible groin/back pain since then. 4 orthos including the original one looked at it, first two from the office that did the surgery. Last two basically accused me of seeking.
    I can’t sit/stand for too long or it’s very painful getting up.
    Car rides aren’t good, took a road trip and couldn’t walk for two days.
    I look at the images a lot, the cup isn’t “sticking out” can’t remember the term they used.
    Haven’t been able to work since then. I guess is there any advice you give someone like me?
    I understand it’s complicated, I’ve heard that all too.
    I’m at a loss and appreciate any advice you can give.
    I’m 5 1/2 months out from a appointment with the university here now. Seeing a phys therapist twice a week, went to a neuro, and found asymmetrical bulging in l5 s1 on the side of the replacement and some degeneration.
    Could the tension of the psoas be causing the issues in my lower back? It makes mechanical sense to me.
    I’ve had injections and ablations in the lower back with no results. Sorry being long winded, any advice?

      Sorry to hear things have been so challenging. Its hard to give any recommendations without knowing the individual's specific situation, but I always recommend people review "Pain Neuroscience Education" to get a big picture of how chronic pain works, to essentially get a fresh perspective and consider some other options in the therapeutic process, especially when it appears the mechanical approach isn't providing the desired results. Some excellent videos on UA-cam on this topic, and of course I have a number of podcast interviews here on my channel in which I discuss these topics.

    Wow! This is just a wonderful explanation. I've seen 2 orthos, 3 PT and I work with a pelvic PT. And I tried 3 chiropractors. I read, watch and analyze everything trying to set a lifestyle that will deal with my hip pain and cannot get a good analysis of my situation. How do I find a knowledgeable practitioner in NJ? Thank you.

      This is a wonderful video and could explain the reason for my hip pain and poor balance. Unfortunately it would be difficult for me to do a self assessment. I've been to orthopaedic doctors neurologists, physiotherapists and more and none of these professionals have assessed my psoas. The main suggestion was surgery to fuse my spinal discs. This cannot be my first option unfortunately. Can you please advise me who can help assess my psoas in Pretoria, South Africa where I live? Currently I'm seeing a Chiropractor who is doing his best to help me but this topic is new to me. I wish you have the name of the professional I can possibly see. Thank you so much .

    Sam, I learned so much from this video thank you! So I failed the rectus femoris test. My tibialis was hanging at an approximate +15° angle which really surprised me. The reason is because I can do the couch stretch for example against the wall and have my head touch the wall behind me. While I feal an intense stretch I can relax and breathe with control. I came here because I thought I have tight psoas but according to the test it's within normal range. Finally, stretching the quads while standing and with a posterior tilt barely feels like a stretch. Any thoughts? I have lower back pain and uneven hip rotations. Left: good internal and terrible external rotation. Right side is the exact opposite.

      In this type of case if someone is standing with the heel touching the glute and with some posterior pelvic tilting and do not feel a rectus femoris stretch, then I would confirm its being done without the femur in an abducted position (stretch being executed incorrectly). If so, then I would next assume the femur is excessively extending during the Thomas test, which may cause the knee to extend during the test. This would occur probably at approx. -10 degrees from horizontal. Additionally perform some contract/relax movement against gravity to see if there is any guarding that is occurring subconsciously. Personally I'm not massively concerned about a little shortness in the rectus femoris by itself, especially if the individual is fairly fit.

    How the heck do i choose the right practitioner to diagnose me!! Hip pain needs to be diagnosed but i dont even know where to start -your a neuromuscular massage therapist, i have never seen one here north of boston. Donu go to an orthopdist or a pt with a doctorate or a skilled chiropractor? Help!!!

    please explain about how to release to soas that attached to L1 all the way down to S1 ! mine is short on my left side for the past 6 years and half and causes back thickening and pain on that area (exactly where my lumbar vertebrae attaches to soas) I got an x-ray and I realized that my lumbar spine bended to left with a 7.5 degree levo scoliosis , also when I raise my left leg up and going back down I hear a big popping noise in my pelvis inside of the thigh , I can feel that my hip flexor click on that area ! please explain how to release the soas completely , cause I do the stretches for the past 4 years , every single day , but It's just temporary ! I need a long term relief solution 🙏🏽 !

    My hip was locked until age 21 then after replaced doctor didn’t think to send me to PT so now age 50 very tight. I can’t even reach my foot behind me or bring knee to chest!

    What exercises could one do for the tfl and IT band. When I did this test it was on my left leg that rolled out. Which I have suffered burning and weakness in this area for a year now after snowshoeing all winter. No one has been able to tell me what this was

      There is nothing really to be done for the IT band. The TFL muscle is not usually weak. Its often fairly dominant, and if we use my average client case, its because there are issues in the back of the hip, or in the joint. Usually the TFL needs to be massaged by therapist or foam roller, etc. because stretching it doesn't work well, and the exercises would target the gluteals/hamstrings, etc. This will be the topic of upcoming videos on my channel.

    PLEASE I NEED HELP. When bringing my left knee up to my chest in the first exercise, my leg on my right side comes off the ground because of shortened psoas and my leg is shortened and feels like it’s up in my abdomen and my rectus femoris is so tight my leg is stuck straight, and when I try to bend knee everything moves as one and it literally shifts my body and back out of natural movements and it’s so painful to be alive, even with Norcos. I feel like my body is imploding on itself and I need help. 5 different PTs for last 2 years have all people younger than me fresh out of schooling with no hands on life experience and they’ve only made things worse..PLEASE PLEASE HELP

    What if I see no movement with the thigh at all, but I DO feel a lot of pain at the front hip flexor at the bend of my knee? When I pull it in, there’s a lot of pain in the groin.

      Hip flexion is primarily limited by tension in the glutes from a muscular perspective, so I always check that. Pain in the groin region (depending on the severity along with other findings) should be referred to doctor for further hip evaluation.

    I was paying attention until o saw his shoes. There’s about 10mm difference between heel and toe. No professional therapist would wear shoes that would affect the way his body moves and behaves

Such statements lead me to believe you are no where near an up-to-date evidence based practitioner. Feel free to make a full video on your channel to fully demonstrate your expertise on this topic and inform us all about your findings. And be open to criticism. Please send when its complete.

      @@otterlyanxiouspenguin think about it. its common sense. no shoes - your feet are flat and straight. your hips are straight and aligned. your knees are in the correct position they should be.
      you put on a pair of shoes that has a 1cm difference between toes and heel. your hips tilt your pelvis, placing the pelvis and knees in misaligned manner.
      i honestly don't care what this "professional" or you think. having your knees and pelvis at an unaligned angle places stress on both your knees and lower back. it's not me making things up. its factual.
