    In today's video I talk through my personal journey trying both Fiasp and Lyumjev Insulins in my Omnipod 5. I can't stress enough that this is just my personal experience, and the more people share their experiences the better, so please, if you feel comfortable share your experience below and we can get a further conversation with all our voices!

    I responded to another comment directly but wanted to reply to the general thread to give more of an overview on my experience. I was using Fiasp MDI when I started on my first pump (the tslim). I asked my doctor about continuing the Fiasp after she said I’d have to go back to Novolog, and she said I could TRY it but not to expect great results but that she would continue to prescribe Novolog vials for the pump and Fiasp pens for me to carry in case of pump failure (and then I could test filling the tslim from a pen). Occlusion after occlusion was my experience in the tslim, plus some stinging (which I don’t really remember being noticeable on MDI), plus some site irritation. Also, I noticed that after a while (I forget but maybe a couple weeks?) I was using noticeably more insulin in total. I stuck with it for a bit and those numbers didn’t decrease. When I finally decided to go back to Novolog, boom - back to my usual ratios and total daily dose. So I decided Fiasp would in fact be just my backup insulin, not worth the trouble in the tslim. Then I heard about Lyumjev and that it reportedly worked better in the tslim (though still not approved). My doc happily prescribed it and my insurance denied it.
    After a while I switched to the Omnipod Dash to start using a DIY looping algorithm. I tried Fiasp in my pods too, with much better luck than in the tslim (zero occlusions) but still found some stinging and an increased ratio and increased total daily dose. I tried mixing it 50/50 with Novolog and that was probably my best experience other than just Novolog alone.
    But then I heard about Eli Lilly’s savings card! It is designed for folks like me (who have insurance that doesn’t cover their products) and I was able to get a paper prescription that I brought into a local pharmacy with the savings card and I got a month’s supply of Lyumjev for $35 (my usual copay is $30/month so pretty similar)! That was about two weeks ago so now I’m just two weeks into testing Lyumjev. I do think it’s a bit faster for me (definitely faster than Novolog which is what I was using most recently), but it also stings SO MUCH MORE. Like almost bee sting level with a larger bolus. I’m still very much in the “let’s see what I think of this” stage. I really want to like it because it’s notably faster than Novolog and it doesn’t seem my body likes Fiasp. My last few pod sites have been very swollen and red and my TDD has been much higher again so I’m unsure - maybe my body just doesn’t like these additives? The jury is still out.

    I tried Lyumjev and didn't find that much of a difference from regular Novarapid but the stinging for me was horrendous. Way too distracting for me.

    Hey Andrea, I started using FIASP the moment it became available in the EU market (Belgium in my case) years ago first with insulin pens and then with my pump (Medtronic 780g). I never noticed any stinging whatsoever... I will ask my endo to prescribe me Lyumjev to try and compare. With FIASP I noticed that once you are in a stubborn high, the correction takes a really long time to act and then suddenly your glucose is plummeting

      Interesting. I hadn't heard of Lyumjev here in the UK, but seems to be available. I MDI Fiasp (previously NovoRapid) and although very fast acting I still need to inject in advance - albeit a careful period, but too quick leads to peaking and too slow hypos with lengthy symptoms. Like you, I find stubborn highs slow to correct and then a sudden dive. I'm having a real battle to qualify to be on a waiting list on the UK's NHS to try a pump (Omnipod hopefully) to see if I can better manage variability by constantly micro-dosing rapid acting, and manage exercise better. I'm told my HbA1c of 53/7.0 is too good to qualify me, but I feel that my time is range is poor (only 55%) whilst I'm high (not hyper) around 30% - I realise a pump may not be the answer, and I may not get on with it, but the advent of closed-loop CGM and pump seems worth a try to stave off complications like pre-proliferative retinopathy that I'm suffering with after 27 years of type 1 (I'm 37) regardless of how reasonably good my control is.

      How interesting - thank you so much for sharing your experience!! I'd be very curious to hear what your interaction with Lyumjev world be. I had an almost identical experience to what you've described with Fiasp - it seemed to 'stack' more for me so those sudden drops were more fierce when they came (and definitely unpleasant to say the least!). I hope you're able to try Lyumjev and compare, maybe it would work better for you like it does for me! Thank you again for sharing and watching!!!

