    For those wondering what all this is: that piece that fell in the hole was either a drill bit or a casing coupling. The clasp on the clamp that grabs it was bent so when the bit got stuck on the side of the rotary table it opened and let the drill bit slide out. Under all that metal is a giant thing called a BOP( Blow Out Preventer) with Rams that close around the pipe. If they're close(they 100% should be) they're good. Will only take a couple hours to retrieve. If not the driller is fired and they'll call out a fishing crew which can take from 12 hours to 2 weeks to retrieve. The services are expensive and ever rig operation cost is $20,000 give or take. If a drill bit is turned upside down and falls down the hole they cannot get it back and the well is void and will have to be cemented in and side tracked with directional drilling. So this is anywhere from a 20,000 to a few million dollar loss. That would explain the floor hands freakout. I feel for him and hope that clears at least some of that up for yall

    Ok boys, here's the old oil patch story: Deck hand accidently drops a pipe wrench down the hole. They can't just drill it up. They cease drilling, and bring in a special crew to fish it out.
    After several 24 hr. Days and lots of $ spent ...they finally retrieve the mangled wrench.
    The tool- pusher "boss" throws the wrench into the young man's chest, that had originally dropped it.
    Take this as a reminder, he shouts, because you're fired!!
    The young man shrugged and said, well if I don't work here anymore. ..I don't need this-
    He tosses it back down the hole...!

      Looks like the part that came loose caught on the way up. You can see the right side shift downward for a moment then the gate opens. Pretty sure that should not happen.

      It's a piece of the coupling that when the pipe attaches to it opens up. That's why you see it's in 2 pieces and folds open like it does and falls in.
      Why it unhooked itself though from 2 people picking it up though like that it shouldn't of happened. It's just a complete accident and to retrieve that you're talking shitting down production for however it took to get something down as deep as the hole is an fish it out.
      Guys gutted about it as people on the rigs genuinely care about their jobs

    For all the people wondering what just happened. That was the bit inside the bit breaker. It's a PDC style bit. Not a "Try Cone" bit. The pin wasn't in the latch, so the bit wasn't locked in. They pulled the bit and bit breaker out of the table, and the bit slid out of the bit breaker, and down the hole it went. That ruffy got into major shit, most likely fired,,,,,,if the blind rams were open.

    The crew just finished Tripping the pipe out of the well, The Driller is at falt in my opinion, when done tripping and the drill string is out of the well and on bank (the rig floor) The driller should have closed the blind rams on the B.O.P stack. Doing this closes the well. This is common practice here in Canada. A drill bit is what went down the hole, The roughnecks did not put the pin into place on the bit breaker safety latch (which provents the bit from comming out of the bit breaker) Its just a steel plate that closes around the bit . Thats what you see swinging open when they both went to grab it. I do feel for that floorhand.. chances are he got fired. In all my years drilling in Canada and over seas; I personally would not fire him, From what I observed He is taking his work seriously and He would realize how close he came from getting fired, He will keep an eagles eye on everything on that floor after that!! That too me is worth more as a driller then getting ride of him and having to start over with someone new. Beside the oil company can afford it. I wouldn't stress it, Shit Happens;-)

      I Maintain you are incorrect, look close that's a bit braker plate with a latch & retaining pin. Notice the collar elevators. Strikes me they have just unscrewed the bit from the bit sub on bottom of drill collar by spinning it out with the rotary table, poor practice in my opinion, a fuck up waiting to happen. The Driller is ultimatly responsable for everyting that happens on his rig floor. Some operators I know don't close the blind rams when above the BOP. Esp. offshore. sooner fish the bit(you know where it is )than have to pull the BOP & inspect Blind rams for possible damage. The driller left the bit in the rotary table. He should have broken the bit, Picked up out of the table with the braker still attached, pushed the collar to one side , INSTALLED HOLE COVER, & only then unscrewed the bit & removed it from the rotary table area. That's why the company give the driller the responsability & pay him handsomly for IT, as an old retired driller & drilling supervisor I would be wanting to talk to the driller before I threw the floorhand under a bus.

      I fix equipment and yes operators get away with anything until murder. Plants often order and use wrong equipment for jobs which put everyone in a bind for safety. I have to repair and then pray over them as they use them overloaded 24 hrs a day 7 day a week.

    Not a common issue, but easily resolved with minimal downtime with the correct equipment. The coupling has failed, it's part of the rod retaining system, and the failed support bracket unfortunately has fallen onto the drill floor and when moved, seperated and entered the drill string. The way to recover this is to use a recovery wire rope that has an electrohydraulic or fully electrical multi jaw claw, light and camera attached to the end. This is introduced into the drill string, camera used to monitor conditions and depth, and when in range, the jaw are released from their parked position, opened and enclosed around to retreived object. A test is done to demonstrate successful attachment, and recovery begins, monitored via camera. The jaw is gyroscopic, so can freely flex independent of the recovery body, ensuring a smooth pull to the surface. Lost time may be less than a full shift. Fun fact : The jaws, usually with independently operated fingers that can extend or shorten to obtain the necessary grasp, can have a clamping force of up to 100 tons and are only restricted in size by the internal diameter of the drill rods.

