    We had the EXACT same experience when we switched to a new PDM. We had been on OP5 for over a year with decent control, but i didn't realize treating lows and correcting high 3x/day at minimum wasn't normal LOL. Then his PDM got a crack in the screen, so we switched to a new one. i was able to re-enter all his settings, and the first basal schedule it used was our "Hawaii protocol" which is 0.3u/hr all day (time zone changes + activity creates craziness); but then put him in auto. His sugars were outrageous, and the auto was never giving more than 0.05u per decision point even if he was 400! I adjusted down all his goals to 110, decreased his correction ratios, increased his max bolus, nothing helped. Here's what we learned (after i spent 3 hours on the phone with OP and my CDES).
    1. When you start a new PDM, NONE of your previous information carries forward. Obviously if you are new as well.
    2. The algorithm only uses ONE data point when it starts - totally daily insulin. That number is derived from your FIRST manual basal schedule (doesn't matter how many you put in after, it only uses the first one).
    3. The algorithm decides every 5 minutes how much it should give to keep your BG in range BUT if can only give your total daily dose (from #2) divided by 24, then divided by 12 (total daily dose/hours per day/decision points per hour).
    4. So we also did a hard reset then put the first manual program using TOTAL daily insulin (basal + bolus) divided out over the day. This allows the algorithm to give higher doses early in a rise so you don't continue to creep.
    5. You are correct, please create another basal schedule that actually is your normal basal needs so you have a manual backup schedule.
    For the first time we aren't giving multiple corrections/treatments. Actually for the first 2 pods, I only gave ONE correction and didn't have to treat a low until maybe the 4th pod. Why is this information not more widely available?!? I had to call our Endo office and educate them about this. So glad you figured it out!

      Sometimes when the pod gets mad for a persistent high it automatically switches to manual mode. How do you get it to switch to the less aggressive basil program during this time and dex changes? Dont want to going into the basil program that is actually your basil+Bolus...
      If I am understanding this correctly this number affects the algo forever but is a 1-time thing. The first basil program/rate per hour sets this number in the algo. No changing after that without a ''hard reset''?

      This is what its happening to my daughter, it only gives her .05 2 or 3 times in an hour when her basil is .65 per hour. She sits between 130 and 140 at her lowest, how do I change it? I want to like it but what do I do?

    Thank you for sharing your experience! My son has been using the Omnipod system 10+ years and I JUST got him to accept the Dexcom G7. He’s on the Autism Spectrum so change is not easy. I am anxiously waiting for Dexcom G7 and Omnipod 5 to integrate but this definitely gives me some food for thought to discuss with his doctor. At times, he runs high, especially when he doesn’t exercise so those highs do concern me. Will have to see how it goes.
    I totally understand your dilemma. At times my son goes off the Pod to do injections (to give his sites a rest) and it’s crazy, it’s like the rules go out the window! I have to treat him very differently than when he’s on the Pod. But when he goes back to the Pod, things go back to normal with his different ratios. As I tell my son, Type 1 does what Type 1 wants to do! 🙄

    I've been on Omnipod for 2 weeks now. Thus far, it has been a very positive experience --after resisting a pod, despite my doctor's coaxing, for years. I'm stupidly stubborn ;)

      Oh nice! I'm so glad to hear it's been such a positive experience - this is wonderful! I personally don't think there's anything wrong with being stubborn and taking your time with the whole pump thing - I was the same way! We have to live with the condition which is already a huge undertaking - so we deserve to do it whatever way we want to and is best for us - both in mind and body! ❤️

      Have you found that you've had an improvement in staying in range with the pod? I've also been resistant to getting a pump but if i do get one i want one without tubes.

