For several reasons, for one, they don't have the time. And in a way, it's a test. In the beginning they talked about only 6 episodes. But better 9 well written and executed episodes, than stretched out thin story.
Did everyone notice something with his interview? They bring Wentworth Miller interview him and they talk about the whole seasons and the new season that is coming out on March 2017 but in the two TV behind them the were showing Batman and not a Prison Break. Lol I know it was an interview at the San Diego Comic Con but at least they should put a Prison Break of any seasons if the new season poster isn't out there yet. If I am wrong please guys correct me.

When he had gotten an operation done, he died but he came back. He was having these outer body experiences or visions I'll say maybe even lucid dreams and he passed away in the middle of surgery but came out of it.
in the last episode of the 4th season, his nose began bleeding again (brain tumor), and then we see link and sucre etc. at his grave stone. in the Prison Break: Final Break movie, he died of electrocution, although it is not shown and we cant say for sure. so yeah, 2 times. but he's alive in season 5 so.... we dont know shit:-) can't wait for the new episodes though!
