There was one in NZ / Barbarians 1973 where JPR scored after long phase of play but think a forward pass at some point in the phase

there is an unknown one that shocked me years ago :
2006-2007 - Top14 - Stade Toulousain - SU Agen - 25th day of the competition - 35th minute
That was a collective action of 90meters including Elissalde, Poitrenaud, Heymans, Bouilhou, Clerc, Dusautoir (international players), but a forward pass invalidated the try
