I have been a general dentist for over 40 years and I would like to give my view.
Although I agree with the premise that crowns are over prescribed by some dentists,
often a crown IS the correct solution for many situations.
Yes, front teeth need crowns less than back teeth.
The stronger biting forces on back teeth indicate crowning more often.
The front teeth are more amenable to fillings and veneers.
But sometimes a crown is the best solution there as well!
It depends on the judgement of the dentist.
I think non-dentists are not able to make this judgement as well as dentists.
So if a patient is skeptical, I suggest one or two additional consults with totally different dentists.
I have practiced conservatively with the idea that the less you do to a tooth the better off it is.
But don't be afraid of crowns; they do an important job in reconstruction of a tooth when necessary.
And no need to regret having had them. I as a dentist have had twelve crowns in my mouth.
And I am happy with them. They saved my teeth!
If faced with many crown recommendations, get multiple consults.

As a dentist, you can afford your crowns. I have a premium dental insurance plan, but front upper incisor crowns were denied as "being cosmetic," even though that is not at all the reason for them. As a 77 year-old, very happily married to a beautiful, very highly educated woman who is 31 years younger than me (she married me for being a medical research scientist, as that is what she is), I most certainly not having crowns to attract 19 year-old bubbleheads!

FYI, I got a crown on my front top middle tooth, that I broke when I was 12, I got the crown when I was 20, no root canal and the tooth was sharpened like a sword point, I am now 65, never had any issues with, it’s as solid as the first day it was installed, and never got pain or sensitivity even after 45 years.

@@williambentley5347 Hi William, the best is to follow your dentist’s advise, if you trust him and he’s good. If your tooth is very strong and I’m not sure since you had a root canal done, but if it is a crown is the way to go, but today’s best option would be an implant, the only drawback is the cost….. here in Montreal, Canada the cost of one implant will run you about 3,000$. My crown is still 100% because he was a really good dentist, he did not do a root canal so my original tooth that the crown is over is very strong and alive, once you do a root canal the tooth is considered dead, it will be more prone to cracking, so the minimum you can do right now is to get a crown to help protect it from breaking/cracking, but this option is not 100% guaranteed since you got a root canal done before, so in this case the best solution would be to remove it and get an implant. But talk to your specialist first…… Hope this helps, good luck.

I’m 63 and my front teeth had broken because of age, so my only option was to have crowns. My bite was also way off. Most of my adult life, my two middle front bottom tooth were hitting underneath my two front teeth because my but was not correct. My dentist adjusted my upper bicuspids by lengthening them with crowns to correct that problem. So in my case, crowns were a wonderful way to get my smile back. But you still have to be careful and look at your X-rays if a dentist is recommending crown because my husband has healthy beautiful teeth, and when he was in his late 40’s, a dentist told him he needed crowns on EVERY TOOTH! That was an absolute lie. He’s 63 and has had approximately 5 fillings and his teeth are perfect. So you do have to be careful, just as this dentist is saying.

The reason why the tooth tissue needs to be removed has no relationship to wether the crown is temporary(plastic) or permanent. It is removed so that there is space for the crown itself thus it cannot be avoided. Crowns are a very good solution in cetain cases. For example- teeth which have had multiple obturations and are very carries unresistant - a well made crown can preserve this tooth and put an end to countless dental apointments for new obturations. Another example a tooth with only a few walls left of the crown wich is very prone to fracture. Also a bridge of crowns can stabilize teeth which are shaky and prevent extractions. Ofcourse putting a crown on a healthy tooth just for estetic purposes is a horrible idea due to the reasons stated in the video. The end result of a treatment with a well made crown in a lab and a crown made with the new technology machines is not much different. Thats is beeing real on the subject.
    peace!

I agree. He needs to clarify specifically that he is talking about conservative issues like esthetics or small decay. And youre right, the preparations are done not because of temporary but so thats theres room of the crown and that it is less prone to fracture. Thin veneers fracture too!

