I remember taxiing down Alpha in the summer of 2004 or 2005 to take off on 19R, and one of these things started their takeoff roll with No. 4 dead, not even feathered. Ay Carumba! They just kept on going and either didn't know or didn't care. It probably would have cost them $500 worth of 100LL to abort, restart and taxi back. Great airplanes, but I guess they don't count on everything working all at once; probably a good policy.
The captain barely knew he was on three engines, the beauty of a four engine machine, always able to get back to a maintenance base.