    • @jamescostigan3721
      @jamescostigan3721 Рік тому

      Yes, definitely. Besides it working with my Dexcom G6 to deliver insulin as needed throughout the day, the two key areas where it has helped me improve my numbers are: 1. Helping me to pay more attention to my carbs and helping to apply the right amount of insulin for my carb intake; 2. I was having a real problem with my numbers rising fairly dramatically while I was sleeping (Sunrise effect, I think it is called) and the pod regulates that by delivering insulin as needed during the night. This was my primary motivation to finally go to a pump.
      The Omnipod 5 is tubeless. It inserts a canular (it's a little plastic insertion piece, sort of like a hairbrush bristle and very similar to, though slightly larger than, the insertion piece for the Dexcom CGM. Painless. The only shortcoming, and it is a very minor one, is that the pod needs to be changed every 3 days. I suppose that's the best to be expected since it holds up to 200 units of insulin. For more than that, the pod would have to be inconveniently large. I would definitely recommend at least giving it a try. The Omnipod Starter Kit comes with 10 pods (i.e. 1 month) and a controller (Android phones can use an app for the Omnipod, but there is no app for my iPhone, yet). I probably almost sound like a hired pitchman for Omnipod, but I didn't know when I gave in to trying a pump that it was tubless. I was very pleased to learn that it was tubeless. I've been on it for just over a month now and have gotten the hang of it and I'm pleased with the results and the overall convenience of it. Good luck with whatever decision you make!@@Captain_Gargoyle

    Don't count on it to adjust to you. I have mine set at a target of 110 and it consistently keeps me at 150-160 if I don't adjust it on my own a lot. They'd rather keep you there than risk lows it seems. I've noticed that it also consistently shuts off my insulin delivery when I start to drop at all when it's 140-150, and then it spikes. The vast majority of of my highs are associated with the device shutting off my insulin. I've switched to using manual mode most of the time now and have much better results.

    • @ShesDiabetic
      @ShesDiabetic  9 місяців тому

      I used automatic right off the bat, however, you can use manual if you’d prefer. But because of the higher ratios I’d be VERY careful in manual because I know if I was in manual I’d be plummeting nonstop 😅 and also to note, the system is “learning” the same, no matter what mode you’re in. I hope this helps!

    Interesting approach with the reset. I'm glad to hear it's working for you now. I completely understand the frustration with the initial highs. Mine basically evened out after 4 or 5 pods. When I was deciding between Tandem & Omnipod, I didn't think tubeless was important (after 13 years of Medtronic), but it's a game changer for me.
    After 9 months on Omnipod, I have 2 gripes.
    1) I wish I could set the target glucose lower than 110.
    2) It's fabulous at avoiding lows, but I wish it was more aggressive at treating highs. Sometimes I feel it's working against me. "Oh, you bolused so you don't need this basal." To combat these situations, I switch to manual mode, bolus & wait for it to trend downwards, then switch back to automatic mode. But when I start to trend high overnight, it's not aggressive enough & frustrating to wake up 140-160. Luckily it doesn't happen often. I'll gladly trade an occasional morning 160 for sleeping through the majority of nights without lows.
    Overall though, I am thrilled with Omnipod. My A1C was already good & it hasn't really changed, but I suspect it's a more accurate number without the roller-coaster highs/lows.

    Very interesting. I have no worries that someone as thoughtful and smart as you will be safe in doing what you did. Indeed doctors have zero imagination when it comes to individualized care such as what you described. They operate on the same kind of “algorithm” as the Omnipod. They just call it: “standard of care”. Of course there are reasons that’s mostly a good idea too. But nonetheless there’s no one on Earth that will be as in touch and motivated to make it work as ONESELF will be. Nice job!

    I appreciate your candor about the system and steps needed to fit your needs. I will be interested in hearing more about the pump as I always thought the thing must be huge and prone to being bumped/torn off, etc. I went from a minimed 760 pump to the tandem slim a few years ago and love it so much better then the minimed pump. My only gripe with it is I would like to use the dexcom phone app but since I have a pixel phone and its always getting system/security updates tandem will never approve their software being run on it. There are work arounds which I did as well as got my blood sugar to display on an old samsung galaxy watch which actually for me has been the game changer as I can just look down at my writst and see what I am trending. Anyways I am glad you are posting more videos again. Thanks