The amount of enamel being removed for my crown really worried me, but at least I got a zirconia one made in one appointment. So, I think less got removed than if I had done it another way. I'll only do major dental work if I felt it absolutely necessary, and I believe it was. I couldn't deny how the tooth being treated had decayed (covered full width of tooth down to gum line and there was a tiny hole developing). So unfortunately I had to get the root canal and crown to save the tooth.

In 1963 I was 12 years old when I broke my front tooth. I had a root canal done and a temp crown. I never got a perm crown because we were poor. Fast forward to the mid 70s, I broke the temp crown. I was still poor so a dentist made me a resin repair or if you will, a resin replacement for the temp crown. It lasted until last year when the back side broke away. I have to get it fixed soon but now that I'm 70 years old, I don't want to spend $2,000 on a crown and that's what the dentists want to do here in my area in Maine. It seems like everyone only wants to do expensive work and not try and understand the patients needs. I could die a week after they put that expensive crown on. What a stupid waste of money in my eyes.

Too late... I've got my temps on the front and bottom . Qualifying admission - I'm almost 60 years old. I've never had much trouble with my teeth (I have all of my wisdom teeth), except for a couple that I broke by trying to use them for something that a tooth is not meant to be used for (popcorn kernels are the devil - lol). Regardless, I'm still very optimistic about my outcome. Yesterday, I had 4 frontal upper and 4 frontal lower shaved for crowns (in for a penny, in for a pound). So far so good but I will not tell you that it was painless. No matter how much local anesthesia the dentist gave me there were 2 teeth that were a problem in that respect. I soldiered through and the whole process and it completed within an hour. I've got temps until the 16th of October. Honestly, the temps look at least 40% better than what I had before with all of prior chips and discoloration. Don't get me wrong, my teeth where not horrible to look at or decyayed before but I did not like the discoloration or chipping. I asked multiple dentists about Veneers because my bite is slightly under rather than over at the front. When I say slightly, I mean a couple of mm. So, I am not a candidate for Veneers due to my bite. I would have loved the option of Veneers but in order for me to get them I would have had to have jaw surgery to realign my bite. I am really looking forward to getting my crowns next month. I'm the type of person who is willing to pay for a 60% improvement of function and aesthetics. I feel good so far with what my dentist and I have done. If that changes on 10/16 I will be sure to let you know.

veneers and implants are for rich people
crowns and bridges are for middle class people
removable denture or none at all for poor people
see, we explained these things to our patients, give them the best options, but unfortunately we, us live in a third world country so i must say , the real deal for this is to educate them as early or as young as possible the importance of dental health.
look you have a HUGE clinic, in my country your place can house 15 dental chairs or more.

If you are getting a crowns, I would have a root canal procedure done first (with or without infection of the root). Because the need for one later exist(s). Furthermore, you can have problems with the tooth being sensitive and painful in the long-term with the crown on a tooth without a root canal. However, I would never use crowns on front teeth unless most of the tooth was destroyed and required one.

I had a crown after a root canal on a back molar. No choice really, 85% of the old tooth was amalgam and what was left had some decay. There was just about enough left to save the tooth or it would have been an extraction. I’ve got older crowns that have been in for 20 years, no issues, so I don’t get what the problem is.

Just found your channel looking for insight for my overfilled root canals. Great content. I'm young and have a lot of root canals done.
I have a front tooth they wanted to do a crown on because it has decay in that backside. I'd be interested in getting work done on that tooth by you. Also: I've wondered why they don't make solid implants (as in one complete piece). I always thought they were. Probably due to pain of bone healing?? If I went to you, could you create a duplicate of the entire tooth and implant it right after pulling it? And could I get a complete implant for a molar?
    Thanks so much!

