    Can I Ask You A Question? SO THE ULTRASOUND CAN REALLY DETECT TORSION?!?

      doppler ultrasound can detect blood flow. the only thing that helps blood flow into the testicles is the spermatic cord which is exactly why having no blood flow means that the spermatic cord is twisted and no blood flow is allowed to your affected testicle - Testicular torsion.

      I m from India. My left tactical spermatic cord abnormal extra long than normal due to tactical injury. That means to say spermatic cord is very longer my color Doppler ultrasound normal.
      What is my problem? Please answer

    Cool video! When you manual detorsion the testes, how do you know which direction to turn ? ie clockwise vs counterclockwise ?

      Unfortunately you DON'T know for sure which way to turn. 2/3 of the time the torsion is in the medial direction, which is why you "open the book" to detorse. But 1/3 of the time you will be wrong (PMID 12544339). Safest to do in the OR. If you need to attempt bedside then ultrasound can help. -Jess

      +EMRAP Productions thanks for the reply. Is it possible to make it worse by turning it in that wrong direction or can the testes take one more twist ? 😀

      I'm guessing few men would sign up for the RCT, and an extra twist in the wrong direction may cause harm. But this is a risk-benefit situation when you don't have a timely surgical option.