    Hi. Great video and very much needed. Your experience mirrors mine exactly! And interestingly, I was sent a new controller recently, though I’m back on MDI, and I wonder if maybe the controller also might’ve been bad. All I know is the reset change the game totally. And in total honesty, I actually went to the T-Slam straight from a week or so where the Omnipod did what it was supposed to do after the reset, just to kind of see what the difference was, and then, one week of T slim, it outperformed Omnipod five. I should also say I experienced a lot of tunneling and my pods were bad sides by the third day for sure, so I had to change every two days. When they get it, worked out to use the iPhone and maybe someday if they get a different cannula that might work better with me and my activity, I will go back to tubeless because I love it. Right now I’m taking a break on MDI, but probably if I had to choose right now, I would still go back to T-Slam because it works right out of the box. For my body anyway. And I think it’s the algorithm that only predicts 30 minutes in the future is actually better for me than the one that predicts an hour like the Omni pod and can get too conservative.. Thanks for making this video to help others. I love your videos. and there is a Juicebox podcast episode about resetting, though not for our reasons and it gave me the courage to just do it. Thanks again!

    I've been on Omnipod 5 for a year now and still experience highs due to the automatic system stopping basal when heading low. I thought editing basal programs only impacted Manual Mode insulin delivery. When in automatic mode those did not work since the system automatically adjust the basal insulin depending on your settings and your cgm readings.

    Thanks for the video and sharing that info, was really great info.
    I guess it goes to show that it's made by a company, and as with many companies, it's ruled by risk-averse lawyers. But they know they won't be sued by people in their 70s with neuropathy so it makes sense they'll run you higher rather than lower.
    My biggest problem with omnipod, and all closed loop systems, is the trust it places in the CGM. I find my CGM (albeit the libre 2) is so unreliable and inaccurate that the idea of a machine using it to decide my insulin is truly terrifying.

  • @kaddiddlehopper

    I had a similar experience for the first week. I went in and adjusted the insulin duration down, and this immediately resulted in increased rates and much tighter control. I think the 5 hours or whatever the baseline setting is doesn't work for most people, and results in the insulin on board being calculated too high and basil too low. Before week 3, i was consistently waking up at 105 to 115. I left my basil rate settings alone. I came from the t:slim as well. I have been on the omnipod 5 for almost a year now. My last a1c was 5.3.

      @ShesDiabetic  Рік тому

      Thank you so much for sharing this with me. I also brought the insulin duration down from 5 to 2 which was very helpful. I'm still ironing out a lot of difficulties but take a lot of comfort from knowing you like the system so much! Do you not eat many hours before bedtime to achieve the 105-115 wake up level or does that not matter re the overnight readings? I notice when I eat later at night my levels are far more stubborn. I'm still struggling with waking up around 130, can't seem to shake that for some reason :(

    • @kaddiddlehopper
      @kaddiddlehopper Рік тому

      @@ShesDiabetic I don't typically eat late in the day. And if I do eat late, I will often do a manual correction. A brisk walk for an hour a day seems to help blood sugar levels for me the most. If I don't get some physical activity in, I get insulin resistance.
      You don't realize how much you hate tubes until you try a pump without them. I think all pumps in the future will be this way.

    My daughter uses the Omnipod Dash at the moment. We find that on the third day the adsorption seems to be a lot worse meaning she tends to go high and a lot more insulin in used. I assume this would cause an issue with the Omnipod 5 algorithm?

    • @ShesDiabetic
      @ShesDiabetic  3 місяці тому

      I must say I find the 3rd day to be rather hit or miss myself too! It does cause an issue at times, but to get around it I've asked my Endo to prescribe a change of pod every two days so I have enough pods to do that if I need and that helps circumvent the problem. I sometimes have a pod that's just good and I let it go the full 3, but If I sense absorption is dropping off I change it right away. I hope this helps!!

    In the FB group I saw a few people mention a hack/method that helped them. "Pump and dump" some call it, after you remove it for a change, leave it connected and dose around 5+ units into the garbage. The algorithm then thinks you're needing more total daily. Never tried it, bud did consider the reset a few times.

