It's crazy how similar this, at least a specific part/angle of it, is to an experience I actually lived through. My grandfather had this illegitimate child, which came from the time he was in the army and fooling around with this girl, which strangely enough, he told several of us grandkids about the "crazy woman who whored around the barracks and eventually we got her a bus ticket just to get her out of our hair when she started accusing us of knocking her up"
And it never occurred to ANY of us grandkids, that our grandmother didn't ever hear of this story. Even more wild.. That child he conceived, when he died, he left EVERYTHING he could to that kid that he'd never even fucking met. Left us with nothing. The family that he'd lived every single hour of every day with for, well, fuck, the entirety of my life, like 45+ yrs of my grandmother's, and he signed papers to make that happen basically right next to her. If she'd just glanced over, she would've realized what he was planning to do the entire time.
Extra fucked up, because the entire time he was with my grandmother, he'd constantly tell her she shouldn't work and how everything will be taken care of etc etc.. and if she hadn't, we'd literally be fucking on the street with nowhere to go right now.

Eldis got it right I think. That man needs to reach out and get clarity because it definitely feels like he was considering doing so for years and now with the woman dying he probably feels an immense urgency but is too scared to do anything about it.
If it's this important to him then he'll fuck up his and other's people's lives by not doing anything about it just as he might when barging in. At least when barging in he's got a coin flip chance that it does work out to something positive in the long-term while if he keeps sitting on his hands and shuts his mouth it'll not resolve the issue for him personally.
Man up dude, find out and if it's your kid give them the chance to make a decision on what they want.
By the way, strong start to a marriage that you didn't tell your partner you might have a kid somewhere.

Coming to this from the perspective of a father and also a son who was abandoned by a father, I think the move is to send her a friend request and send her a message that says “I’m really sorry to hear about your diagnosis. I don’t want to take up any of your time or energy right now, but I’ve always wondered if I contributed any DNA to your son and I’m putting the offer out there to do a paternity test at any future point if you or he desires. I have my own life now, but I would be happy to support my son if he was ever in need of it. I wish you the best.” But you have to clear it with the fiancé first. I think starting with the Mom by simply reaching out to say,
"Hey, I just wanted to let you know that I've thought about you from time to time over the years. I recently creeped on you through Facebook and saw that you were in a really difficult place. I wish there was a way to make such a dire circumstance easier to bear. Knowing that things must seem so inescapable, I would at least like to wish you well and hope you're able to attain the best state of mind that is possible for the incomprehensible challenges you must be faced with. I was really saddened and troubled by the unnecessary cruelty that fate can bestow on us. I really hope you're able to find some solace or peace, given the circumstances."
Something to this effect could at least open a door to gaining some insight about how she may feel about him, what her son might be like, and whether this guy should make one mention of any of it to his fiance.
I think he needs a lot more concrete intel before blowing everyone's world up.
Reaching out to a dying woman (who he cared enough about to not forget) and offer some condolences will never be a bad move.

Btw, my dad has black hair. Having a red headed kid isn't out of the realm of possibility.
But this dude should tell his fiance about the situation and tell her he never knew if it was his but he's concerned about not helping with his responsibility and he wants to ask for a DNA test to know one way or the other. He'll look responsible, but if this girl is cf it could go either way. I'm cf and I can't guarantee I wouldn't leave. Lol I would probably at least put the wedding on hold. He should see about just sending money to a family member of the mother, that is willing to take care of him if he is the father.
He obviously didn't want any responsibility of it, but is feeling guilty and concerned because he thought the kid had his mom so he'd be fine. But doesn't want his kid to struggle so hard in life that he has no family or someone to take care of him. I would feel that way too if I was a man and the woman I knocked up was dying. Plus he made a child. It's his responsibility to make sure he has what he needs to survive. Even if he doesn't want to extend any physical support. You could offer financial support and if they say no move on. Definitely not offering anything they don't want. It's not like he has to adopt the kid just because he does a paternity test.

Almost this exact thing happened to us. It's not easy and most marriages would not survive this. But as his father, there is only one thing you can do. I already have a daughter, and found out I had a son that was 18 out of the blue. He was not doing great with his mom.
It's not going to be easy, but I love him as much as my daughter pretty much from the first time I saw him. There's nothing you can do, he's your kid.
We took him in immediately, and he's been living with my family for years. My daughter absolutely adores him, my wife is a bit more down to earth but they get along.
Most marriages would not survive this, but that's ok I think. Your sons wellbeing should be more important, and a good wife will understand and do the right thing.
I hope you read this and figure it out, the kid needs you.

Lots of variables here as always, but if i were to give my suggestions it would be:
    1. Talk to your fiance. If she shuts it down and youre unwilling to give up the relationship with her, you have your answer.
    2. If after 1, youre still on board, reach out via text/email to the mom, putting the ball entirely in her court as to whether she wants to bring you into her son's life. If she refuses, there is your answer.
    3. If youre not going to be a POSITIVE influence in his life, youre not needed. The right thing to do here is communicate with the women who are invloved in this, and be involved to the level they require.
    4. If everyone says no, then walk away. Years later can reach out to him directly, but then know it is 100% for you.

I practically knew nothing about my dad, you could probably count how many words we'd exchanged until I was 17 years old. I went through some pretty insane, bordering on traumatic shit.. And he was the only sane and responsible person around I could talk to really. And it's been a trip since then, to realize just how much we have in common. Like, shit that may actually be evidence of nature over nurture. Some things are trivial, or at least up to random chance, like our taste in women, or food preferences as well as dislikes, political and spiritual beliefs etc.. But it's not like your parent's favorite color is going to determine yours obviously.
But on the other hand, there's a lot of sort of things like speaking mannerisms or personality quirks we share, as well as my grandfather on that side/his father, despite the fact neither of them were ever around for me to pick up and imitate that, yet it occurred naturally anyway, completely independently. I have no explanation for that other than it being something on some kind of genetic level that manifested.
Also just to throw my two cents out there, I put a LOT of stock into one of the things one of the guys said.. About, eventually that kid is going to wonder and want to know. Even if it's for the worst, still, at least then he'd know.
I think even a sort of bad answer/result is preferable to a complete unknown, and letting your mind endlessly wander, since you're going to consider those worst case scenario possibilities, anyway. It's the same thing as closure for families who've had missing relatives and whatnot for years, they're still relieved to hear "Oh, it's official. We found their bones. They're dead, they were murdered." opposed to wondering if they're alive somewhere out there and suffering, or questioning why etc.
