Have a sprayed or butter parchment/waxed paper lined 8x8 inch pan ready to pour and spread fudge with the back of your spoon (is best to press into pan). Chill in fridge until hard enough to cut. This is delicious!! It comes out about 1/2 inch thick or better. Enjoy!

@MissySancha milk that has been heated in a process to "evaporate" the water out of it which makes the milk thicker and denser than regular milk. evaporated milk has a high vitamin content and high calorie. and thats evaporated milk.
@MissySancha i am not 100% sure but it's almost like cream but normally comes in a can. I don't know the exact difference, but my dad always seams to be drinking it out of the an around Christmas time :D

@xdaysx Sticky Fudge is old-fashioned fudge. It certainly isn't a brownie or anything, that's why fudge syrup is so thick on ice creams and such. You can put it in the fridge to harden it a bit and it taastes great.

Tried this. Yeah, it didn't firm up proper. Not sure why. I didn't have evaporated milk, so I just used the normal 2% at double the amount, and judged based on thickness. Looks rather similar to yours, though the boiling down, and the pan size left me with a thin sheet of it.
When candy turns out gritty, it is (I believe) crystalized sugar. There are two pieces of advice given to avoid this. First, make sure there are no sugar crystals stuck to the sides of the pan, as they will be the seeds for more crystals. Second, once the sugar has melted, do not stir the mixture anymore, as this can trigger crystals. Many recipes also use something to interfere with crystals, like corn syrup or lemon juice. I'd expect the butter and cocoa to play that part here, though.

Jack, I love your show, I've gotten some good ideas, it is funny, and I look forward to your new videos on Fridays...but that was some nasty fudge. It looked like ice cream sauce. There was too much sugar and not enough heat. I've never been a fan of timing the boil of candy. You have to use a candy thermometer. I just made some peanut butter fudge for Christmas and I think it needed to be about 235 deg. It turned out awesome. Firm, creamy, and not too sweet.

oh i have a good tip for making fudge, never scrape down the sides (as hard as it may be) because it'll make it grainy ^^
