    The drive ribbon cable was plugged in to the controller card incorrectly at some point. Either backwards, or misaligned pins, because of unkeyed connections.
    It may have sent voltage down the wrong lines. Check for burns on the bottom of the board too.
    That socket should be replaced, if it's not too much hassle.
    I tend to replace all single-wipe sockets I come across with double-wipes when I can anyway.

      Yup. I've been gathering that the last couple of days from multiple sources. It must have come to me that way, because I've only ever had it connected to my Apple maybe four times over 25 years, and I'm always very careful plugging it in.

    A blown 74LS125 is pretty common with Disk ][ type drives because the original controller card connectors weren't keyed. If you plugged it in with the pins off by one, you would usually blow the 125. I did this once while in college and actually blew a chunk of plastic right off the chip. I think I still have that chip in one of my electronics boxes. Also, I'm not sure what type of connector that you fixed is but there are some crimp connectors that are designed to bite through the insulation of wires. It connects with the wire inside and the insulation acts as a bit of a strain relief.

      said. I think the original owner did that to the drive, because I've only ever had it connected to my Apple maybe four times, and I'm always very careful plugging in Apple floppy drives because of that issue.

    I think my drive has the same problem caused by a similar burnt IC. I inspected it and it is very suspicious. :) I get courage from your video, and I'll try to repair it! (Sadly my IC is not in a socket, so I'll have to solder a bit, but I'll put the new chip in a socket at least!) :)

    If one replaces many IC's might want to invest in a pin straightner.

    Yeah, My UniDisk 3.5's eject motor gave up the ghost, and killed my IIgs. Smart on not plugging it back in to show what happened

      I was new to the world of Apple IIs, so I didn't know the eject mechanism could cause problems.

    My 5.25" drives keep blowing up. They last less then 30 minutes. I have replaced everything but the power supply. Can a faulty power supply blow up drives?

      It can, but that will usually break more than just the floppy drive. It won't hurt to replace the power supply with one from ReactiveMicro anyway, even if it doesn't fix your floppy drive issue. It's hard to make further diagnosis without seeing the machine and drives in question.

    I have a Apple IIe with two 5.25 drives. Is there any extra hardware that I need to use a 3.5 floppy drive on the computer?

      Aside from a drive, you would need a "Liron" or "SuperDrive" card. You can get a SuperDrive card from ReactiveMicro.

      Plug doesn't fit an IBM machine. It's certainly not MFM. Doesn't have a round connector for a C64. Can't hook it up to SIO on my Atari 800xl....