      Jaws, camera, lights?? Thats too complex and unfeasible especially in drilling phase where the well is filled with water or oil base mud which is completely non-sea through ( full of viscofying agents like bentonite and weighing material like barite, and some cuttings debris…) also wire line added through the pipe to control those jaws is another major challenge.. in reality, a Flow Release Overshot, GS pulling tool, or a Bowen Type overshot will be utilized to either latch on the external or enternal fish profile thats in case a clear fishneck profile was identified by a slickline Lead Impression Block run (LIB).. if no clear profile to fish, then a SL with a HD Super magnet can be RIH to fish out the metal part as per weight indication..

    I'm genuinely interested why there is a two piece mechanism right next to a tiny hole that could easily fall apart and fit down the hole causing catastrophic consequences, I understand human error and of course theyre paid well for this not to haopen. However due to the cost of the error do you think they couldve re-engineered it slightly differently

      You make or break your bit in the rotary table, there’s slots into which that plate fits so you can properly torque your bit, I’m assuming they neglected to latch it properly prior to lifting the bit.

    This is catastrophic. For everyone who thinks it’s just an accident; this would be the equivalent of driving a forklift and running into a pallet stack of extremely rare and expensive bottles of wine, causing them to fall over, and every one of them breaking. This looks like an on-shore rig given the attire of the offsiders, but it still costs $10,000s a hour to operate, not including scheduling costs and production costs.
    Of course it is an accident - but you are being paid good money for this EXACT THING NOT TO HAPPEN. Not only do you now need a special team and equipment to fish out the drill cap, you also will stop drilling until that’s finished and depending on the roster you may need to fly a roster out and organise new people to fly out. That hole in the ground is the only reason anyone is out there.
    You can see his reaction and it is warranted. He was probably sent home 30 mins after this incident and fired after he had left site.

While a few thousand won't cripple the company (assuming it isn't a small one), the possible multi-million dollar price tag to fix the issue may. I'm not a driller, never have been, but I have friends who are, and what little I've gleaned from them about this industry, my personal 10+ years as a business owner, and my formative years spent as the child of business owners, tells me this is potentially, nearly business ending bad. Now, if everyone did their jobs correctly, and it was the equipment malfunctioning, it should be a few thousand for the retrieval, plus daily expenses. However, if people -didn't- do their jobs, well....

    I worked on a supply vessel when I was younger.
    One trip we spent a full week on location to deliver fishing equipment, that was our last container on board. We waited trough a storm, and on the seventh day we delivered the container. I did never understand why we wasted all that time for just one container. Now I understand.

    Surely if there's a recognisable risk of that happening (it looked to me like it fell apart), then the equipment should be redesigned to mitigate that risk? Blaming someone is all very well, but it doesn't stop the same problem happening again.
    Drives me f**king nuts, all the places I've worked, all the times i've said "we need to do something about XYZ, else a mistake is going to happen" and then when it does happen (I f**king told you so), all they did was point a finger at someone, or fire someone and carry on exactly as before until it happens again.........

      Behind every mistake there is a faulty procedure or work instruction. Mistakes should not occur if the process is engineered correctly.... At it's core, quality engineering is about robust processes, which when followed eliminate the possibility of a mistake. Unfortunately "quality" is difficult and expensive to measure and most companies BS their way through it, whilst "quantity" is simple to measure and looks great in boardroom presentations.

    One time we tried to stretch out the bit trip for the next crew.. We saw the relief crew pulling onto the site and just then the whole string of pipe jumped about 2'... It was then we realized that wasn't good. We lost all three cones off the bit. They got the first two with two trips with a basket, but took another two days of tripping to get the third. Thank God our driller was the Tool Pusher's brother. We should have been run off! Hard lesson learned that day!!

      The rotary table spins, which is what they grab when they bend over and there is a shaft that mounts to it which is what was lifted up, when they bent over to grab the rotary table the mount pin was not in it and caused them to drop the shaft head mount into the drilled hole. It

    If you look carefully when they go to pick up the thing out of the hole the inside part of the mechanism falls into the hole when the holster that the two technicians grab onto releases The latch holding on to the center part. So it looks like either the latch broke or the latch wasn't put on properly.

