Frasier, you NEVER use something like a Jack in the Box, that's just going to terrify your patient even more (the shock of the instant appearance will add to the negativity the client has towards the clown).

This is a very funny clip, but it reminds me of years ago when I took my son to the circus. At one point some of the clowns greeted the children in the nosebleed section of the arena, where we were seated. My son was very polite and shook hands with the man... but a couple of rows down from our seats, a little girl took one look at the clown and had hysterics! That poor man's smile dropped off his face like it had been cut off. I'm sure he went to clown college to bring joy into people's lives... not to make little children cry!
I always felt bad for clowns in that situation. I wasn't a fan of it as a kid, but now, I know how hard clowns work to make people laugh. Honestly, I think there's a lot of nobility in dedicating yourself to a job like that.
Mind you, this is the same Mimi who Niles ran into on the Voyage of the Damned who was the cause for Maris throwing(or rather the waiter throwing) 3 drinks on Niles so somewhere Mimi's fortunes went down hill because apparently she was in Niles and Maris' social circle.
