It’s like when you are closing the wound like the osteotomy ones and placing the device then sutures are given such that the appliance is not inside the mucosa.. and mostly they are placed buccally in case of mandible to avoid interference with tongue and they can be covered with acrylic for comfort of the patient and if patient is comfortable then they are only said to do activation of it.. now there are various types of distractor like external then internal ones are also tooth borne, hybrid etc so I will attach some clinical pictures in the next part of it so that you will get clear idea of it. Like we activate the orthodontics appliances so like that we actually do the activation of this also
