  • We definitely need to be tracing the contacts of people who have tested positive for coronavirus, so that we can know who else has been exposed. This is a herculean task, but we have to do it. And there are some ideas and practices that can make this easier.

    I came into close contact with someone who subsequently tested positive 8 days ago. Let's call that day one. My symptoms started on day three. When I called the doctor on day four. She recommended I don't come in for testing until day five because they wanted to have symptoms for at least 24 hours before testing. I received results on day seven. I am positive. And I'm now fully recovered. Today it is day eight and I still have not been contacted by any contact tracer here in northern California. If my experience is anything to go by this pandemic will continue pretty strong here in the US for a long time.

      You only had symptoms for four days? Were they mild? For some reason I thought this thing took people out for longer than that, but I don't know anyone personally who caught it. Of course, I say "only" four days, but if you feel like Hell warmed over, that's certainly long enough!!

      wow, here in Singapore we only count patients as recovered on Day 21 of illness.

      Glad you are better. Have you been asked to quarantine for 14 days from the onset of symptoms? That's what they ask us to do in Canada, unless you are a severe case. They are finding out that people who seem to recover fast might shed the virus longer than that 14 days though, more science needed there.

    It’s definitely odd watching this from NZ, the oddities that are produced by the US health care 'system' seem to increase daily.

      Most of these oddities are produced by the fact that our President was put there by Vladimir Putin. This whole episode was about what can be done GIVEN THE FACT THAT the federal government has been paralyzed by what is essentially a foreign-sponsored coup.
      I think it would have been better if the video had started by explicitly stating that the federal government is barely able to do anything positive due to interference from the corrupt party that is funded by Russia, and that the video is made under the assumption that WILL NOT BE REMEDIED in this calendar year.

      @@ahgflyguy Honestly, when it comes to response, yes, I can see how Trump and his administration is the problem. But(as someone from EU, bear in mind) can see how a lot of what is happening in US is direct result of "How the system is built", mainly - Private healthcare, healthcare tied to employment, very scarce social netting, no mandatory sick leaves, autonomy of states, etc.
      In Europe, rent freezes were implemented in a lot of countries. The unemployment benefits were available, workers that felt sick or needed to get tested simply took time off(no positive tests necessary), pretty much everyone, even unemployed and poor communities could get tested and get decent healthcare for free. Hospitals, although shortages were real, had stockpiles of ventilators and supplies, which isnt really profitable in for-profit just-in-time supplied hospitals.
      Some things could be blamed on current administration, but in the middle of March, when Europe was beginning to close and US still had low number of cases, I was already saying that USA is super fucked. American hard-capitalism approach is just not able to handle crisis like this.

      it's the worst when you've seen this coming for years and do everything to advocate for a better system and to vote the people out who propagate it and there's nothing you've been able to do to stop it

      It's obviously not an IDEAL way to handle the crisis. But I REALLY think that with aggressive testing, aggressive tracking and tracing, and aggressive mask-wearing, this could have been handled well and kept under control. It still could be, if there were a leadership change today. We have the ability to pay for the tests. We have the ability to require masks. The orange buffoon is standing in the way of both of those things intentionally.
      In an environment with adequate testing, our Congress could easily pass a paid-leave-for-COVID bill, effectively granting paid leave for this particular sickness, and that would further encourage people to get tested if they become symptomatic.
      Our structure is bad, but that doesn't mean that it can't adapt quickly in times of threat, especially if that adaptation is advertised as short-term.

    I think some of these interventions are relying on something we don’t have-cooperation of the general public. We can’t even get people to wear masks in close proximity. Do we really expect people to self-report to a database?

      The thing is, SOME people are wearing masks, and SOME people will be willing to self-report. We don't need 100% buy-in. More is better, but anything is better than nothing.

    In Germany, when a county has more than 50 new cases per 100K people in one week (35 in Bavaria), the area goes into lockdown. I suppose that works best to stop outbreaks locally though. I'm not sure about the US where it seems to be everywhere. At least that's what it looks like following the events there from Europe.

    I'm interested to know what the HCT team think about the recently published PREDICT 2 study and the previous PREDICT study about personalised responses to food and the implications on nutrition advice. I know it's different to the more pressing matter of Covid-19 but it could make an interesting HCT news video. Still loving the channel and look forward to next weeks vid!

    Thank you, Dr. Carroll. There are so many issues that health departments are facing right now for covid surveillance. At mine, we are so behind in creating PUI's on our electronic surveillance system (2-4 week lag for much of our data), which puts the case investigators and contact tracers behind, making efforts seem pointless. We desperately need COVID-19 lab reporting to be harmonized and fully electronic!!
    We also have an issue with the lack of Spanish speakers who work in health departments. The Spanish speaking population is a large portion of our caseload, yet we have trouble reaching them because there's so many and some PH workers don't want to spend extra time using a translator service. This means that our highest priority/most affected communities are being left behind (what's new?). I'm so saddened by our lack of resources and organization. The base issues here are lack of funding and pandemic preparedness.

