No Destiny, you just dont understand the subject matter. If Boogie had low EPO and high hemoglobin, without kidney disease, while being on testosterone, obese and with sleep apnea (which raise EPO), then there really arent any other alternatives in terms of differential diagnosis except PV. EPO is a minor criteria (because it can be normal in some patients with PV), and in 5% of cases patients do not have the JAK2 mutation (a major diagnostic criteria). The only thing left might be a biopsy, which was not performed; but regardless, the doctor might have told him he most likely has PV while nominally writing his immediate diagnosis (polycythemia) because he simply does not meet the criteria on paper. Now, i am not saying Boogie isnt lying, he is woefully ignorant on a disease he has had for what, two years? But watching you be unable to steelman this due to ignorance is just painful.
@@thedj3319 yeah agree with this take. Diagnosis of PV follows WHO classification which has 3 major and 1 minor criteria. Major criteria: 1.) elevated hemoglobin and hematocrit 2.) Bone marrow biopsy showing hypercellularity 3.) Jak 2 mutation Minor Criteria: 1.) Low EPO Generally it seems that in order to fulfill the diagnostic criteria you have to have all 3 major criteria, or the 1st 2 major + 1 minor criteria. Sometimes you can get away without needing biopsy if you have really elevated hemoglobin and hematocrit and Jak 2 mutation + low epo. So it may be possible that he indeed didn't need the bone marrow biopsy but wanted it anyway. Regardless, the biopsy seems pretty important in the diagnosis and just saying that having the JAK 2 mutation is enough to make the diagnosis might not be completely true. This also only applies if you rule out secondary polycythemia which boogie might have. doesn't mean boogie isn't lying tho like you said.

This causes a lot of complications due to having thickened blood which can cause clots which can cause a pulmonary embolism and/or stroke. Polycythemia Vera is a type of primary polycythemia, which means that there isn’t a precipitating factor that the increased red blood cells is in response to, the body just started doing it on its own. Polycythemia Vera specifically is a type of cancer (cancer is caused by uncontrolled division of cells, which creates malignant tumors that spread through the body). Secondary polycythemia happens in response to another thing going on, in this case hypoxia is the main driving factor. Hypoxia means lack of oxygen reaching your tissues, and since your red blood cells are what carry oxygen, the body attempts to adapt by making more red blood cells (but not uncontrolled making it not cancer). Hypoxia can be caused by lifestyle habits like smoking or obesity, since obesity can cause sleep apnea (stopping breathing while sleeping). There is definitely a difference between the two conditions and it is quite malicious to call secondary polycythemia (which can be solved with lifestyle modifications alone) to cancer that can kill you and requires actual invasive cancer treatments.

I think it's possible (or even probable) that Boogie's doctor _mentioned_ polycythemia vera as a _possible_ cause of measured high blood cell count (polycythemia). Polycythemia is a symptom, not a disease, and as such it is _always_ secondary to something else. One of those things is _potentially_ polycythemia vera, which _is_ a disease, and whose primary symptom is polycythemia. None of that is problematic. What _is_ problematic is Boogie's choice to represent this as a definitive cancer diagnosis. The reality is that his doctor probably only mentioned PV at the end of a very long list of other, _far_ more probable comorbidities that Boogie obviously has as an explanation for his polycythemia (which, again, is just a symptom-- high red blood cell count). This would be like if you went into the doctor with persistent headaches and the doctor said "it could by dehydration, high blood pressure, stress, etc. but in some rare cases persistent headaches can be something more malicious, like an aneurism or a tumor" and then you went home and told everyone that you had brain tumors. Did the doctor _technically_ say it? Yes. Is it a gross misrepresentation of what was said? Also yes.

