I have been Keto for three years and off diabetes meds for almost three years. I have totally taken control of my diabetes via food not meds. I am not cured but I am in control. My system was or is too broken. I have hope that it may improve the longer I go. I can’t and don’t cheat... there is no option for me. I have to maintain the lowest carbs I can. I would rather do this then take meds.
Hi! Have you tried alternate day fasting (ADF)? If you have skyhigh insulin resistance, doing keto alone might not decrease basal insulin levels enough. I am a T1D so I can see the exact basal insulin levels :) ADF is miracolous for lowering basal insulin. I tried it for 6 weeks and I reduced basal insulin. It still remains, 3 months later. I could not achieve that on keto alone.
I have been on the ketogenic diet for 8 months. I lost 60 lbs in the first seven months and attained my ideal body weight. It's best to combine the ketogenic diet with intermittent fasting. I do one meal a day (23-24 hr fast) five days a week and two meals a day (an 18 hr fast) on the weekends. I eat to satiety. My triglycerides went from 400 to 68 and my HDL cholesterol went from 20 to mid 30s. I expect the HDL cholesterol to improve further with time. Good video. Keep up the good work.
I have been full circle on eating habits, diets etc. I have never been overweight just always looked for a healthy way to eat. I am still very athletic at 77 and don't get sick. No meds. I have found what works best for me is IF. I eat everything but only once or twice a day. Sometimes fast for 24 to 48 hours. I'm convinced fasting is the answer no matter what you eat. Our bodies need the chance to repair. Sounds like you have found what works best for you.
It took me two years to get my A1c from 9.8% to 5.1%. For me, it was simple but not easy. I just progressively reduce my carb intake and I'm not talking about the vegetables. The hardest carbs for me to give up were breads and pasta (grains). My total daily carbs now are generally 50g or less and I'm no longer taking insulin or Metformin. I'm no longer "diabetic" enough for my insurance company to pay for a CGM which I cannot afford but would be hugely beneficial.

With respect to medications when switching to a low carb diet (under 5% of calories), your mother needs to consult with a doctor to carefully control how the bloodsugar and ketones vary. This is done by bloodtests, or a device she can attach to her backside of the upper arm that connects to the phone via app and constantly monitors blood levels. From these levels, the doctor can advise how much to cut back on medication. As stated by Dr. Phinney here, it can be dangerous to change diets without consulting with your doctor if you are on medication. Can't stress that enough. The challenge though, is finding a doctor that does not spout the classic food pyramid and high carb diabetes diet. Good luck!

So many people are still chasing elevated ketones with more dietary fat than they need because of this outdated chart! ‘Fat to satiety’ works because it gets people out of the fat+carb hyperpalatable zone, but fat is not satiating when removed from protein. The benefits seen can be explained by reduced appetite, but we now have a better understanding of the various factors that drive satiety that can enable people to achieve better long-term outcomes in terms of fat loss in the long term after glucose stabilises initially. People with better metabolic health tend to have lower BHB values even if they are on a ketogenic diet. Pre-release two-year Virta trial results can be found here showing that BHB at one and two years is 0.3 mmol/L vs baseline 0.2 mmol/L (i.e. not greater than 0.5 mmol/L and not ketogenic based on their own definition.). With MCT oil all of the longer chain triglycerides are removed from the coconut oil do you get more ketones. Again, no flavor.
My last blood test reported my A1c as 6.1, down from 6.7, which is pretty good. However, I had asked my doctor to also measure my insulin, something not normally measured. It came in at 20! I am hypothesizing that my issue with stubborn weight is due to too much insulin, both endogenous and exogenous. I decided to switch to carnivore as an experiment. I’ve been eating carnivore for about six weeks now and I’m only taking insulin if my blood sugar is more than 170. My blood sugar seems to be stabilized between 160 and 180. Very high. If I’m not mistaken, this will translate to an HbA1c between 7.2 and 7.9. If I cheat and eat some carbs, only if my body seems to be craving them (I don’t know why), my blood sugar sometimes climbs to just over 200 and I will take up to 20 units if insulin. Sometimes this will lower my sugar to under 80. But it doesn’t stay there. I’m going to ride this out until my next A1c and insulin test. My A1c has been much higher in the past so for the sake of learning I’ll risk it. I’ve had very slow and inconsistent weight lose so far. My weight hadn’t gone down for a couple of weeks when I have into a craving and ate a couple pieces of low-carb (22) toast with peanut butter and butter and the next morning my weight has dropped more than 3lbs! Is that what’s called a “confounding variable”?
Personally, I just can't maintain these "ratios". I did Keto for about 5 months (not much weight to lose by the way), and ended up eating way more protein than I ever did before because I was not satiated otherwise. Still stayed in ketosis though (regular blood measuring). But I just didn't feel so great after a while. At first yes, but then my body got weak and my workouts got harder. I started eating more carbs (God-made carbs, not sugar or flour), and feel much better. I still will measure ketones of up to 0.9 after a 15-18 hour fast. Feel better this way!
you're supposed to eat high fat , not protein. I've been doing keto for years and I only never get full for long unless i eat lots of fat. keep the protein lower , fat much higher