    I commented this on the last video but ima go ahead and say it again :) I think it would be great if you made a video on unexpected/unusual covid related symptoms, especially skin conditions. I was never sick with the respiratory symptoms but I’ve been getting giant hives on my legs for months and my doctor believes it is an exaggerated immune response triggered by exposure to covid. I did some reading and apparently a large portion of patients experience skin rashes or swelling (think covid toes). I’ve never had serious allergies in the past and my skin isn’t particularly sensitive, but since April I consistently get huge itchy welts that last multiple days and have no other identifiable explanation. I bet there are many people experiencing medical problems that could be due to coronavirus but don’t have an obvious link, since the virus mostly attacks the respiratory system but is able to get all over the body. It would be a very interesting video!

      You may have eaten a food that you have not eaten before. A new medication could cause an allergic reaction. Hives are usually an allergy symptom.

      Helen 2 yes it could definitely be an allergic reaction, but since it’s ongoing and I haven’t identified anything I’m allergic to, I think it is more likely an immune system disruption

    This sounds very similar to what is going on in Israel (tracing, surveillance, self reporting, etc) though we've had a rise in new cases over the last few weeks after lowering new daily cases to under 30. It seems people felt that now that things were opening up slightly that they didn't need to care anymore.

      @@Medhead101 we never had a true lockdown ever, besides passover eve (imagine Thanksgiving in the US) to prevent any large gatherings. And with a total of 306 deaths and managing to bring down the total active cases to under 2,000 I'd say that it was pretty effective. It wasn't even that hard.
      The reason there's a spike now is because people aren't properly social distancing or wearing masks.

      @@Medhead101 Yes, look at Sweden. One of the highest infection and death rates in Europe. Higher than Spain, higher than Russia. And their mortality rate? Five times higher than Denmark.
      What point are you trying to make? That you can kill a lot of people if you pretend there's no epidemic?
      And their economy? No better than their neighbors who did a better job at protecting the lives of their people.

      @@steveh46 Sweden did similar mistakes to NYC by sending sick patients to nursing homes and that's why their deaths are inflated. However not locking down their society was much better than locking down. It's about peoples rights and the psychology of people not giving away their freedoms too easily to a government that would happily keep taking them away.

    Thank you for listing out the reasonable forms of contact tracing. The only one I have a slight issue with is closing specific communities. Are these communities based on area? Such as closing downtown New York for example? Or are they based on type? Like closing down all food establishments. If it's based on type then I have a problem with that. Because it could be used as a tool to control and harm certain communities. It wouldn't start out that way, but it would be very naive not to realize that malicious people would seek out a position that would allow them that power, and would use it for their own benefit at the great expense of those communities effected. I guess the same applies to areas now that I think about it. I guess my point is we need to be very careful with something like this. Because the fear and economic control could and will most likely be weaponized.

    [edit: you gave this a brief mention as "smart thermometer use" without explaining why that might be significant]
    One thing that struck me was recent mention of an internet-connected thermometer that tracks patient temperatures and gives good predictions of where covid outbreaks are about to hit. From the article it appeared they were merely collating recorded temperatures. I suspect that they may also get useful information from how often a patient uses it. It's the sort of thing that may not be used until the patient feels a tad under the weather and checks their temperature. Covid can be present without a fever, so tracking measured temperatures wouldn't show that individual has a problem (not with temperature). But increased usage would indicate the patient has symptoms other than temperature that are worrying.
    Reasoning: Google has previously found they could track flu outbreaks from the increased number of queries concerning flu symptoms from a given area. People wondering if some sign they're showing means they might have flu. The same would apply to people taking their temperature more frequently. For an individual, it might be a fluke but aggregate the results over an area and a lot of people checking their temperature more frequently is a sign that something may be wrong.

      there’s lots of discussion! but sometimes it’s necessary to sacrifice a little bit of privacy for public safety. America is not recovering from covid due to this selfishness and entitlement, being opposed to anything slightly uncomfortable or inconvenient (masks, surveillance) to curb a global crisis is insane. the government potentially finding out where your ass goes grocery shopping should NOT be your biggest concern when people are fucking dying. sorry for the harsh tone but the orwellian argument is immature and incredibly selfish. I can guarantee you that any information on google and facebook accounts is more sensitive to exploitation than a contact tracing app

      A) They're voluntary apps
      B) The NSA, FB, Google and Apple already know more about you than your mom or partner, with very little privacy oversight or law restrictions in the US.
      This is a huge, HUGE problem in itself. I just always find it weird that people get upset about these apps when we've all been cyber-stalked for over a decade now.

He wore a mask around those who didn't in public. He bitched about them to me. My very healthy BIL died this morning from covid 19. Screw your "freedom".

