      Dr Perlmutter, am I confused, but I thought uric acid is a result of eating carbohydrates and other plant products? Am I confused?
      It seems to me that the question is: what caused the inflammation and the excess of uric acid.
      Does he find giving insulin causes blood sugar to go down for a bit? How about giving other drugs that treat symptoms?
      Am I missing something or is this another treatment of symptoms instead of eliminating the cause?

      I can’t help but wonder if Dr Johnson would be interested in knowing that there’s a correlation between elevated blood sugar and diabetes?
      Maybe he would suggest lowering blood sugar to help diabetes! Maybe injecting insulin could lower blood sugar this lowering diabetes?

    As a physician who graduated from medical school years ago I happended upon this knowledge by accident in studying low carb/carnivore diets. This is life changing, for me and my patients. I cannot believe the studies have been around for quite awhile, yet are virually unknown. Dr. R. Johnson deserves a Nobel prize for his work. Doctors are taught nothing of nutrition in med school. Over the last two years, I have been on a quest to seek proper nutrition, metabolic health and the causes of the rampant Diabetes, HTN and CVD I see in almost every patient. This is likely THE fundamental link in the chain. Everything I was taught in med school was wrong. Now, I have found out that saturated fats are GOOD, PUFAs/Seed oils bad. Statins bad, Cholesterol good. Food pyramid...a sham and making us sick. Big pharma and the food industry has americans scammed. My journey began with the Low Carb Down Under youtube videos and Dr. Paul Mason. But this revelation on Uric Acid is the quintessential element to the basis of a new revelation on nutrition and health.

    My mother had kidney stones later in life; she checked herself out of the hospital after reading Eba Wayland's book, on nutrition. Not sure of the correct name; she lived in Europe and now I believe her work has been banned, along with many other early and present day researchers. Mom changed her diet and within months, cured herself, and did not need surgery! This was in the 1960s. She died at age 107 years by eating healthy!

      Hi Bella, Like Mr. C and GrainOfSand, I would like to know anything more you can tell us about Eba Wayland's book on nutrition.
      Incidentally, kidney stone sufferers should buy "Chanca Piedra". I had K.S.zzz about 6 years about when I was in my mid 50s. Terrible, terrible pain. Would rather have another baby...
      I was told about Chanca Piedra (means Claw in Spanish?) 3 years ago, and when ever I feel "that " pain, I take two immediately. No more pain!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!
      You can buy it at a health food store. Bottle says 500mg, vegan, non-gmo, gluten free.

      Bingo! Chanca Piedra has a direct effect on Uric Acid and is taken as a cheaper alternative to Uricase! See UA-cam "How This Herbal Supplement Prevents Kidney Stones"

    This is very eye opening for me. I take allopurinol and for high Uric acid . Even while losing weight they check my Uric acid. I knew sugar was bad for diabetes but didn't realize the relationship between Uric acid and insulin and insulin resistance.

      I use to take allopurinol. Than I started taking whole lemon 🍋 everyday some 6-7months ago. I'm 67 and can play tennis for 2-3 hours. I eat everything now and have no joint pain. I'm starting NMN from today.

    when I lived on the farm in the late sixties and 70s I worked hard and walked 6 miles daily to see a friend and back. and ate from the garden and had well water. I was not fat. I had no gout. we never ate anything artificial or had sprays on our fruit trees or in our vegetable gardens. I get up early as the sun rose and went to bed when the sun sat. i never had a need to have a curtain nor did we locked our doors. we had dogs cats and pet monkeys. I always had a deep sleep. never took any medication.
    when i moved out of home left the garden ate at fast foods and stop living off the land and i got gout. i gained 40 lbs. i developed acne. and i got a ton of cavities even though i go to the gym and walked still and brushed my teeth. it wad coming to the city that changed my life. and yes i started to drink beer and pop etc. then i was told to eat margarine and no salt.

