After surgery, the extra was left permanently brain-damaged and paralysed on the left side. Additionally the left eye had to be stitched shut. To clarify, no injury happened during the filming of the car hit by the plate in The Island (on set the plate was fixed to the ground, and they remotely controlled the car to drive into it).

Some of the clips from onboard ships and fleets I understand reusing as it is incredibly difficult to get access to basically have an entire fleet film one shot as well as the personnel to do it

I never had a problem with it in the first three films. All reused moments where so brief and cleanly edited that you'd barely even notice them. But it seems like in AOE and TLK they got really lazy with it. In The Last Knight they reused the shot of Megatron hitting the submarine from Revenge of the Fallen and you can very clearly see Megatrons old design in frame. It really felt like they just stopped caring after Dark of the Moon, at least IMO.
Michael Bay is one of Hollywood's best Action movie directors but I wish he never did those Transformers movies because it just wasn't for him he should have done Bad Boys 3.
