Interestingly, Donna who was from Pride Louisiana was perfect for the part of Ellie May, but was diverse enough actress she was able to do western american accents, such as the Twilight Zone you mentioned as she found it a necessity to land parts. Once she landed the part of Ellie May Clampett, she was free to use her own natural southern drawl.

(1) the medical procedure was her 11th, not the 7th; (2) she thought she was ugly; she did not think the other people were ugly; she wanted the medical procedure to make her look like them; she was afraid of them because they were going to ship her into exile, not because she thought they looked scary; (3) two actresses played the role: the actress in the bandages was Maxine Stuart (42 at that time); the actress after the bandages were removed was Donna Douglas (28 at that time).
