You just have to aim it forwards like swipe your q towards the enemy after you use e, if you press it it will not work I think. If you play the video at slower speed you can see I aim it towards the Annie
Blade of the ruined king, divine and mortal reminder are most important. Other items are not as important but bloodthirster and twinguard can be good last. Just make sure you have giant slayer as a rune and you will be fine