I remember when SImone Giertz dropped her video about her brain tumour. She was in tears, she was super scared about dying, what that would mean for her fans who enjoyed her content. But the biggest part of her video was gratitude, she was grateful for all her viewers, subscribers and not in a "say it because it's the end of the video and you need to do a call to action" it was a genuine appreciation from someone. Even her spot in that garbage fire of UA-cam Rewind was her stating that without the support of anonymous people around the world she probably wouldn't have made it through. I saw none of that with Boogie. There was no fear for what this meant for the future. It was "I need money for the immediate present, today, now"
That's because the cancer involved was one that doesn't significantly threaten a person's life for a long time. It's the kind of thing that will only cause suffering 15-20 years down the line, and death maybe 30 years down the line. It's not a valid comparison to make. Also Boogie wasn't asking for money. In fact in the video he explicitly tells people that he doesn't want money for it. He says stuff like that because he is used to being a victim, probably wants to be a victim, and probably because he gets better treatment as a victim from his experience. He would have done it for "power" (sympathy). Not only that, but people can react to having the same cancer in different ways, so it's not like it's an indicator of a person lying, nor an indication of someone being a bad person or ungrateful or such. Obviously in Boogie's case he _is_ misleading/dishonest/manipulative, but I'm just saying that people's reactions are never proof of that so don't trick yourself into thinking the wrong things.
Although I suppose you could argue that he will still get more merch purchases and donations even when he says not to donate due to having cancer and that he's just being extra-sneaky, but the gains from that are probably like 10x less than if he didn't say that he doesn't want money, or if he had specifically asked for support, and the moral difference is quite low, too. So overall I'd say it's unlikely motive.

People react differently to these things. People claim being happier than ever after getting terminal cancer, which is very paradoxical and yet true for a lot of people. Others go into deep depression. Others are quite indifferent to it. The reaction to these things is so different among individuals, and cannot be used to confirm/deconfirm the cancer diagnosis.

I used to be a healthcare professional. Worked with cardiologists for 16 years. There is no way in hell a medical doctor, especially a PCP, is going to diagnose and treat any form of cancer without first having it confirmed. If a GP suspected that the patient could potentially have cancer, that patient would be referred to an oncologist and the oncologist, technically, becomes the PCP due to the diagnosis. Then that MD is going to refer the patient to other specialists, cardiologist, radiologist, he probably already has a pneumologist, and in his particular case, a hematologist. After all of those MD’s confirm the diagnosis with a plethora of tests, THEN treatment would begin. Depending on the severity of the cancer, a treatment plan is developed headed by the oncologist. If a GP diagnosed and treated a patient for any form of cancer without having confirmed the diagnosis, that MD should be sued for medical mal practice and they would lose their license.
Yeah, that's weird - did Boogie mention a specialist of any kind at ALL? Complicated patients only hang out with specialists, not GPs. That said, I have some autoimmune issues, and my symptoms don't quite fit into nice, neat categories. For instance, I have Sjogren's locked-in as a diagnosis, but SLE has always been more suspected than perfectly confirmed. Still, they throw SLE on the paperwork and monitor me with certain tests as if it's there, as it's better to be safe than sorry. My treatment plan is not affected either way. I understand it must be different for something more serious like cancer, though. And regardless, I think Boogie's full of beans and needs to be off the internet. Even setting this current debacle aside, he's deeply vile.
You're right that you don't treat a cancer until it has been confirmed, but your description of the diagnostic process isn't entirely accurate. In 99% of cases you won't get referred to an oncologist until AFTER you have a tissue diagnosis of a cancer (ie biopsy confirmed) - otherwise imagine how many non-cancer things that may look like cancer would get referred to an oncologist, this would be a total waste of their time and the patient's time. By the time you arrive at the oncologists office you should already have a biopsy-confirmed cancer, although you may need further working up re staging etc depending on which other tests have been completed. There are some exceptions to this process but this is generally how it proceeds in the vast majority of cases. Of course PCV is a haematological cancer so an oncologist wouldn't even be involved, it would be managed by a haematologist. Source: am a doctor who is training in the specialty that diagnoses cancer (anatomical pathology).

I used to be a cardiovascular sonographer and one of the cardiologist I used to work for would regularly evaluate patients from an oncologist/urologist that would perform robotic surgery on patients so I would have to perform carotid sonograms on those patients. I just know that there is no way a patient would get any sort of treatment for any disease without it being confirmed. I live in Puerto Rico so even though we have great physicians, we don’t have the same access the US does to many treatments and specialists and the protocol for diagnosing patients is to first confirm.

If your doctor suspects that you have polycythemia vera, he or she might recommend collecting a sample of your bone marrow through a bone marrow aspiration or biopsy. A bone marrow biopsy involves taking a sample of solid bone marrow material. A bone marrow aspiration is usually done at the same time.
Yes that’s true, but before that they would do a blood test. For instance in polycythemia Vera va polycythemia EPO levels are different. Vera is an actual mutation in the JAK2 pathway. Vera does not come from other cancers. Also he mentioned a colonoscopy…. Colon cancer doesn’t present with increased hemoglobin.

