Mainly the video is inspired by the film "The Color of the Pomegranate" by director Sergey Paradyanov, in the gaga video you can see many references to this film, for example the grenades that appear in the sand at the beginning of the video, or many other planes such as the man kneeling with the cushion, or the lady gaga in this kind of colorful window. References are also taken from other films such as Federico Fellini’s 8½. (when Gaga is elevated and restrained with a rope not to die) there are also references to Frida Kahlo.

My opinion, whether the "dream sequence" was caused by drugs (or lack of drugs, olanzapine), or the accident, she was dead until they restarted her heart. She was on the way to the afterlife when they pulled her back down to earth and saved her.