BRUTAL. Tripping pipe can take days and it can SUCK.unfortunately this kind of shit happens to even the best guys and this wasnt one guys fault. This is the push, driller, and both guys pulling the slips fault. If the blinds were closed these guys would just be getting shit on and made fun of... now wireliners have to come and that is not cheap. Would not want to be the guy waking the consultant up with this

    As per the rigs construction, shortly below that bit there should be a valve/door, easy to close and easy to open. Pull the lever, it closes, then pull the bit out, because this simple error costs a lot of money to fix. Youd figure someone would make a failsafe to prevent potentially losing an entire drilled hole from a simple oops

    They were pulling the drill pipe slips, a 3 hinged device that supports the drill stem when tripping pipe. The slips fell apart and part of it went down the hole. The piece will have to be “fished” out.

    For those wondering what just happened (other people have commented but don’t quite get it):
    The worker had just purchased a brand new IPhone 15 Pro Max for his wife and opted not to have AppleCare. In this clip, you can just see the new phone, which he has in his pocket, fall out and down into the rotary hole. It would take a 5-man team days to weeks to recover the device and even if they could, the fact he didn’t chose AppleCare means that the damage to the device is likely going to be so extensive that Apple will advise in favour of him having the whole device replaced. Since it was accidental damage, he’s liable to pay for the whole value of a new device.
    That’s why you can see him clutching his head in his hands over on the right.

    it is the company job to have a written procedure to tell each and every single hand on the rig what, how and when to do a certain task. 99% of operations have procedures in order to avoid such a tiny yet a costly mistake.

    The part they were holding on to were the wedge shaped holders of the hardened piece that fell into the hole. The piece that fell into the hole is called a "slip", not because it easily slips into the hole, but it is intended, as part of it's function, to allow the drill pipe to slip past it as it is pulled out of the hole. Then, when the drill pipe is as high as the driller wants it, he simply lowers the pipe back into the hole past the slip. The wedge shaped holders, with the handles they grabbed push against the slips which wedge against the drill pipe and locks (binds) it into place, usually with the joint above it.
    When the driller is "going back into the hole" after changing the bit or something like that, a new piece of pipe is put onto the stem that is already in the hole, screwed down, and the column is lifted so the helpers you see can pull out the slips, thus allowing the stem with the new piece of pipe, lower back down into the hole. As the top of the pipe nears the table, the helpers put the wedge holders and slips back into place and they lock down on the stem, allowing the winch block to be loosened and removed for the placement of the next piece of pipe. Usually the holders and the slips are bolted together to act in unison, but in this case, the slips were a separate item from the holders.
    That kid didn't watch the slip as the stem was raised and, as you saw, it fell into the hole. Often the slips are attached to one another by a short piece of cable, but you always have to have a hand on it when the stem raises because the slips want to go sideways and fall into the hole.
    I did this once at the very end of the hole. The tragedy in my case is that we lost the slips. I felt worse than that kid, who was about to lose his job and this was before the days of really strong magnets.
    The real problem is that the slips are hardened steel - really hard steel that the bit likely cant drill thru. This means that, first, you have to go find another set of slips to use, second, you have to trip all that steel back out of the hole along with the bit and, if the original slips didn't get hung up on the bit and come out with it, you've got a fishing job to do. That can be heaven - or..... that can be hell.
    Sorry for the whole crew when they have to tell the boss what happened. Ouch. Pipe wrenches are going to fly....

      Ok so about $22000 an hour and it could take anywhere from 2 days of 24 hour shifts to 2 weeks of 24 hour shifts.
      It looks like a casing head or diamond drill bit.
      Hopefully they can get it out after that or if it fell diamond side up then that's several million dollars gone and they have to fill the hole back in to redrill.
      Let's not forget that if the blind rams weren't closed then literally everyone is fired and the driller themself will never work in the business again unless they change names and face.

    Having a portable hole loader winch with a high powered magnet on the end might work. Probably a big spool of winch cable, depending on hole depth and a guide to keep the magnet from grabbing walls on the way down.

    It seems like any pieces of equipment used on or around the drill shaft opening should be attached to some kind of tether at all times to prevent that very thing from occurring

      You understand that hole is 20,000 feet deep, and that they can't resume drilling if that thing is in there? This will cost the company hundreds of thousands in lost operations, and millions if the bit can't be retrieved because then they have to close the well. This is a VERY serious fuck up. 100% it killed this guy's career.

      They can do that however the cost isn't in the actual thing, it's in the ramifications of his accident. What he just did is going to cost them thousands of dollars, they can't continue working on the well without retrieving it, they have to bring in specialists at fishing stuff out of the well which is very expensive.

    I didn't find the answer but my guess is that they cannot drill anylonger because that piece of metal that fell into the hole is too hard and they cannot continue drilling because will broke the drill

      Yes, that's exactly what they will do. The new bit will either drill thru the back of the old or force it into the wall of the shaft. The cost of the bit and downtime for inspecting is the only real issue. And explaining to the driller that you just dropped an expensive bit down his hole. One hell of an ass chewing at least.