    • @ShesDiabetic
      @ShesDiabetic  3 місяці тому +1

      It only affects the 'jumping off point' for the algorithm. When you up the initial basal settings that's informing the algorithm that you need more total daily insulin (which is what the algorithm is primarily based on). Becuase I find the Omnipod to be pretty conservative this pumping up of numbers is one way I've found to influence the system to give me more insulin right off the bat. Just a personal thing I've done - it might not be the right thing for everyone, but for me it was really really helpful. I hope this makes sense!!

      @@ShesDiabetic thank you. Yes, I get it. Another question I had relates to the “pump & dump” hack. Does the system translate the bolus you give down the sink as “Insulin on Board” and thereby does it impact on delivery of automatic micro doses (and corrections for that matter) for the next “x” amount of time as it “processes out of the body”?

    The other frustration I have with the auto mode is that In case of a “ fake “ low , like a compression low or a sensor issue. There is no way to tell the system you weren’t actually low, with the end result of a even more conservative dose.

    My biggest issue with O5 is that it doesn't allow for extended bolus in automated mode. I have gastroparesis, so digest carbs slowly, which means every single time I eat carbs I need to switch to manual mode to extend, then remember to switch back...If I take the full bolus I'll start to drop, my bolus will switch off and then of course I'll skyrocket as a result of that and my carbs digesting. I just don't understand why they don't allow it. They really should have consulted more diabetics when developing the system, or gotten more feedback after it was widely available to improve it.

    Just started using the omnipod. This video is only telling you ONE thing. Get your basal rate correct to start with! If you need to 'correct' throughout the day your basal rate is not set properly or your carb entries are not set correctly. So far my BG regulation is MUCH better.

    I switched from Medtronic to Tandem several months ago and I was running high all the time. My endo didn't offer any ideas on how to fix it and the trainer just said to talk to my endo. I did the same thing and I increased my basal rates dramatically. It has made the algorithm work as it should! When the trainer saw my doctor's note to "use current rates", she told me that my manual mode Medtronic settings wouldn't work and she was right. She just wasn't allowed to tell me WHY they wouldn't work and what I'd need to change to make them work.

    Off topic. WERE you to go into a hospital for any length of time, would you feel more comfortable being on MDI or an insulin pump? Same question for the aged or those with vision problems... I.e., is MDI SAFER in certain circumstances?

    • @ShesDiabetic
      @ShesDiabetic  Рік тому +1

      Hmmm...that's a really tough question. I think I'd vote pump if it were today because there are a few semi-closed loop systems to choose from that could make life that little bit easier. With vision problems, it would probably be easier to see some screens over others, so I'm not sure about that, it would really depend on the individual. And in terms of age, my Dad was on a pump in his 70's and did awesome, so as with all things in life, I truly truly truly believe age is just a number and a pump can suit anyone providing they're willing to put the work in to learn a new piece of technology and way of doing things. I hope this helps answer your questions!

    I tried the Omnipod 5 for almost a year and gave up. I was able to get good automated mode numbers with my 4th controller. However, the controllers would have app errors/system loading when I tried to bolus, so I had meals without the bolus I thought I did on numerous occasions.😥 Has anyone else experienced this? I tried one pod on my phone app once my phone was finally supported, and it didn't happen there, so I'm thinking it's just the operating system on their controllers....

  • @PKWeaver74

    That's weird because i switched to using AAPS with my Omnipod Dash and although at the moment I'm not authorised by the app to go into fully closed loop (I have to authorise each basal change until I've passed certain milestones) and I'm already considering it as close to a cure as i need. Kept my blood sugars flat between meals at 6.0mmol/l which is what i set it to, and during the night it fluctuated as normal as i returned to my standard basal.
    All due to an open source non approved app, almost like the community has been more successful than big pharma.