If you are very insulin resistant (as I suspect you are by still being diabetic on a keto diet), what research has found (See Ekberg etc), it may require you to go on extended fasts. By that I mean starting out 24 hr between meals, then working your way over weeks/months to get a few 5 day fasts where you drink nothing but water and add some necessary salt during the day. Light walking helps this process too. Do not do any kind of high to moderate intensity stuff when going for a 4-5 day fast due to dangers of muscle loss. Only light, as with a 20 min walk daily. You may need to do this for quite some time, 6 months to a year, where you squeeze in a 3-5 day fast maybe every 2-3 weeks. And the rest of the time stick to an eating window of about 4-8 hours, and thereafter no caloric intake (not even fluids with calories), safest is water only when not eating. I wish you luck. problem is grains do need some processing to be edible. eating wheat fully unprocessed is not good for you or edible. yes pesticides make things worse but wheat is marching military fodder, not a food of choice. also modern wheat contains gluten which is harmful to your gut. look up "leaky gut "
People have beem getting obesity and type 2 diabetes since at least ancient Greek times. So for humans, carbohydrate-intolerance has been around long before pesticides. Although, that doesn't mean pesticides don't do bad things to the body.
Gluten in ancient wheat was way lower. Plus modern threshers did not allow the wheat to rest in the field. Then our modern lifestyle had us impatient to wait on starter cultures.
Phinney, like others, tells us what our macro nutrient intake should be "if we have weight to lose" but never seem to address those who are already thin. I have been on the ketogenic diet for 4 months. All of my markers have improved significantly and I am off my diabetic meds, which is great. But I have also lost 18 lbs and my BMI is now 19. I am skinny as a rake, always have been thin, and really don't want to lose more weight. Am I forced to choose between diabetes control and healthy weight? I wish someone like Phinney would mention how the diet might be adjusted for people like me.

As far as I know fasting insulin could be used for that. Unfortunately the test's cost is over $100 and a lot of times insurance companies do not cover it.
You do not need a blood test to determine if you are insulin resistant. If you are overweight, it is because you are, or that you have prediabetes. Tim Noakes had diabetes even though he was training him self slim, so if you exercise a lot you may have insulin resistance even though you are not overweight. But if you are overweight you are insulin resistant.
Every time I try the ketogenic or carnivore diets, I end up succumbing to carbs, such as ice cream with berries, to restore my energy. What's the solution?
My experience, after being in constant Ketosis for 4 months, is that I haven’t been as energetic or mentally alert in many years. Just say NO to carbs and stay on track.
There are a number of keto ice cream recipes that are very simple to make and have very low carbs. My wife makes a batch that lasts weeks. Google Gnom Gnom for the recipies.
Willpower. When you surpass the 2 weeks barrier, the carb cravings disappear. And help yourself, if you are constantly on a situation where there's carb available, you may end up cheating, and cheating sets you back to zero mark. Find some keto snacks you'd like and have them around. When the cravings come, have a keto snack. Cheers

The frequently referenced range for being in ketosis is between 20-50 grams of NET carbs. If there are "added sugars" listed on the product level you can roughly deduct half of those added sugar carbs to further lower the net carbs. The reason is that depending on what mix of artificial sugars they add to the food, the body is not able to absorb half of those carbs.

