I've seen plenty of foot specialists channels but man, none of them handles them tools like you do Dr.Jefferson, you are a real specialist and one can see the years of experience you have when you handle your tools. This patient is lucky to receive your attention and concern.
You subscribe to The Toe Bro? He is a chiropodist, not a podiatrist, and lives in Ontario, but is a good educator, has interesting cases, lots of empathy and mad skills like Dr. Jefferson.
That looks so painful! It seems like the home health nurse should b checking his feet everytime she sees him since he's prone to these and a diabetic. 🤷♀️
@@fuzzylop9164 I'm not saying mess with it but they need to b checking his feet as well. They're hired to come into their home and check on their patients so to me that would include his feet. Idk..I'm a nurse and I don't get how his feet got that bad with someone looking after him.
This aide should be sent back for additional training to identify potential ulcer problems for the client that would need to be reported to the nurse or physician.
Great job....Everyone please check on the elderly....I volunteer at a Senior Facility, I clean and paint their nails and toenails. I'm not a doctor, but I do report any potential problems I see occurring with my clients. I just want everyone to understand how satisfying it is to help others.
As a former home health aide in NYC, I can say that we were thoroughly instructed as to our limitations with patient care. In this case, we would have only been able to INFORM the visiting nurse about the condition of the patient's feet, because they were the only ones authorized to clip nails, etc.. We had to follow the care plan given for each patient. So legally, the aide can only do so much. It also depends on the laws of each state. Now the home health aide CAN accompany patients to medical appointments, such as the podiatrist. Home health aides can also assist with bathing and applying topical treatments such as lotions and ointments. But the bottom line is that the PATIENT and/or their primary caregiver has to stay on top of the situation. They have to take responsibility for their health and be as proactive as possible.
I remember the first time this gentleman came to see you... That callus was like cement!! I like your approach this time around.. Instead of chipping away at it slowly, you took the whole thing off in one swoop! Nice!
It’s too bad he doesn’t have a family member who can check to make sure she is doing her job. I took care of my sick father at home until it got to the point that I didn’t have the physical strength needed. I did not want him in a nursing home for the reason you gave. I tried slipping the nursing aides cash in an attempt to get them to pay attention to him, but that didn’t help. I visited him almost every day. I felt sorry for the patients who had no one visit. Those aides didn’t pay much attention to them. And Hospice was no better. I found out they ordered a hospital bed for Dad 2 weeks prior. It still had not come. I got on the phone with them and raised hell. It was 10:30 on a Friday night. They tried telling me nothing could be done until Monday. I raised more hell. Finally around 1:30 they brought one in. Dad died the next week.
He's stubborn and admits to drinking too much. Nurses and even doctors can't make you take care of yourself, much less a home health aide. They're not nurses, and the patients aren't children, adults need to take care of themselves.
Most diabetics still have full feeling in their feet. But yes, for those with some degree of neuropathy, they or someone who can actually see "down there" should be checking on a frequent basis.
My mom is a diabetic. She lives with my sister who checks her feet daily. She's got a dremel tool and files her toenails and moisturizes her feet. The thing that gets me is that her PCP has never once checked her feet nor recommended her to a podiatrist. It's only by watching these videos that I've learned how important foot care in diabetics is. So I thank you for putting these informative videos out.
It IS her job to bring or accompany him to doctor's appointments! She should have taken that convo outside if she couldn't come due to time limitations. It sounds rude otherwise!
I am hooked on Podiatry videos but you Dr. are on another level! Straight to the source with the right tools...In my eyes you are the best Podiatrist on UA-cam and perhaps in all of the USA. I just love that you give a foot massage at the end. Nobody does that on UA-cam! Why not? It makes the patient feel relaxed and the feet look 10X better...God Bless you Doctor and keep up the wonderful work! Greetings from Canada....🙏🏼🍀🧡👋🏼🇨🇦
people need to realize A LOT of these “home attendants” really have NO! training whatsoever or no sense as to WHO? to inform if their client is having problems! A LOT! goes into HOW!? they’ve been trained.