    thank your for this video, it's really interesting! i'm usning a diy loop system and i'm really curious about these commercial APS systems.
    there are similar issues with my AndroidAPS when it seems that the APS is giving you more/less insulin than it should, you start correcting it, but it doesn't work, only makes things worse. usually it means something is off in your input data (insulin sensitivity might be wrong, or the insulin/carbs ratio, or even the duration of insulin activity) because all of these aspects affect how the algorythm works. for example even if you are not eating at the moment the IC ratio still affects the calculations for correcting highs or lows
    not sure how it's all done in the Omnipod 5, but I had a similar period with my DIY loop where it seemed I know better than it does, I can clearly see I need more/less insulin, why is it giving me so much/so little?? the target glucose is obviously not reached, what the hell?? and each time it was fixed by correcting my input data. i've read tons of documentation for APS users to understand how everything affects the perfomance of the algorythm, had to literally un-learn some stuff about pump control (like getting used to having my basal turned off most of the time, it's counter-intuitive)
    tl;dr: it takes time to understand how the algorythm works and un-learn your old understanding of pump control to make it work

      @ShesDiabetic  Рік тому

      This is so interesting and I really thank you for sharing this with me. It's HUGELY insightful! Omnipod claims that nothing you do affects the algorithm and it's going off of Total Daily insulin dose only, but I just have a very hard time believing this is the case...?

    Hi Andrea, thanks for posting such helpful content. Do you struggle with your “fake” +40% basal rate when you switch the pump to manual? Or is it a non issue because you never do?
    Also, did the algorithm eventually lower your background insulin to make its’ 50/50 split of tdi?
    Thank you!!

    • @ShesDiabetic
      @ShesDiabetic  9 місяців тому

      I set up a separate profile of my actual rates so whenever I go into manual mode, I go to that profile, if that makes sense. And yep - the algorithm has eventually lowered my rates and I'm sitting within that 50/50 split for sure. Actually it lowered it right away, which meant that it was lowering to my normal rates (due to the settings being so aggressive). I hope this helps!!!

Instructor used the settings from the Medtronic to set up the Tandem and it worked very well from the get-go. Adjustments have been made in time but is a great system. Filling the reservoir is a bit more of a hassle but becomes second nature after a new infusion set changes. I can understand your apprehension but you will do fine.

    I currently use the Tandem Pump, and it has been very good to control my type1 diabetes. I currently use the quick bolus operation to support my carb intake. If my glucose level is at 120 and rising, I have been inputting a higher level of insulin to combat the increase. I usually follow a 5-15 increment insulin increase until my glucose levels off. I do this because my carb intake is low at both meal and snack time. My question is, can you follow this process on Ominipod-5? By utilizing this process, I consistently report within 99% of range. The idea of a tubeless solution is very appealing. I'm very interested in your response to my inquiry. Thank you in advance.

    • @ShesDiabetic
      @ShesDiabetic  Рік тому

      If you mean just bolusing more on top of what the system is giving you as basal, you absolutely can do this with the OP5. However, coming from the Tandem Pump myself, I do wonder if you can achieve this level of control on the OP5. You can bolus all you want, for sure, but you can’t do things like extended bolus, nor can you increase your basal rate when in the automated mode. From what it sounds like (and I know I don’t know the ins and outs of your usage of the Tandem) I would say getting this kind of control isn’t impossible but it would probably take a very significant amount of work (up front at least). I hope this helps answer your question!

Thank you again for shedding light that this is a possibility while utilizing the Omnipod pump. As you stated in your review, the tubeless solution is a game changer. Tim

      @@ShesDiabetic You bet. Originally my number was 15, but my bolus requirements were constant long after meals. My doctor told me to start reducing that number by 1 each day, and that I could reduce it by .5 for fine tuning until I was getting better results. I’m now at 7.5 for many months and happy with that result. My target now is at 120 during the day and night, with 110 between 9:00pm - 12:00am and 110 between 3:00am - 6:00am. The early morning target reduction helps withe the dawn phenomenon situation which my body often experiences. I hope this information is helpful and like always, communication with your own doctor’s is recommended for all users of Omnipod 5.

