Filial piety, i assume, is practiced mostly in Asian countries. It's like an obligation to your parents, elders, and family name. It's kinda like the belief that everything you do will reflect on your family so put them first instead of yourself (I hope i explained that well) Neung was basically telling the grandmother that she was taking advantage of that and going too far with her demands for Sam.
Grandma's excuse is about Kirk would be Sam's spouse legally, Mon could never because of Thai laws. Kirk could authorize surgery, Mon would just be helpless if grandma was gone and Neung wasnt in country, Sam would die basically. And the same would have been true if something had happened to Mon and her parents, Sam wouldn't have any legal standing. And you're so right about Freen and Becky, the ep just highlighted how good they are.
