The foreskin is fused to the glans (like a fingernail to the nail bed) and separates naturally over time anywhere from age 3 to 18, but usually around puberty. Telling parents to pull it back during diaper changes is dangerous as it's most likely still fused to the glans and prematurely pulling it back will do more harm than good. The owner of the penis is the first person who should retract it. Not a doctor, not a nurse, not the parent. During diaper changes, clean like a finger: only wipe what is seen.