      Dietary Guidelines for Gout Prevention and Treatment
      Avoid very high purine foods and limit high purine foods to not more than a serving per day (see chart below). Avoid meat extracts, broths, bouillon and gravy.
      Avoid rich, high-fat, meat-centered meals. Rely on plant foods as your primary sources of protein.
      Eat several servings of fiber-rich plant foods such as whole grains, vegetables, fruits, each day.
      Minimize intake of refined carbohydrates, including both starches (white flour products) and sugar. Concentrated fructose can increase uric acid levels and increase insulin resistance.
      Drink 2-3 L of fluids each day. Most of this should be water.
      Avoid alcohol, as it tends to interfere with uric acid excretion.
      Maintain a healthy body weight. If you are overweight, aim for a slow gradual weight loss of ½-1 kg (1-2 lbs) per week. Rapid weight loss from fasting or severely calorie-restricted diets is not recommended as this can raise uric acid levels and aggravate gout.
      Increase your physical activity. (Check with your doctor first if you are currently not active).

    CP Kelco Okmulgee Oklahoma a high fructose GMO corn syrup food additive processing factory, is directly responsible for obesity and diabetes epidemic in USA. Parent company is Monsanto, which owns Phramcia which makes drugs to treat diabetes and associated obesity conditions. A circle of human manufactured disease for profit. This is a crime against humanity.

    In 2016 I was diagnosed with osteoarthritis. Lab results showed elevated uric acid so my doctor gave me dietary instructions intended for people who suffer from gout. Basically, it was aimed at reducing intake of purines. Now, I am a meat eater and can hardly survive without meat. Otoh, I have already been on a reduced carbohydrates diet and close to zero refined sugars. No fructose syrup is used where I live. So I was directed to reduce meat intake. The food that was deemed safe was carbohydrates that I despised. In a rather short period of time I managed to reduce my uric acid by dietary measures to very low, but my rheumatic pains persisted. Now, it is very possible that I had high levels of uric acid for many years but didn't know it as it has never been measured. It is of note that I started suffering from digestion problems three years prior to developing osteoarthritis. These digestion problems were not taken seriously by the doctors who simply said it's irritable bowel syndrome. I was very puzzled and still am to this day as to what caused those digestion troubles as I didn't smoke, barely drank alcohol, avoided refined sugars and limited other carbs, ate sufficient fruits and veggies and was reasonably active. Possibly ate more protein and drank less water than necessary. So, I while I think that corn syrup is certainly bad and should be banned, it cannot be the only cause of elevated uric acid. There ought to be other factors at play.

    I was diagnosed with type 2 diabetes in 1998. My uric acid has NEVER been measured. One more failure of modern medicine.
    I have successfully been managing my T2D with very low carb/keto diets since diagnosed. No thanks to medicine again.
    I have been communicating with Dr. J. He is a sweetheart!

    I'm probably the biggest cheerleader for Dr Johnson, ever since I saw him on Mercola videos years ago. The reason I like his theory on obesity is because his is the only one that makes evolutionary sense.
    I have known the dangers of fructose and high uric acid for years, thanks to him. Dr Johnson is even more of an expert on fructose than Robert Lustig, and I wish I could nominate him for a Nobel prize...

      One of the things I hate about medicine is when they tell me something which doesn't seem to make sense with what we know about nature. I actually do think his thoughts on salt is one of those... since I don't see Bear's searching for high salt prior to the winter.

      @@CelineNoyce Dr Johnson says that high salt blood concentration is a problem, not just high total salt. He says that if you eat something salty to drink water to lower the concentration because high salt concentration can lead to obesity, but it's a much slower process.

    Thank you, doctors. I have tried to control my gout attacks over the past two decades by avoiding high-purine foods, with mixed success. Then three months ago my left hand suddenly swelled to three times normal size and I was forced to consult the professionals. My uric acid level was 8.7, glucose 245 (after fasting) and hemoglobin 4.6. So I submitted to a strict regime of prescribed pills and controlled diet, emphasizing liver-healthy foods. My readings last week were uric acid 4.9, glucose 108 (after not fasting) and hemoglobin 4.1 -- along with losing ten pounds. So it is amazing what improvements can be made in a short period of time with a combination of the right medications and dietary discipline. Your book Drop Acid (which I got last Fall) helped lay the groundwork of understanding what needed to be done to restore my good health. Thank you both for your lives' work.