What does "reversing" really mean? Does it mean they just don't need to medicate? does it mean their blood sugar levels are in the normal range? or does it mean that their glucose levels are normal even after eating at breadstick and a pasta dinner at the olive garden like someone that is not diabetic? in 10:30 he talks about it but I don't quite understand.
My interpretation is that one can normalize their blood sugar and eliminate symptoms but if they begin eating carbs again, they quickly regain insulin resistance and metabolic disease.
It means that people who get diabetes T2 are carb intolerant. So you can either eat carbs and have diabetes or eat no carbs and be healthy. If you eat a reasonable amount of carbs with intermittent fasting so that your glucose and insulin can come down, you can consider it a reversal with no or less medication. If you think you can go back to binge on carbs then you are going back to what caused the disease in first place.
I am so miffed I am type 2 . Strict keto for 2 years. I lowered my carbs to leafy greens (under 20) yet my A1C went from 5.0 to 5.9. Blood tested in ketosis above 0.5 usually 0.8 sometimes 2.
6.4 or less is still successful reversal of type 2 diabetes. You can try Metformin if you can handle GI side effects as it has longevity health benifits according to this doctor in other videos.

I am a type 2 with normal A1c and no medications. I follow a strict keto with very little carbs. Everything I eat is organic. Lots of leafy greens, eggs ,cheese and lard. I am a bit confused with this study since he adjusted the meds according to the blood sugar but never stopped them. What is he proving by restricting carbs and still using medications? If his patients used vegetable oils, and consumed fat from non organic source, they were eating lypotoxins from sick animals GMO corn fed in cages etc. Half of the battle in Diabetes is to address chronic inflammation caused by linoleic acid and hyperinsulinemia. The carbs are just one part of the puzzle. There are plenty of studies showing the same results with high carbs low fat and protein diets. Each individual metabolizes carbs differently this is why these clinical trials are a waste of time. the truth is once you get sick you will never be able to eat both carbs and animal protein and fat at the same time for the rest of your life. No more pizza or burgers with fries ever. You have to first understand what causes Insulin resistance at the cellular level. No one knows.. Phinney is 72. Alot of the visible effects of aging is sun related. He does a lot of biking and outdoor activity. I have a friend who lived in Arizona for 30 years, fit and outdoors all the time and at 58 she is more wrinkly than my 75 year old mom who covered herself from the sun when she did go out (for a walk, no 3 weeks climbing the Andes and whitewater rafting!). If u live in n a dry place, that takes its toll on the skin too. My friend moved to the humidity of ny and she said people look a lot younger. But still agree. He looks kind of old. Another friend of mine, vegan, looks like he is 100 but he just turned 70...and he's looked about 100 for the past 20 years....wrinkly and saggy. Very fit otherwise. Thin and gaunt looking tho
I'll suggest 2 days a week with 300 to 500 calories during the 'fasting days'. A couple of eggs in the morning and a lettuce type salad at night. If you are adjusted to a high fat diet it is easy.
Vegan diabetics may find better health, and here is where I doubt the long term success of veganism, regardless of the success of reducing the T2 diabetes. Would like to hear Phinney on a panel with Saladino and other carne mds discuss.
Elleanor: If your blood sugars were higher, it is because your liver kept releasing glucose into your blood stream. Eating something, especially fat or protein, will stop your liver from releasing glucose. The insulin released as a result of eating will stop it. I have that same problem. I am diabetic.

Omad for 6 days and Sunday 8-16 intermittent fasting. Low carb, high protein, medium plant fats. Hopeful it will reduce my type 2. Goodluck OP

Light walking helps this process too. Do not do any kind of high to moderate intensity stuff when going for a 4-5 day fast due to dangers of muscle loss. Only light, as with a 20 min walk daily.
You may need to do this for quite some time, 6 months to a year, where you squeeze in a 3-5 day fast maybe every 2-3 weeks. And the rest of the time stick to an eating window of about 4-8 hours, and thereafter no caloric intake (not even fluids with calories), safest is water only when not eating.

problem is grains do need some processing to be edible. eating wheat fully unprocessed is not good for you or edible. yes pesticides make things worse but wheat is marching military fodder, not a food of choice.
also modern wheat contains gluten which is harmful to your gut. look up "leaky gut "
People have beem getting obesity and type 2 diabetes since at least ancient Greek times. So for humans, carbohydrate-intolerance has been around long before pesticides. Although, that doesn't mean pesticides don't do bad things to the body.

Phinney, like others, tells us what our macro nutrient intake should be "if we have weight to lose" but never seem to address those who are already thin. I have been on the ketogenic diet for 4 months. All of my markers have improved significantly and I am off my diabetic meds, which is great. But I have also lost 18 lbs and my BMI is now 19. I am skinny as a rake, always have been thin, and really don't want to lose more weight. Am I forced to choose between diabetes control and healthy weight? I wish someone like Phinney would mention how the diet might be adjusted for people like me.

