According to ISPAD protocol.....in case of severe dehydration without shock we give NS 10_20ml/kg followed by .1 units insulin/kg/hr.. and in case of DKA with shock we first give bolus NS 20ml/kg and then repeat until circulation is restored...then we follow the same as we do for DKA without shock i.e: 10-20 ml/kg followed by insulin .1u/kg/hr.. If tissue perfusion is poor the initial fluid bolus volume should be 20 ml/kg.." In the given case scenario, as the child is not in shock and tissue perfusion is fine, we should initially give 10 ml / kg NS in the first HR f/b Insulin 0.1U/kg/hr