Basically, there are situations where you just cannot go straight to CEREC milled veneers and achieve good looking results. It is really not okay and kinda funny at the same time saying that you don't really need a lab anymore. I bet that as a clinician you have not enough time already and, I deeply doubt you're using the CEREC - if yes - then show me your results, your veneers. I hope they are great, then sure go for that if it's efficient for you. But the major amount of dentists can't even contour a temporary, so I would be worried about applying this to everyone. Aside from that, the video is massive! Great job!

I’ve had this discussion many times with colleagues. I would agree with what you’re saying. On a virgin tooth, in ideal or nearly ideal arch form, veneer all the way. But the vast majority of patients I see, have large restorations and/or don’t want orthodontic movement. Veneers do not add strength and can’t make significant movement but crowns can.

    • @InnovativeDentalofSpringfield
      @InnovativeDentalofSpringfield  4 роки тому +1

Great points. In the majority of the patients I see, they are happy to align their teeth with Invisalign prior to investing thousands on their smile with taking more tooth structure away with crowns. Invisalign prior to cosmetic work almost always in my opinion. ;)

Front teeth are not supposed to touch each other when the teeth are in approximation (occlusion).
The 1mm space given between upper and lower front teeth is called overjet and overbite. That's basic dentistry that a student would know.
If a surface has decay, that surface acts like it is corroded. We can't do any restoration over a corroded region. It would eventually fail because the tooth structure underneath the restoration would decay over the long run.
Problems with veneers are gum problems - the same as with crowns.
My recommendation - you may get a crown or veneer done, whichever you can afford. But you will need periodic checkups. If gum problems arise because of improper hygiene, you may have to remove crown/ veneer , treat the gum, then get a new one done after gum has healed. This could be anywhere from 3 to 10 years down the lane, depending on a lot of factors including habits and general health of the patient and initial condition of the tooth.

I went to the dentist (which is my dentist since i was 10 years old) last march 31, 2019 I have 4 crowns in my front upper teeth. And i have missing lateral incisor, first and second molar teeth on the left side upper. I told the dentist to replace my old crown front teeth and then put partial denture on the missing molar but she told me that i should get crowns on my upper from canine to second molar of my upper teeth so total of 8 teeth so it will look straight because the crowns on my front were obvious. I was hesitant at first, but I was drive with my decision to go for crown because my canine and first molar on the left is moving backward and because my front 4 crown teeth is obvious. I went home REGRETTING the decision i have made and going through the advice of my dentist and thinking why did she even recommend me such though she told me that i need fillings on those teeth still i find it healthy but only with gum receding on it, knowing that in the future i'll suffer. I promise myself years ago that i wont touch my canine teeth but i did. I should've listen to my mom but i didn't. I regret every single day i made that decision. Though i have accepted the fact that in the near future i will have denture because i have 2 teeth upper right molar that has fillings. @Yanbo Yang I have the same thought as yours as to why these people that are so blessed of having straight healthy white teeth go through procedure as i have seen on social media also they have straight good looking teeth. I am sorry for saying this but this is the truth, whatever shape your teeth or are crooked or etc BE HAPPY with it as long as it has no decay and cavity. Take good care of your remaining real teeth because nothing could ever replace it and that sensation of having your natural teeth, it is a struggle of having a fake one. In my case its in our genes that our teeth easily get decay. But as much as i have my lovely remaining real teeth to care of I AM REALLY TAKING GOOD CARE of it now. I started avoiding soda and other foods that can contribute to the damage of my teeth. I drink a lot of water. if your body doesn't get enough nutrients just like what its needed same with your teeth it will eventually decay (Based on my research). I am sharing this to all of you because the anxiety it brought to me the distressed and even my work is already affected, I woke up early remembering sitting on that dental chair that I SHOULD HAVE SAID to the dentist "NO DOC I'LL HAVE MY REAL REMAINING TEETH. Now, I still have my temporary which i feel is too big and felt like i was wearing a teeth guard for boxing, hence i was even thinking if its possible that i don't want it to get cemented so i could still brush my teeth inside of it when i get my still temporary because I could not be able to afford porcelain crowns. Moreover, thinking that if i could go through procedure like composite veneer though it may sound very impossible but I've seen case of tooth that went through composite even with small teeth remaining, thinking of ways to restore my teeth in a way i can still feel my natural one even if isn't as durable as the real teeth or veneer or crown, I just want to feel it. ;( I am really not ready for dentures. :'( And if by any chance in the future we can possibly regrow, or even missing enamel teeth to all dentist out there PLEASE make it even possible for everyone rich or poor it maybe. I know part of your work is business, and really dentistry is very expensive but I hope to all dentist out there that please do recommend to your patients what is best for their health and both with your pockets. This is not to make you feel bad for those that really desired to enhance their teeth but this is a precautions. I am hoping you get something out from here. It is way way better you look beautiful in natural.
Been watching your videos Doctor O, if only i have seen these vids earlier i would not have went through for crowns. Thank you for being an honest and conservative Dentist as you are. Only if i have so much money that i could easily fly from Philippines to your country (Imagining to win a lottery haha). Hope you could give me some advice, I am really looking forward to it. God bless you and God bless everyone. :)