      @@ShesDiabetic I've found Fiasp can feel either fast or "slower" depending on circumstance, but often seems to keep things level for the first two hours, then either drop, or seems to run out and start to creep up quite distinctly. Of course, I'm carb counting and injecting rather than dossing as a pump's ongoing feed. I've tended to feel I need to inject correctively again after 3 hrs, despite risk of stacking, but sometimes do see it come back to roughly where I'd like in that 4th/5th hour, without action. So difficult! And very frustrating. Recently diagnosed with a "frozen shoulder" which I'd never heard of, but apparently metabolically linked to diabetes too.

      ​@@champagneshutterphotography Just sharing my frozen shoulder experience in case my experience can save you some frustration. My dominant arm developed FS and after about 5 months of dealing with it went to a doctor to find out what to do. I did PT for a couple months that didn't help and was told I could do surgery or just wait it out as the shoulder would "thaw" over time, usually 1-2 years on the long end. I just let it be and it took probably 4 years to get my full motion back, and 2.5-3 years to cease experiencing sharp pain with movement. During that 2.5-3 years I developed FS in my other shoulder, and my experience and length of recovery being what I had observed opted for surgery right away. It took 2-3 months of PT post-operatively (I continued exercises at home to the 6 month mark) and got all motion back. I am very happy I opted for the surgery, as my dominant shoulder did eventually "thaw" but it took an enormous amount of time comparitively to do so, and the surgical recovery was simple.

    Hi Andrea, I'm going to try Lumjev in the Flex Pens because there is a current shortage of Fiasp. I'll let you know how they compare with me. I've been very happy with Fiasp Flex Pens though as I have maintained an average a1c as a type 1 for the past 7 years of 4.8 to 4.9. Lately I've also been in range (65 to 140) approximately 95% so wasn't really looking forward to making a change. I use Lantus Flex pens for long acting. I'll definitely let you know how they compare in a month or so. Thanks, Jeff

    Nice to see that more poeple are using the ultra fast insulines with pumps. Until now I was thinking that was either rare but after this video and the reactions the oposite seems to be the truth. Happy for that!
    Before my Tandem pump I had the Medtronic 640G. I was first using it with Humalog but got not the results that I wanted. When Fiasp become in Belgium on the market the hospital recomended me to try that and my results were fast much better then on humalog.
    Then I switched to the Tandem pump. The hospital tell me that the algorithm was not designed on the ultra fast insulines so they want to try first with the humalog. The results went backward again so I asked again for Fiasp.
    They recomend me not to do with Fiasp was thicker and and less viscous what could be result in blockages with the tandem pump. There was a new insulin that had the same ultra fast acting as Fiasp and not this problems. So possible better for the Tandem pump. There were more reports on the stinging with this insuline. I give it a change and the result was very good. I had less blockages then with the humalog on the tandem pump, less stinging than humalog on the Tandem and got much better results. Even with the case that the Control IQ algorithm is not designed to that. Last time I went to the hospital I had a hemoglobine A1C of 5.0 and 93% of the time in range. And I'm very happy with those results. I can clearly much better manage my diabetes with Lyumjev on the Tandem pump. I installed also the xDrip application on my cellphone what was also a huge help and much better then the Dexcom app.

    i am using Lyumjev u200 , only available on quick pens. Don't sting . I am using a medtronic pump.

    My endo switched me to Lyumjev at fist it was very uncomfortable the pain every bolus but I switch to my arm and I don’t feel any more stink and i have notice huge different don’t have to pre bolus 20 min before meals love it so far..

      I've totally experienced different sting levels depending on the injection site! I'm so glad you've found an area that doesn't sting and that you're loving it so far! That is awesome!!!!!!!

    I have been on Fiasp for several years and now my pharmacy says the Fiasp is on backorder and dont know when it will be available Is Lyumjev possible more available?

      I have no idea honestly, but I feel like due to Lyumjev having a little less brand recognition you may have a better shot at finding it in stock. It's certainly a very comparable insulin (in my experience) so would be a good one to try should you get into a bind. Just my two cents! Good luck my diabuddy!!! 💙

    Thanks Andrea for this video. I also have tried both types of insulin and in both pumps, T-slim and Omnipod 5. I'm relatively new to type 1 diabetes and when I switched from Humalog to Lyumjev and experienced pump site reactions, I just figured that was the norm. Then, I tried Fiasp and was pleasantly surprised by the lack of swelling and itchiness of those infusion sites after the three days of wear. So now I am preferentially using Fiasp. I don't see much of a difference in onset of action between these two types of ultra-rapid acting insulins, but both worked way better for me than Humalog in terms of glucose control. I have never tried Novolog. I think every type 1 should give these insulins a try because anything that makes dealing with this difficult condition easier is a win!!