You do not need a blood test to determine if you are insulin resistant. If you are overweight, it is because you are, or that you have prediabetes. Tim Noakes had diabetes even though he was training him self slim, so if you exercise a lot you may have insulin resistance even though you are not overweight. But if you are overweight you are insulin resistant.

My experience, after being in constant Ketosis for 4 months, is that I haven’t been as energetic or mentally alert in many years. Just say NO to carbs and stay on track.
I would say to start NJ intermittent fasting and slowly extend the time and gradually reduce carbs. Removing sugar is the best thing to do.
There are a number of keto ice cream recipes that are very simple to make and have very low carbs. My wife makes a batch that lasts weeks. Google Gnom Gnom for the recipies.
Willpower. When you surpass the 2 weeks barrier, the carb cravings disappear. And help yourself, if you are constantly on a situation where there's carb available, you may end up cheating, and cheating sets you back to zero mark. The frequently referenced range for being in ketosis is between 20-50 grams of NET carbs. If there are "added sugars" listed on the product level you can roughly deduct half of those added sugar carbs to further lower the net carbs. The reason is that depending on what mix of artificial sugars they add to the food, the body is not able to absorb half of those carbs.

What does "reversing" really mean?
Does it mean they just don't need to medicate? does it mean their blood sugar levels are in the normal range? or does it mean that their glucose levels are normal even after eating at breadstick and a pasta dinner at the olive garden like someone that is not diabetic?

My interpretation is that one can normalize their blood sugar and eliminate symptoms but if they begin eating carbs again, they quickly regain insulin resistance and metabolic disease.
It means that people who get diabetes T2 are carb intolerant. So you can either eat carbs and have diabetes or eat no carbs and be healthy. If you eat a reasonable amount of carbs with intermittent fasting so that your glucose and insulin can come down, you can consider it a reversal with no or less medication. If you think you can go back to binge on carbs then you are going back to what caused the disease in first place.

I am so miffed I am type 2 . Strict keto for 2 years. I lowered my carbs to leafy greens (under 20) yet my A1C went from 5.0 to 5.9. Blood tested in ketosis above 0.5 usually 0.8 sometimes 2.

Keto alone didn't fix mine either. Keto + fasting did.
6.4 or less is still successful reversal of type 2 diabetes. You can try Metformin if you can handle GI side effects as it has longevity health benifits according to this doctor in other videos.

I am a type 2 with normal A1c and no medications. I follow a strict keto with very little carbs. Everything I eat is organic. Lots of leafy greens, eggs ,cheese and lard. I am a bit confused with this study since he adjusted the meds according to the blood sugar but never stopped them. What is he proving by restricting carbs and still using medications? If his patients used vegetable oils, and consumed fat from non organic source, they were eating lypotoxins from sick animals GMO corn fed in cages etc. Half of the battle in Diabetes is to address chronic inflammation caused by linoleic acid and hyperinsulinemia. The carbs are just one part of the puzzle. There are plenty of studies showing the same results with high carbs low fat and protein diets. Each individual metabolizes carbs differently this is why these clinical trials are a waste of time. the truth is once you get sick you will never be able to eat both carbs and animal protein and fat at the same time for the rest of your life. No more pizza or burgers with fries ever. You have to first understand what causes Insulin resistance at the cellular level. No one knows.. I would guess processed food, hi refined carbs vegetable oils as well as milk, meat and fat from GMO fed animals as the probable culprits. Plus of course hi fructose from corn. Phinney is 72. Alot of the visible effects of aging is sun related. He does a lot of biking and outdoor activity. I have a friend who lived in Arizona for 30 years, fit and outdoors all the time and at 58 she is more wrinkly than my 75 year old mom who covered herself from the sun when she did go out (for a walk, no 3 weeks climbing the Andes and whitewater rafting!). If u live in n a dry place, that takes its toll on the skin too. My friend moved to the humidity of ny and she said people look a lot younger. But still agree. He looks kind of old. Another friend of mine, vegan, looks like he is 100 but he just turned 70...and he's looked about 100 for the past 20 years....wrinkly and saggy. Very fit otherwise. Thin and gaunt looking tho

I'll suggest 2 days a week with 300 to 500 calories during the 'fasting days'. A couple of eggs in the morning and a lettuce type salad at night. If you are adjusted to a high fat diet it is easy.

Elleanor: If your blood sugars were higher, it is because your liver kept releasing glucose into your blood stream. Eating something, especially fat or protein, will stop your liver from releasing glucose. The insulin released as a result of eating will stop it. I have that same problem. I am diabetic.