      Thank you for the good information. Did you follow the diet in drop acid book? I kind of have the same problem is that I'm not a drinker and I don't eat fast food and I have sugar on occasion. But I believe glucose levels due to eating bread pasta foods that elevate those levels are contributing to those higher uric acid levels. Did you find that to be true with specific foods that are not necessarily bad carbohydrates like pasta and bread?

Yes, I was totally out of control at Christmas time. Do you think I could go into solitary confinement next December?

      @@intrinsicfactor5425 I'm far from being a professional but there is something wrong with what you're saying. Keto for 4 years and all my blood work was fine. Twice as much pink Himalayan salt as potassium, magnesium plus a fair amount of calcium rich food. 110/70 is my norm. No swollen ankle anymore. No panic attacks. No more neuropathy. Something is going right for this 66 year old.

      @@nancyfahey7518 As an addendum: most people are unaware the RDA for potassium was very quietly cut in greater than half from 5700mg to 2300mg. Optimal sodium range studies still place longevity between 4 to 6 grams (4000mg-6000mg). For whatever that is worth to you. 😉

      Your blood sugar was probably more stable. Early in insulin resistance one actually becomes hypoglycemic.

      Vegan junk foods are the same as ultra-processed foods! Eat only Whole Foods plant-based to preserve health.
      Many scientists and doctors have performed numerous studies to support the benefits of a WFPB diet for various chronic diseases, including Type 2 diabetes and coronary artery disease.

      whole food plant-based (WFPB) diet is an excellent choice to help with gout. Uric acid from purine breakdown is helped by a WFPB diet because plants have MUCH lower levels of purines.

    This is absolutely a revelation. In particular... the information / theory about how your natural instinct to survive could be making you absent minded... which then becomes dementia. Of course. that does make sense. I have heard Dr Johnson before but too often the interviewer will not allow him to just spew his knowledge... you did, great info resulted.

    There appears to be "fructose" added to just about all the packaged food products we have been using for years. Even the packaged high "protein" Beef Jerkey product we have considered an energy booster is flavored with "fructose".

      I'm severely allergic to corn. Normally I can taste it. But if it's well hidden I feel it in my bones and joints within a half hour. Yes, it's in everything. Corn syrup.

    Oh my, I had a gout attack without knowing. My joints hurt constantly. Also blood pressure is higher. Listening carefully

    This was very informative and helpful as a life long gout sufferer. I hardly ever drink, exercise all the time but have struggled with fructose. I need a daily or weekly test kit to help stay dialed in. I am severely restricting Carbs and cutting fructose and alcohol out entirely. I shall see. ? Thank You Dr. Perlmutter

      Also lower salt intake. Dr. Johnson's research indicates excessive salt converts to fructose.

      Salt can often appear to be a problem when potassium and or magnesium are deficient. It’s not always the actual salt, but rather the imbalance of importer ratios. Think sodium potassium pump.

    A few questions, which brand Uric Acid home self test is best. How long did it take for the Uric Acid to go down when you stopped eating sardines (I also have been eating them every day). How much Quercetin to take and should you take it with anything else? Thanks

    This is awesome. I had gout a few years ago. I have lost 45 pounds since then and stopped drinking beer. No more gout. In trying to lower my purines I also lowered my fructose consumption. Funny thing is, when I got gout, my uric acid was not THAT high, it was around 6. Maybe it was the fructose.

    Most excellent information, sorely needed and very understandable to the layman.
    The sugar uric acid relationship to insulin resistance is plain to see and is easy to fix, STOP eating carbohydrates and fructose and the problem will fix itself!

