It wouldn't have to be a retcon though. Many men have conditions (e. g. low sperm count, or the sperm being slow) that don't make it impossible to get someone pregnant, just more unlikely/difficult. So, especially before IVF became a thing, many couples just kept trying for years and at some point, it worked. We don't know how long Fred and Serena were together before Gilead - maybe they got married very quickly after meeting (which would make sense given their evangelical/fundamentalist backgrounds). Maybe they "only" tried for a few years. And then within Gilead, they would've been given a handmaid pretty quickly - at which point they stopped having sex which each other. Then getting pregnant after the spent one night together in season 3 is maybe not super likely, but it's not impossible at all. Fertility is WEIRD.
Yea it just doesn't stop with being fertile. Some people's dna just doesn't work together aswell. But it doesn't mean you can't get pregnant. My aunt tried for 13 years until they got a child

So it was heavily implied that he was sterile, but this is an interesting development :D

This scenario would be very interesting because of the moral - let`s say - ambiguity. This society punishes handmaidens with the colony if they do not get pregnant early enough and commanders who do not impregnate their maidens get the wrong kind of attention from their colleagues. So if the commanders or some of them are sterile which one of the docs said to June during a check up when he offered his assistence, then it is a hopeless situation, in which every woman and man has to betray and lie to protect themselves. If the maidens do not find a fertile man they get send to the colonies, but they might get caught with the other man and end up hanging on the wall. The commanders get scrutinied if the cannot impregnate the maidens so they have to hope that the women find a solution and they themselves have to turn a blind eye and in case of success they have to live the lie that it is their Baby from the start.
