These fans come in many shapes, sometimes cleverly disguised as accessories. 银碳Gintan uses an integrated fan in the fursuit head (in the mouth), one fan under his chin beneath the fur (you can sometimes see it swing out as he dances. It might be a battery for the integrated fan, but I am pretty sure it's a separate unit) and carries that handheld.