The thing I don't get.. and maybe I'm wrong... when I went through cancer I got tested and then got referred to my cancer doctor he's the one that put in for my bone marrow biopsy. It was my cancer doctor that handled my medication and blood tests.. my primary doctor didn't have anything to do with it.. so how is boogies primary care doctor handling it as cancer when he should have a separate doctor to do that for him?

I guarantee what happened is that he went to the doctor, the doctor said that he had polycythemia and it was a small possibility that he has the polycythemia vera variant. He then went home and claimed the cancer version for clout.
This is what I'd guessed after the first stream, too. The doctor probably just said polycythemia and while he was googling it, he came across the cancer variant and over time it morphed from "I may have cancer" to "I've been diagnosed with cancer"

If Boogie had low EPO and high hemoglobin, without kidney disease, while being on testosterone, obese and with sleep apnea (which raise EPO), then there really arent any other alternatives in terms of differential diagnosis except PV.
EPO is a minor criteria (because it can be normal in some patients with PV), and in 5% of cases patients do not have the JAK2 mutation (a major diagnostic criteria). The only thing left might be a biopsy, which was not performed; but regardless, the doctor might have told him he most likely has PV while nominally writing his immediate diagnosis (polycythemia) because he simply does not meet the criteria on paper.
Now, i am not saying Boogie isnt lying, he is woefully ignorant on a disease he has had for what, two years? But watching you be unable to steelman this due to ignorance is just painful.

Generally it seems that in order to fulfill the diagnostic criteria you have to have all 3 major criteria, or the 1st 2 major + 1 minor criteria. Sometimes you can get away without needing biopsy if you have really elevated hemoglobin and hematocrit and Jak 2 mutation + low epo. So it may be possible that he indeed didn't need the bone marrow biopsy but wanted it anyway. Regardless, the biopsy seems pretty important in the diagnosis and just saying that having the JAK 2 mutation is enough to make the diagnosis might not be completely true. This also only applies if you rule out secondary polycythemia which boogie might have.

Polycythemia is a condition caused by increased production of red blood cells. This causes a lot of complications due to having thickened blood which can cause clots which can cause a pulmonary embolism and/or stroke.
Polycythemia Vera is a type of primary polycythemia, which means that there isn’t a precipitating factor that the increased red blood cells is in response to, the body just started doing it on its own. Polycythemia Vera specifically is a type of cancer (cancer is caused by uncontrolled division of cells, which creates malignant tumors that spread through the body).
Secondary polycythemia happens in response to another thing going on, in this case hypoxia is the main driving factor. Hypoxia means lack of oxygen reaching your tissues, and since your red blood cells are what carry oxygen, the body attempts to adapt by making more red blood cells (but not uncontrolled making it not cancer). Hypoxia can be caused by lifestyle habits like smoking or obesity, since obesity can cause sleep apnea (stopping breathing while sleeping).
There is definitely a difference between the two conditions and it is quite malicious to call secondary polycythemia (which can be solved with lifestyle modifications alone) to cancer that can kill you and requires actual invasive cancer treatments.

I think it's possible (or even probable) that Boogie's doctor _mentioned_ polycythemia vera as a _possible_ cause of measured high blood cell count (polycythemia). Polycythemia is a symptom, not a disease, and as such it is _always_ secondary to something else. One of those things is _potentially_ polycythemia vera, which _is_ a disease, and whose primary symptom is polycythemia. None of that is problematic. What _is_ problematic is Boogie's choice to represent this as a definitive cancer diagnosis.
The reality is that his doctor probably only mentioned PV at the end of a very long list of other, _far_ more probable comorbidities that Boogie obviously has as an explanation for his polycythemia (which, again, is just a symptom-- high red blood cell count). This would be like if you went into the doctor with persistent headaches and the doctor said "it could by dehydration, high blood pressure, stress, etc. but in some rare cases persistent headaches can be something more malicious, like an aneurism or a tumor" and then you went home and told everyone that you had brain tumors. Did the doctor _technically_ say it? Yes. Is it a gross misrepresentation of what was said? Also yes.

But the biggest part of her video was gratitude, she was grateful for all her viewers, subscribers and not in a "say it because it's the end of the video and you need to do a call to action" it was a genuine appreciation from someone. Even her spot in that garbage fire of UA-cam Rewind was her stating that without the support of anonymous people around the world she probably wouldn't have made it through.

