Once is a mistake, twice is a bad habit.
That was the philosophy at the japanese companies I worked in, in the 80's. Mistakes are allowed, but only ONCE.
If we only pay attention (and punish) when an incident happens, will employees speak up and tell us how work is REALLY getting done, or will they hide those things? Wouldn't we rather know so we can improve BEFORE an incident occurs?
