If this method doesn't work....go ahead and install LCDP:FR for GTA IV 1.0.7.0 (A police mod...only for getting the scripthook work) and then install the turnsignal script it worked for me

If you already have GTA 4 1.0.7 and have installed the lcpdfr mod, all you need to do is copy the 2 indicators and copy them to the script file in your GTA 4 folder. love it if this works

i can't change button from left and right arrows, even if i assigned them to " , " and " . " but it keep turning signals with the arrows