      You don't need to replace either. Your body does not require carbohydrates to exist and fructose is sugar. Sugar is toxic to our bodies.
      If we were able to look back in time, we'd find a time when humans started experimenting with our fruits and criss-breeding, e.t.c. to make them more full of sugar than when God created them.
      The banana has been modified so much its ridiculous - and then they turn around and say ... eat bananas. They have potassium. I think it's around 343MG. Our bodies need 4700MG of potassium daily. One current day banana is a small dent in that big number, and jacks our blood sugars levels up considerably.

      I have been keto a year still having gout flare ups. Finally went on 300 mg of allpurinol. My number 6.7 in Jan and won’t be tested again till next Jan. But it had come down from 7.7. I keep my carbs at around 20 a day and eat no sugar .

    very interesting. Thankfully, scientists are developing humane alternatives to animal tests. New non-animal methods are quickly and regularly becoming available. This doesn't mean halting medical progress. Instead, replacing animals and using non-animal testing methods is aimed at improving the test quality *as well as the humanity of our science*. Most people are against animal cruelty but think there's no other choice. If we can live happy healthy lives without harming others, why wouldn't we? Best to you,

    Always had high BP (had an atrial.fibrillation issue in May probably due to stopping meds), but now back.on.meds and having a big toe joint pain. Thanks for this great advice. Will def try to reduce this uric acid. Much appreciated.

    I've been high protein plant based for 7 years and I'm here to find out if there's such thing as too low? My last test came to 1.8mg/DL from 2.5mg/DL previously, which is apparently borderline the range. My doctor didn't say there was anything of concern on my blood and urine test but online tells me low uric acid could be a genetic disorder but I'm very active and haven't noticed any issues.

      These are some natural solutions that can help you with gout:
      1. It turns out that baking soda is an excellent remedy in cases of gout. Create a mixture that contains half a glass of water and ½ teaspoon of baking soda. Drink it every day. In case you are dealing with gout attacks, you can use one teaspoon in this solution.
      2. If you keep your body’s environment alkalized, you will be able to protect it from the negative effects of gout. That’s why you must increase the intake of alkaline drinks and foods. One good example of a drink like this is lemon juice. It can prevent the buildup of uric acid crystals. Take half a lemon and squeeze it in one glass of lukewarm water. Drink this beverage in the morning before breakfast.
      3. In addition, apple cider vinegar is a type of vinegar that can boost the alkalinity in your body mostly because of the fact that it contains malic acid.     my uric acid has been lowered it had no effect on my blood pressure at all,i have 3 doctors on this did have gout it has taken them 2 years too get gout down did all they said to do my uric acid was around 6 i do not consume alcohol at all for over 25 years at all !!!!! what do you say about this ?

      Dietary Guidelines for Gout Prevention and Treatment
      Avoid very high purine foods and limit high purine foods to not more than a serving per day (see chart below). Avoid meat extracts, broths, bouillon and gravy.
      Avoid rich, high-fat, meat-centered meals. Rely on plant foods as your primary sources of protein.
      Eat several servings of fiber-rich plant foods such as whole grains, vegetables, fruits, each day.
      Minimize intake of refined carbohydrates, including both starches (white flour products) and sugar. Concentrated fructose can increase uric acid levels and increase insulin resistance.
      Drink 2-3 L of fluids each day. Most of this should be water.
      Avoid alcohol, as it tends to interfere with uric acid excretion.

    Vitamin D deficiency
    can activate the parathyroid to induce the release of parathyroid hormone, which was thought to increase serum uric acid level, and low vitamin D status may also be associated with risk of CVD.
    working outside as a kid I got a lot of sun everyday. I also built a ton of muscles. we all should live off the land if possible and stay away from watching t.v. all day like a lot of people do . if you go to bed 2 hours earlier than normal I think that your uric acid will be lowered. we do not live long because we live in the city. our life has changed so much in the last 40 years.

      I normally suffer from acute gout attacks on a background of type 2. Diabetes, I don't have high BP , sugar is controlled, what diet should I eat and what should I do to keep the uric acid at bay.

