Kirei knew about the battle continuation. Kirei didn't expect Cu to survive because of the nature of the Gae Bolg spear. Its curse is one of the most powerful death curses in existence in the Nasuverse, but it only triggers if the heart is pierce. It can even deliver true death to immortals and other nigh-immortal beings who wouldn't bat an eye at losing a mere organ like a heart. Heck, in an interview, Nasu once said that if someone somehow managed to pierce the heart of Arcueid with Gae Bolg, that even she wouldn't be able to get back up even though she's a being for whom the very concept of death doesn't apply. However, you'd need to actually be able to pierce her heart with Gae Bolg. My point is that the spear can give death to even the deathless. Battle continuation isn't enough to get back up. Heck, this is the reason why Kirei dies when his heart is pierced by Cu here. Kirei's heart doesn't beat. His heart is already dead and has been so since the end of the previous grail war. The reason he's still alive regardless is because of how the mud of the grail brought Kirei back to life.
