Fairly common. I used to take the bus to work for 3 years and twice we the trans failed and 3 times it was another fault. Worst was I just got picked up and the bus broke down just after the next stop. He pulled fuses but it would only go about 5kph. No 1st or 2nd. Why the worst? Because where we stopped was between stops and could have jumped on the next bus that went past. Really annoyed they didn't stop to let us board. 30 mins later another bus came and did stop but it wasn't express. So it took 1:30 mins to get to work, plus waiting 30 mins for the bus. Thankfully my boss didn't care that I was over 2 hours late.
I remember when TTC ordered the 1991 Vs and they initially came with the ZF transmissions and they constantly were slipping. Then they tried to modify it to where they start out in second gear and of course it blew the transmission. So then TTC retrofitted them with the Allison B400Rs, I believe.
As a bus driver here in Australia, I've had that happen to me, twice, was funny the first time since it was an easy fix, the 2nd time, not so much, 2 hours waiting for a tow truck while my boss was on the phone to me
sounds like he blew 4th, I've been in buses in my country with Allison gearboxes that have blown 4th like that, the bang will scare the crap out of you indeed
if they were struggling to go 5kmh it probably was not 4th, unless somehow a failure caused it to stay in 4th and then it burned up quickly thereafter.