You don't need formal training just common sense especially for a diabetic patient its worse for them they can loose limbs and potentially their life wake up and care for others how you would yourself! Period that's the bottom line. One quick assessment and cream or cleaning the wounds. Im sure it was in his chart that he's both diabetic / with previous ulcers on the feet which or pressure points . That's the most important to pay attention to.
It’s good to see you being firm with this patient. I don’t think some people with diabetes take that condition seriously. Like you said, they become complacent. It’s a good thing you are a great doctor. I’m sure you have saved a lot of people who just didn’t care to follow your instructions. Thanks for an informative video!
Dr Jefferson I admire your ability to slightly change your pattern of speech to speak with each of your patients. Many doctors fail their patients because they are either unwilling or unable to interact with their patients in order to communicate effectively. I enjoy watching and listening to you work. Thank you!
You are so good at treating these terrible problems Doctor. You are also so good with your patients. Thank you so much for keeping us your viewers informed about things that are so important to know about. ❤️

Thank you for this interesting case. Also a big thank you for making people aware about what a nasty disease diabetes is. Lots of diabetici are not aware of the severe complications it can cause in the hands and especially in the feet. Ulcers because of pressure or wounds on the feet can easily lead to amputation of toes, a part of the foot or even the whole foot. This can easily be prevented if patients inspect their feet before going to bed. It cost nothing and it can prevent a whole lot of misery and pain. I hope the patients ulcer will heal. He may benefit from orthopaedic soles in his shoes to take away the pressure points.
Unfortunately, most type 2s anyway are never taught anything about their disease. Nothing. Unless they want to get educated and they're readers, they don't try to learn, either. My experience is that primary care doctors are good at diagnosing diabetes, but they know nothing (ie, zero) about the daily management of either type, T1 or T2. AND add to all of that, if someone has no health insurance, they are really up a creek and will never be able to afford the medical care, testing supplies, or meds they need. It's a problem with many ramifications. And you're right, diabetes can be nasty. OTOH, it's possible for a diabetic in good control to actually be healthier than their non-diabetic friends and family-- they eat better, exercise, watch their weight, and generally speaking just take care of themselves better. Hard to make generalities.
I agree but I'm not sure how old the Gentleman is but if a Aide is being paid to care for this man it is her responsibility to see he is keeping up on his appointment's. Sometimes when we are sick we want to be left alone but your family would make sure you are cared for. Knowing his past history of this issue causes great concern cause it could lead to bigger issues later. Just curious though of a Aide is hired doesn't she or he have to report the concerns to her bosses? Or is this a under the Table kind of Aid. I just know my heart 💔 for anyone who is not getting the proper Care from a caretaker. Doctor Jefferson is a great Podiatrist and shows great concern over his patients. Sure some would say well it is job and a paycheck but you can see in his actions band tone of voice of how much he cares about his Job and the Patients he takes care of. Wish I could find that here in my State.
Robin Barbour By law, CHHA, companions, aides, cannot cut, or clip anything. And if the patient/client is of sound mind.. the aide has to wait for the patient/client to agree to go to an appointment.
@@michellelangschied9255 I agree with you, it's no telling with regards to the aide. She could be an actual paid worker or someone they may personally know/under the table kind.

That said....We were so pissed we moved mom, after many independent, vibrant years... in with my sister. To home care her at the end of her life. She died at 95 at peace, with love surrounding. Outstanding video Doctor Jefferson!!!! You do such as amazing job and you are terrific with your patients. I wish they cared as much about themselves as you care about them. That must get frustrating. But being the kind and patient doctor you are, you just tell them all over again what they need to do to prevent it from happening again. You are so good!!!!
Very informative and educational lesson on diabetic feet issues, I hope the patient understands the importance of keeping up his appointments and taking care of his feet.
Not me. My worst fear is blindness, by far. Kidney failure is #2. Long term diabetes is not for sissies, however it affects somebody, though, for sure.
Your first-ever is very informative. Education of the public through U-tube videos will hopefully make for awareness of potential problems. Keep up the good work Dr Jefferson.
