So true Señor Chavez! Sometimes this problem can be uncovered by a process called a use related risk analysis. This is basically a task analysis where you pay very close attention to tasks that could result in harm. Other times, these problems are not revealed until a full-on usability test. The main thing is to identify and fix the issue before the product goes to market and someone get injured - or worse.