That's because the cancer involved was one that doesn't significantly threaten a person's life for a long time. It's the kind of thing that will only cause suffering 15-20 years down the line, and death maybe 30 years down the line.
It's not a valid comparison to make. Also Boogie wasn't asking for money. In fact in the video he explicitly tells people that he doesn't want money for it. He says stuff like that because he is used to being a victim, probably wants to be a victim, and probably because he gets better treatment as a victim from his experience. He would have done it for "power" (sympathy).
Not only that, but people can react to having the same cancer in different ways, so it's not like it's an indicator of a person lying, nor an indication of someone being a bad person or ungrateful or such. Obviously in Boogie's case he _is_ misleading/dishonest/manipulative, but I'm just saying that people's reactions are never proof of that so don't trick yourself into thinking the wrong things.
Although I suppose you could argue that he will still get more merch purchases and donations even when he says not to donate due to having cancer and that he's just being extra-sneaky, but the gains from that are probably like 10x less than if he didn't say that he doesn't want money, or if he had specifically asked for support, and the moral difference is quite low, too. So overall I'd say it's unlikely motive.

People claim being happier than ever after getting terminal cancer, which is very paradoxical and yet true for a lot of people. Others go into deep depression. Others are quite indifferent to it. The reaction to these things is so different among individuals, and cannot be used to confirm/deconfirm the cancer diagnosis.

I used to be a healthcare professional. Worked with cardiologists for 16 years. There is no way in hell a medical doctor, especially a PCP, is going to diagnose and treat any form of cancer without first having it confirmed. If a GP suspected that the patient could potentially have cancer, that patient would be referred to an oncologist and the oncologist, technically, becomes the PCP due to the diagnosis. Then that MD is going to refer the patient to other specialists, cardiologist, radiologist, he probably already has a pneumologist, and in his particular case, a hematologist. After all of those MD’s confirm the diagnosis with a plethora of tests, THEN treatment would begin. Depending on the severity of the cancer, a treatment plan is developed headed by the oncologist. If a GP diagnosed and treated a patient for any form of cancer without having confirmed the diagnosis, that MD should be sued for medical mal practice and they would lose their license.
Yeah, that's weird - did Boogie mention a specialist of any kind at ALL? Complicated patients only hang out with specialists, not GPs. That said, I have some autoimmune issues, and my symptoms don't quite fit into nice, neat categories. For instance, I have Sjogren's locked-in as a diagnosis, but SLE has always been more suspected than perfectly confirmed. Still, they throw SLE on the paperwork and monitor me with certain tests as if it's there, as it's better to be safe than sorry. My treatment plan is not affected either way.
I understand it must be different for something more serious like cancer, though. And regardless, I think Boogie's full of beans and needs to be off the internet. Even setting this current debacle aside, he's deeply vile.

You're right that you don't treat a cancer until it has been confirmed, but your description of the diagnostic process isn't entirely accurate. In 99% of cases you won't get referred to an oncologist until AFTER you have a tissue diagnosis of a cancer (ie biopsy confirmed) - otherwise imagine how many non-cancer things that may look like cancer would get referred to an oncologist, this would be a total waste of their time and the patient's time. By the time you arrive at the oncologists office you should already have a biopsy-confirmed cancer, although you may need further working up re staging etc depending on which other tests have been completed. There are some exceptions to this process but this is generally how it proceeds in the vast majority of cases.
Of course PCV is a haematological cancer so an oncologist wouldn't even be involved, it would be managed by a haematologist.

I used to be a cardiovascular sonographer and one of the cardiologist I used to work for would regularly evaluate patients from an oncologist/urologist that would perform robotic surgery on patients so I would have to perform carotid sonograms on those patients.
I just know that there is no way a patient would get any sort of treatment for any disease without it being confirmed.
I live in Puerto Rico so even though we have great physicians, we don’t have the same access the US does to many treatments and specialists and the protocol for diagnosing patients is to first confirm.

The thing I don't get.. and maybe I'm wrong... when I went through cancer I got tested and then got referred to my cancer doctor he's the one that put in for my bone marrow biopsy.
It was my cancer doctor that handled my medication and blood tests.. my primary doctor didn't have anything to do with it.. so how is boogies primary care doctor handling it as cancer when he should have a separate doctor to do that for him?