    You should do a “Back to the Basics” type of video about what people call “hard carbs
    Through watching T1D videos I’m seeing that to be a granola bar or pb/cheese sandwich crackers. I want to see what you say you define hard carbs as.

    • @ShesDiabetic
      @ShesDiabetic  Рік тому

      Interesting - I've always known hard carbs to be something like bread/crackers, etc after treating a low with more aggressive means like glucose tabs (for example), to sustain the higher BG level. It's not something I personally feel necessary most of the time, but to each their own. Tell me more about what you'd like to see in this kind of video - I'd be very interested to hear!

    I'm actually thinking about switching to Omnipod 5 System in AID with the G6 for my 6 yo daughter, since she isn't gaining enough weight to qualify for the t-slim x2 / Medtronic 780G... She uses the G6 + Medtonic 640 G and by handling everything manually, we cannot fall below 7,4 HbA1c. First I couldn't relate to this aesthetic issue, but -like you said- I believe the kind of "wireless freedom" will give her much. SInce she's too small to handle the calculations on her own, I have to really think about the PDM-handling. If the boluses could be given via smartphone, it will be nice (she carries a lot atm, a pump, a cgm-monitor and a phone to upload the sgm-data). I'm impressed by the view you do share here, thank you very much

    I am a totally insulin dependent (novolog & LANTUS) type II diabetic. Because I use the Dexcom 7 CGM and InPen, and love what they do for my insulin therapy I know that I have
    Use 22-26 units of novolog per day and 14-16 units of LANTUS per day. I am going for my initial consult to use the Omnipod 5, should I insist that my basal rate daily insulin for novolog be set at 26 units, or perhaps a skosh more like you did at 3 months, say perhaps 34 units to try and baseline my blood sugar at 110? Your education is really helping me to go I not this therapy meeting better prepared. I very much value your videos. Thank you thank you thank you! It's very easy feel isolated as a T1D. I live alone and despite the highs have been told to give the omnipod time, it's been nearly 7 weeks since starting the system with the omnipod /dexcom and like you, it doesn't keep me anywhere near target and that target is 1/3 higher than my previous target on another pump. My Hba1c is due soon so that will be an interesting conversation with a DSN! I now feel 1) I'm not a freak because this system isn't working for me. 2) there's a plan I can try to do, like you said so it's on my side not fighting me all the time. Thank you again 😊 Lisa

    I am on week 3 & it is always waking me up at night for trending high, like 276!!! The nurse said to adjust the carb ratio & so far it's a little better. Although from taking shots 6 times a day to now being on the pod it is still way better. I just thought it would treat highs much better.

    These pump algorithms remind me of software on a car that's designed for autonomous driving. Different automakers are at very different levels when it comes to the car being able to drive itself and the extent to which a driver needs to take over. The same with these pumps. I currently use a Tandem pump that has Control IQ, but I stopped using Control IQ a long time ago because it frustrated me for the same reasons you expressed. And while I would love for my next pump to be tubeless, it's more important to me to have the smartest algorithm available when my current pump's warranty expires. I'm not even sure which pump fits that description yet. What I do know is that if you have to trick your pump's algorithm by inflating your basal rates to get the control you deserve, the software on the pump is not as good as it should be. If your target glucose is 110, it should not matter what your basal rates are set to. The software should continue to give you more insulin until you reach that target. If the algorithm is not smart enough to override your basal rate settings to get you to a healthy target, it is probably still too dumb for me. Type 1s deserve to have fully autonomous pumps with the level of aggressiveness they are comfortable with. Until then, I'm glad that you were able to trick the Omnipod 5 to get the glucose readings you are happy with.

    Hi Andrea,
    Thankyou for your great information. 👍
    I am still using pens, and never liked the ideal of giving total control to a machine, scary.
    But I am tired of never getting a break from always giving little top ups , to keep in range. I like the ideal of no tubing, so this would be the one for me. Does the omnipod read the Interstitial fluid , as part of the pump.
    In Australia we use mmol/L like UK, and if its not too much trouble would you be able to put on the video's in the corner , what your readings are.
    Thankyou Rex ❤️