    Hi, doc.. I suffering from gout attack every 2 weeks. My doctor told just to drink my meds like colchicine once a day. But i dont want to drink meds anymore, or make it less..what is the best way to make my uric acid lower..
    And watching your video i supposed that your right that diabetes is also bad for gout sufferers.
    Is the key here only fruits?
    How about vitamins C.
    Can barley grass helps, without taking colchicine?

    So my takeaway is sugar, fructose, yeast in beer raise uric acid. Uric acid contributes to obesity, high blood pressure, high blood sugar, gout, and metabolic syndrome. The cure was not mentioned other than dont eat fructose, sugar, drink beer. But I know tart cherry juice or capsules is one treatment. Tks for the info.

      @Taiwo Omotosho R.W. Knudsen makes an organic tart cherry juice with no added sugar. Trader Joe’s also carries tart cherry juice that has no added sugar. I have seen the juices of which you speak. They are not the juices once should drink for gout or anything else for that matter.

    Your research must be true..
    I had cold hands, feets , ears and nose,,,high blood pressure..weakness, and. i went to the lab the next morning surprise?? Uric acid at 95% of the range , and I felt already better at that time.
    Probably it was even higher at nighttime, when the crisis was at its peak.
    It is related with high CK (Creatine Kinase, probably from my legs, from a hike), maybe it is insuline resistance of the sceletal muscles, in combination with high lactic acid due to the blocking of eNOS in arteries and capillaries , and as a result of the muscle has broken down glutamine for fuel.
    Thank you, to bring my attention to uric acid!!

      @nigwasme So I got rid of the evil gout in two days (well the worst of it). Drank loads of water with a pinch of baking soda in it. I have read that making tea out of blackcurrant leaves is good for removing uric acid. Also soaking feet in a brew of the leaves. Plenty of that on the farm so we will try to incorporate that in my future days. I figure I had been working hard for two straight days with only a coffee in the morning for hydration. That probably set the thing off!

      Okay, so the black currant leaves are a go. Love the tea. I take 12 leaves, wash them and break them up. Put them into a two cup mug. Pour over boiling water and let it sit until cool. This has helped my aching feet and toes a lot. If I feel stiff and sore in my body, this tea helps a lot. Looking at adding devils claw. Also vitamin C and msm. I am never taking pharmaceutical help. That is is a downward spiral.

      @@lynlawley8903 Easier if you dry them. Apparently, nettles and holy basil also help. I also had very high blood pressure so drink a tea of celery seeds and juniper berries. Blood pressure is down to normal. Celery seeds also help with gout.

    I have Stage 5 CKD. If I have sugar in the form of cookies, cake, soda!, Etc. I am guaranteed to have a gout attack. I am normal weight with normal blood pressure but tend to have high uric acid.

    A great addition would be differentiating between the gout risks from an animal protein diet vs. the absent risk for gout from consuming plant protein (e.g. chick pea-based produce).

      It is probably purines they talked about, that you are referring to. And this is basically the amount of cells and nucleic acid which can cause uric acid to go up. Plant protein isolates have few nucleic acids, but so do some animal protein sources like chicken breasts.

      Of course there's always the issue of inflammatory potential inherent to the diet, on top of the uric acid level impact, which may then call into question whether a diet incorporating animal protein wouldn't end up being challenged to match those very low inflammatory potential levels commonly found in a gluten-free organic (and ideally fresh) plant-based diet.

    in January/22 at the end of my 14 days total water fast my Uric Acid went up from 340 to 743. After a month stabilized at 260-300 range. Then, at the end of my 7 days water fast, Uric Acid went up again to 614. Now, with OMAD, it stays at 356. Any comments? Thx.

