Breads need a high enough heat to set the risen dough on the top. Heat too low doesn't set the top resulting in a "cave-ln." Every time the bread maker cover was opened during the bake resulted in heat being let out. I bet there was a significant drop in temp in the machine. As far as the wetness of the dough I'd double check your measurements because those you listed would yield a 55-60% hydration which is fine. But your dough was way more than that. I couldn't make out the units on your measuring glass but maybe it was more than 1-1/2 cups?

Add the milk, salt and then the flour. Put the yeast in last. Salt will kill your yeast if it comes into contact with the yeast directly.

So I noticed the milk was steaming when you poured it in. Do I warm it up before adding?