    This may be the wrong place to ask this, but I haven't heard an answer anywhere else, and I'm not sure where else to ask.
    Obviously I wear a mask when I have to go out, and avoid touching my face, et cetera. But everyone says that you should avoid touching the outside of your mask, even when you arrive back home.
    I get that it's been exposed to the outside world, but how is it any more contaminated than, say, the front of my shirt? Why specifically am I meant to treat the outside of my mask as especially bad? Nobody says to avoid touching your shirt, or anything like that.
    I'm doing it anyway, because people smarter than me all say to do so, but I can't make any sense of it.

      You breat air in during your mask and (hopefully) the aerosols (viruses particles, dust, cigaret smoke and much more stuff) will be held back by the outside of your mask. So if you touch the mask you will get a high concentration of unwanted stuff on your fingers. If then you do rub your eyes exidently you will deliver all the stuff that you have held back from your lungs into another "door" into your body.

    Question: we're getting better at keeping severely ill COVID patients from dying, but there is also growing evidence that even a mild infection may cause long term health problems - some kind of post-viral syndrome like what some suspect causes Chronic Fatigue Syndrome. Is there any research into this post-COVID syndrome, especially the prevention and treatment of it?

    Could you do a video on masks? Joe on It’s Okay to Be Smart said that you should be able to blow out a flame through your mask, but other sources say that you shouldn’t. How is oxygen supposed to get in if a mask actually keeps you from exhaling germs all over other people?

      Well, it depends on the type of mask you are using, and what type and size of particle you want to block, as you may know, the weave on a surgical mask is different from that on an FFP 1, 2, 3 or 4 mask, and that the size of oxygen molecules are much smaller than viruses, which are much smaller that bacteria, which are much smaller than some dust particles which are much smaller than droplets from the mouth or nose. Plus there is the fitting aspect of make, surgical make are kinda loosely put on the face allowing crap to enter and leave from the sides, whereas a properly fitted ffp4 or n100 masks may block everything but relatively small molecules (i.e. singular viruses could be blocked by them). The more the mask blocks, the harder it would be too breath with it on, but the safer you would be, until the filter on it gets clogged up (as intended, instead of you breathing the stuff in). And that's an indicats that you should probs change the mask. I am not a doctor nor a mask expert. Pls refer to someone more experienced for advice on real life.

    This all feels like capitulation to our shitty lack of healthcare infrastructure, though I understand we might not have much choice in the short-term. It's embarrassing, however, how little infrastructure we have in place vs. the rest of the developed world and how little we support regular working folks who are most vulnerable to COVID spread and the economic implications of this pandemic.

      We're going in the wrong direction in places where governments went out of their way to loosen lockdowns and pressure people to NOT use masks. This is not a coincidence.

    Contact Tracing = I Don't down load or buy any phones with contact Tracing software built in.
    I don't accept any telephone calls from any numbers that are not confirmed in my address book.
    I've deactivated my voice mail so no one can leave a message.
    I do not accept or sign for any registered or certified mail I am not already expecting.

    Yeah, that’s great. And the odds of anyone doing this is the US in time to be relevant is ... what? Basically zero? Actually zero? It’s nice to have a list of things that could, in theory, be done but it doesn’t seem to actually be helping.

    I realize that the point of these videos is to talk healthcare and not politics, but when you're making a video that says "here are some of the things that can be done at the state and local level" but does NOT state "and we HAVE to talk about what can be done at the state and local level because the federal government is not just inept, but is actively being hamstrung by the efforts of Russian assets, such as the President and several Republican senators, among others" you're leaving out probably the most important bit of context: that the USA is under attack and has been for years, and COVID presented another way for that attack to be successful... and that working at the state and local level are the main means available to us at this time, make sure to f***ing VOTE".
    I personally think that ALL articles and video that are created that reference us being reduced to oddball one-off strategies because the federal government is not being allowed to act effectively need to explicitly state that, and state who is causing the attack.
    Not calling out the information attacks from russia, and the election attacks from russia, is another way to normalize living in a country where fascism is becoming commonplace. Surprise: the right-wing dictatorships are handling this worse than the actual democracies.
    Please say it out loud.

    Doing contact tracing using the traditional means is way too slow to be effective. As long as smartphones w/GPS have been dominate in the market, this work should be done automatically using the available tech. It is dangerous, irresponsible and stupid to do otherwise. The software for this work should do 3 main things. 1) force contact tracing only in times of a health crisis, 2) made such that the tech cannot be defeated as long as the phone is in use, 3) it needs to preserve privacy.

    Doing this work in this way will not only help to contain COVID-19, it will help to contain all communicable diseases going forward in the future. There are other laws that would help in this effort. Mandating that all phones, mobile or otherwise, contain GPS and contact tracing tech that cannot be defeated.