      69 y.o. generally healthy, got rid of fat in the liver and pancreas + got rid of IR, lost weight to healthy BMI, no HBP (never), no meds (never), healthy diet OMAD. NMR: High LDL-C large buoyant pattern noticed especially at the end of my prolong total water fast. It looks like LDL-C and Uric Acid go in the same direction during my prolong total water fast. Just finished my 5 days total water fast before Christmas. Will check blood LDL-C and Uric Acid after New Year. I feel very healthy. Big improvement in body strength over last 2 yrs of improved life style. Should I be concerned about Uric Acid spikes - good/bad/indifferent?     What is the difference between high uric acid, and high BUN (blood urea nitrogen)?

      Uric acid is formed from the breakdown of purines, some food stuff high in purines could be beer, peas, asparagus, liver and sweet breads.
      BUN is a waste product produced by the breakdown of proteins thusly called urea nitrogen.

      Serum uric acid was significantly correlated with blood urea nitrogen and serum creatinine. Groups with a higher serum uric acid level had an increased risk of impaired renal function.
      Urea is the major byproduct of ammonia metabolism in vertebrate animals. Uric acid is the major byproduct of ammonia metabolism in birds, terrestrial arthropods, and reptiles.

    True we are related to primates so essentially we should be eating a diet specific to our species. Health is all about basic chemistry. There are only two sides to chemistry, acid and alkaline/base. Acids burn (cause inflammation) All of our body processes are acidic. This means we need a diet loaded with alkaline minerals (fruit and greens) to balance out all that acidity otherwise we create a massive imbalance. If we do that over several decades we build massive amounts of acids in the body which need to come out or they will sit and burn our cells. We were not designed to eat the diet that most people do today therefore the body is working hard to buffer all the acidity in the body and to eliminate it.
    Have you done any studies on alkaline diet?

      We are related to other primates. Some like chimps eat meat, some like gorillas do not. Our diet should be almost entirely meat. I stopped eating any plant material a couple of years ago and almost all my health issues resolved, I take no medication and blood pressure is normal, now I weigh what I did at the end of high school.

      @@Ron_the_Skeptic glad your diet has worked for you but it still doesn't get away from the fact that apes are mainly frugivores. Ask Google and it will tell you that only 3% of chimps diet comes from meat. Long term you may find you do not do well without fibre

    I was doing a detox. My uric acid went up. I got gout in my thumbs which was painful. I had kidney pain and said aaahah it’s. So I started on uric acid pills and drinking a lot of lemon water. Than my kidney felt better but peeing hurt. I’m guessing I had crystals in my urine. I’m 60 have arthritis and I can’t loose weight even on just eating 1 cup of food a day. I’m guessing I’ve had an uric acid and gout my whole life but no dr diagnosed. My brothers autopsy showed gout. My 2 brothers and father died at 50 and 52 of heart attacks.

      I was diagnosed with gout after a flu shot in 2020. I've also had kidney pain and pain when urinating. Dr. treated me for a UTI. I get it every few months. I was given a free medical reading for Christmas 2023 by a psychic. She informed me that I had crystals in my right kidney. She asked if I had gout! It explained everything! Now I'm going to talk to my doctor the end of the month!

    How about when high uric acid is from CKD (pre liver xplant HRS) despite a good diet? When you eat low carb, and no fructose, would you still describe uric acid pathology the same way, for a UA level of 10?

    Doesn't narrowing of the arteries cause hypertension? Maybe there are different hypertensions and uric acid raising blood pressure is another one.
    Uric Acid also causes problems with erectile dysfunction/ nitric oxide so there is a relation to the blood flow, the same as the narrowing arteries near the heart.

    My uric acid level was 10.4mg/dl a month ago. But after taking febuxostat it dropped to 6.2mg/dl. I am still feeling pain in my joints. The pain is migratory moving from my leg to fingers. For how long should I take febuxostat tablets because my uric acid levels are normal.

    My blood labs shows low end of reference range for Uric acid as 3.8 mg/dL. I'm 67. my recent uric acid was 4.0 mg/dL, not too bad. My daughter, 27, has a report with a reference range low end of 2.8 mg/dL. So, it seems that reference range is based, once again, on population data rather than healthy range.
