Sir very informative presentation. There is lot of confusion regarding fruits. I myself have gradually increasing asymptomatic hyperglycaemia in 8yrs till it manifested at 13mg/ dl with 1- 2 attacks in 1st yr then 2-3 since last 2yrs. Take Allopurinol after Artifin or Ponston which relieved pain with strict diet control. However, I found flare ups especially after chick peas, chicken even gravee, grapes, oranges, pomegranate, mangoes dahi, white flour bakery items, beside all others well known. I've nothing left to eat except potatoes, banana, en egg, half cup milk. But once it goes away I can eat moderate amount of all these. Uric acid levels have little role than its sudden change especially due to drinks & street food.

Next day I started on Febustat 40mg OD dose. 1 week later he complain of shoulder pain or discomfort. Is it because he stopped taking NSAIDS abruptly or is it due to his previous shoulder discomfort problem (as he had earlier). He stopped taking NSAIDS himself as the pain subsided after 3 days

Good one sir.Clear explanation. But one doubt sir.We see many patients with on and off swelling with suspicious gout but uric acid is normal. One way is to confirm with polarized microscope- but unfortunately many centers don't have this in India. Any suggestion sir.

Sir ,what if serum creatinine is high and hyperuricemia but the patient is asymptomatic and no flares noticed till now ,got on routine checkup ,what will be treatment?
Sir my uric acid level is 8.4 to 8.9 from last 1 year. I was 106 Kg. By lifestyle modification i came down to 78 kgs. Whole blood report is always fine sir but only uric acid comes between 8.4 to 8.9 when i do blood test. When i eat febuxostat it comes to 3.4 but again after leaving meds it gets to 8.4 to 8.9. I have no symptoms of gout or any other symptoms but i am worried about this level Sir. What should i do Sir ?

What is the protocol for asymptomatic hyperuricemia without flare if uric acid >9 mg/dl? Should we still avoid ULT?
Patient on ATT and develop painful enlarged red hit ankle joint with uric acid level 7.4 mg/dl. Should be start ULT and till how long?