Makes me wish we could pay and train home health aides more! Plus I guess a lot of them are part time and have high turnover; makes it hard to learn everything that might come up.
Goodness gracious! Pay attention to your feet, folks. Home health care professionals need to look at these patients at least two times a week to see what’s going on. He’s blessed to have such a patient and skilled doctor.
It just amazes me how they don’t know how big or that they have a ulcer. I need to be tested for diabetes which is very frightening. Mine is due because I’m obese weight. Watching these makes me diet better. So Dr DC you not only help ur patients your helping me to make a difference in my lifestyle because of ur films. I’m in Aussie so we don’t have anyone like u here. So I would like to Thankyou from the bottom of my heart for showing me my future if I don’t change now. 🙏🏻🇦🇺🤗❤️
Doc you really are a fine physician! I would be so tempted to lose my temper (at least it would show on my face) when people come in and have not been doing what you’ve told them to do (I’m not talking about this patient in particular but rather all patients) and, as a result, continue to have problems. But you stay calm and even tempered as you try to guide them to do the things they need to be doing to maintain good healthy feet and toes.
I’m so glad he is in your care. This could’ve been so much worse within just a week or one month. Thank you doctor for helping him heal and explaining what needs to be done. You are one cool, 😎 well knowledgeable, caring doctor. You’re the best, and definitely the coolest😎. Bravo! BTW, I like the image in the corner. 👍 I don’t think you’re that gray on our chin yet, but it still looks really amazing! Lol😊
Prayers for this gentleman; thank God he is receiving care from one of the best♡♡ Bet you couldn't find many people from the current generation that would work their feet, literally, to the bone and keep on working. Beautiful work Doc, thank you for sharing
I'm surprised it doesn't hurt, I'm sure if I had a wound in/under a callus, it would hurt a lot more, this video tells me to take care of my feet, Dr.Jefferson is pretty awesome at what he does
I don't even understand how this man could stand the pain that that is causing him. It is such an awful wound I'm so glad that he saw the DC doctor because he is a great doctor and he is caring and he will heal that foot if the patient follows the doctors orders. But, I really feel for this man. Take care of yourself and may God bless you and your doctor.
I'd rather have a few minutes of the "tickles" then the rest of my life without my feet. Well done Dr. Must be frustrating when patients are non compliant
I am very happy to see these very educational videos. Makes me aware of how much more I should watch my health! Thank you DC doc, for making these awareness videos!
Everyone needs to take a step back on this care aide he has with him. You don't know the situation with what she is hired to do & how the patient responses. My mom has had people in before, and they are hired for very specific tasks, like checking a wound vac, or even just blood pressure/blood sugar. They are not there for full body care, in a number of case. A lot of the time, she never had the same person twice. I lived with her for 2 years and i couldn't get her to go to the doctor for things, because she was too stubborn. Don't comment unless you know the situation at his home.
Lots of calus build up for ten weeks. Seems to me if he is predisposed to these type of ulcers the aid should check his feet at least weekly. I know I do my husband's, already six surgeries due to ulcers. They can go from pea size to silver dollar size almost over night. Great video
Touché touché touché to you podiatrists. I could NEVER. just the smells alone, how the smells must be atrocious. So many don’t even come to their appointments having washed their feet. Poor Drs. God bless ya’s
Wow from healed to an ulcer. So many diabetics don't take good care of their feet until major problems arise. I have seen so many diabetics who have lost toes and even a leg because they either are lax on seeing their GP or just don't keep an eye on what is happening to their toes/feet. BTW. I like the voice over explanations you give. Did anything grow from the culture? Also good blood flow was a bonus.
When we get older, we really have to get over ourselves and ask for help, ask someone to take a look at our feet, if we can’t reach down and see for ourselves, especially if we are diabetic! It is our feet, our responsibility.
Yes, but people get mad st their bodies for letting them down. It's odd but the phrase really is "cutting off your nose to spite your face" for a reason. If we have medically fragile individuals in our lives we need to check them over.