Thank you so much for this, wish I seen this before I went to get my temporary crown a week ago (but on my molar, not front tooth) - since then, NOTHING but problems- I should have listened to my gut "If it's not broke, don't fix it" . I had to drive over 100 miles twice this week (taking time off work) because the dentist did not smooth out the temporary and it was literally cutting the side of my tongue for 5 days. I have to go back for the 3rd time as it's still rubbing the tongue and I am really unhappy. He also injected my mouth and half of my tongue is still numb and tingling 6 days later, I think he hit the nerve. A total NIGHTMARE. If I saw this video, I wouldn't gone as nothing was wrong with my tooth before this, now I'm paying for it, literally, with money from my own pocket, lost time from work, gas money not to mention entire weekends of irritability because my tongue is getting sawed against my defective tooth!!!! The only good news: I am eating less and talking less. Lost a few pounds in week because it's too sore to chew or even swallow.

Thank you for this very informative video!
I have a crown on the lower right hand side of the jaw and it fell of after around three years of being installed.
I noticed that the inside of the crown when it came off had a rotten tooth, bad smell to it.
The tooth itself did not have any visible decay. Instead it looked ok.
Went to the dentist three hours after it fell off.
He put cement on it and cemented it back in place.
I noticed straightaway that it was too high.
I told that to the dentist and he said that it should settle with time.
Right now it is around five hours latter and the tooth with the crown and the tooth opposite it, feel a sore or tender from the presure of the bite.
    Is this normal?
Prior to it falling off, it was not too high. The only thing the dentist did was adding cement to it.

Thank you for this video! I have veneers that are on my four front teeth. I had them since 1993. I have no issues and love them. I just had one implant done recently and the dentist said he wanted to replace all my veneers because of bacteria build. I was not sure about that since I have no issues with cavity or gum issues, not even nerve issues? I like my dentist, but I need to tell him tomorrow at my appt. that I DO not want to change them. I love my smile, I did not go to him to change my front teeth, only one tooth implant. Which has not been done yet because of a receding gum line that he did gum graft on? But the gum is still not taking to the receded area. Anyhow, that was painful and I just want the tooth implant service which is halfway done, only waiting for the post. I was told they wanted to give me a canine tooth implant and this is why they want to do the crowns so they can match the canine on the other side. BUT I never had canine teeth, to begin with? Yes, it looks normal, but I am starting to wonder now? Any advice would be greatly appreciated. Thank you, Kat

After the initial wait period and your able to floss and from the start after flossing a noticable odor. How dangerous is this the dentist makes the excuse not able to see it take anymore patients because if the Corona Virus. A crown should last 5 to 10 years or more not under one.