When you get over 75 or so, it can get more and more impossible to "get over yourself". Ask? Expect decent care? Or even to remember to ask or expect decent care? That's beyond a lot of older people's capabilities anymore. Oldsters are often like small children-- you wouldn't expect someone under the age of 5 to get over themselves and "take responsibility" for themselves--- it can be the same thing, for many older people.
Wow that’s one tough callous hate to see those wounds hiding underneath them. Hope you can get him lined back out. At least he’s in good hands. Thank you doc

It's 1:58a.m.in Gadsden, Al. I am watching you Dr.Jefferson. Great job as always. Thank you.
This is a ugly foot problem. Thank God the doc can help him. A big great thankyou to the DOC. ✨✨✨
IloveMarkHarmon jethro 😂
Such great care and compassion. Thank you Dr Jefferson for your beautiful humanity!
This doctor does amazing work for his patients.
Very informative and educational lesson on diabetic feet issues, I hope the patient understands the importance of keeping up his appointments and taking care of his feet.
I'm diabetic, this is my worse fear.
Not me. My worst fear is blindness, by far. Kidney failure is #2. Long term diabetes is not for sissies, however it affects somebody, though, for sure.
You are such a caring doctor , you always give your patients advice to take care of their feet 💕💕
Todd to know he was having bloodwork done. I always enjoying watching you with your patients, Dr Jefferson. Thank you.
So happy I’m back in the country to watch these videos I missed in the last 2.5 weeks
Great job
Thank God for caring foot doctors. Keep doing what you do best. Happy New Year and to all the fellow foot watchers.
Your first-ever is very informative. Education of the public through U-tube videos will hopefully make for awareness of potential problems. Keep up the good work Dr Jefferson.
poor guy. he really needs help. thank you for helping him.
I’m Diabetic and seeing my Podiatrist today. Thanks Dr. Jefferson. Like you say: Take care of your feet.
Makes me wish we could pay and train home health aides more! Plus I guess a lot of them are part time and have high turnover; makes it hard to learn everything that might come up.
I love these types of videos it’s satisfying
I have to give it to you Doc, you’re very thorougher! Great work as usual.
Why am I watching this at 3am 😂😂🤷🏾♀️
I'm asking myself the same question, lol
I'm watching it at 2 am UK time x
NetaBell we all are.
Same
No clue
I’m so glad he is in your care. This could’ve been so much worse within just a week or one month. Thank you doctor for helping him heal and explaining what needs to be done. You are one cool, 😎 well knowledgeable, caring doctor. You’re the best, and definitely the coolest😎. Bravo!
BTW, I like the image in the corner. 👍 I don’t think you’re that gray on our chin yet, but it still looks really amazing! Lol😊
I like when you voice-over in the middle of the treatment after the video is shot.
Prayers for this gentleman; thank God he is receiving care from one of the best♡♡ Bet you couldn't find many people from the current generation that would work their feet, literally, to the bone and keep on working. Beautiful work Doc, thank you for sharing
He needs another aide. His health is in jeopardy. Shame on this lady!
I am so glad this gentleman receive excellent medical treatment for his feet. Thank you doctor!
DC Doctor you are just amazing!! Helping so many people in need❣️
That foot ulcer is a nasty beast. This is a great public service to tell people health is important.
Great video
The home health aide is incompetent! This is inexcusable! Thankfully he came to see you before it got gangrenous.
I can learn a lot about taking of myself and others by watching your videos. I love them.
Wow! That came back pretty fast! I remember this patient!
Happy new year doc
That wound looked really painful thanks fir doing voice overs to explaining other aspects
Now that's a straight to DVD title right there
When we get older, we really have to get over ourselves and ask for help, ask someone to take a look at our feet, if we can’t reach down and see for ourselves, especially if we are diabetic!
I’m going to be a podiatrist in my next life.
Now,that is a BEAST of a callus right there.
Wow that’s one tough callous hate to see those wounds hiding underneath them. Hope you can get him lined back out. At least he’s in good hands. Thank you doc
Once again Dr your awesome ! I just love when I see you posted another video your so good with patients.thanks for all the hard work you do!!