I'm SO glad you've found a winner in Fiasp in that you get that fast onset without the stinging - that is WONDERFUL to hear!! It's all about finding what works (and having the patience and persistence to try them - which clearly you have which is a stupendous trait!)     I too changed from Humalog to Lyumjev & have noticed a great decrease in BS after breakfast vs the +200 & not having to prebolus for meals. I’ve also noted some stinging, but very minor. I don’t have any experience w/Fiasp, so no opinion. I’ve lived w/diabetes for 56 yrs & still active w/EMS Fire service & pump therapy has made my life so much better w/A1C’s of under 6 for over 25 yrs. I’m interested w/Fiasp, jg my Endo doesn’t support that medication! Yes, still waiting on OP5 to work w/G7 !!

    I am really sad that I will switching from Lyumjev to Fiasp. The reason For the change is because the insurance I have now doesn’t cover Lyumje. I will start the Omnipod pump in two weeks. I will see how that goes 🙏 thank you so much!

    Interesting comparison. I had made a video about Fiasp actually, but it is of course worth trying both ua-cam.com/video/hYd2Pl0dJ4c/v-deo.html I think it is good for people to try different insulins so that they can find the best one. I sometimes wonder why doctors choose Fiasp or Lyumjev for their patients, given that both insulins, at least on paper, are very similar in action profile.

    I am on fiasp. My endocrinologist gave me a bottle of lyjmjev to try, and I liked it very much. However, my insurance doesn't cover it. Yes, it did sting/burn when going in, but being accustomed to injections for decades, it wasn't a big deal. I actually viewed it as a benefit because I could be absolutely certain I was receiving the dose. Using a tslim, I had to make some adjustments for fiasp to work. It gels and causes occlusions if it is exposed to direct body heat. I initially carried my tslim in my pants pocket and started having regular occlusions. Once I put my pump in a clip case that stands off of my body a half inch, the occlusions stopped. If my insurance ever covers lyumjev, I will likely switch. Until then, fiasp is close enough. Although, I would love the option of a u200 insulin, since it would greatly increase the number of days a pump fill lasts.

      Oh how interesting! I have heard about the occlusion issue - I've not experienced it myself but man oh man that sounds frustrating!!!       This matches my experience with Fiasp in my Tslim - so very many occlusions and I couldn’t get the cartridges to last over two days before it was just back to back occlusion. Keeping the pump away from my body heat enabled me to get to 48 hours instead of 24 when it was in my pocket. Also, I found that I slowly needed more and more Fiasp to sustain my usual control.
      I have since switched to the Omnipod (I use a DIY open loop algorithm with the Dash) and didn’t have a problem with Fiasp causing occlusions in that - but I still noticed that my needs slowly increased, and my sites were always inflamed. I maybe my body just doesn’t like it??
      My insurance doesn’t cover Lyumjev even with a prior authorization (though I’ve been wondering if a formulary exception might work) but I learned about the Eli Lilly savings card which is really what I’m commenting to tell you about! I was able to get a paper prescription for Lyumjev at my last appointment and bring it to a local pharmacy with the Lilly savings card on my phone screen. It requires you to HAVE insurance for the pharmacy to prove your insurance doesn’t cover it, and then they sold me a month’s worth of Lyumjev for $35! So this could be a good way for you too to try it out in the meantime?

    Great to see you back Andrea, and thank you for the really interesting video!
    I too had assumed they'd be pretty equivalent, but I think I'll try lyumjev next review. I get great results with fiasp, MUCH better than novorapid, but there's always room for improvement.
    Interestingly, while on holiday I started to suspect my long acting Levemir wasn't very effective, since I needed more Levemir than rapid acting fiasp every day. As an experiment I tried going without any Levemir at all, and just using Fiasp on MDI for a few days. I found that my body didn't really need any more Fiasp without Levemir, suggesting the Levemir isn't really doing much for me.
    I might see if another long acting insulin gives me better results.

    Thank you for the video. I'm on just about 4 years with Lyumjev. I have tried all fast-acting options except Fiasp, when diagnosed. The only insulin that was reliable for me was Lyumjev. I started MDI with Novalog then Humalog, and the results were very hard to deal with. I have been using the OP5 for over a year and after a few issues on the 3rd day my Endo got me approved for 48hr pod changes. I would say half the time I get a slight stinging, but worth it for me to deal with it. It all depends on the area, I've had a pod burn, then months later try near that site again and have none. It seems like the sites that have a little more fat absorb better and barely ever sting. Iam a leaner person, it makes it tough to find fattier sites. Lower thighs, and center of abs were 2 that seemed to have a bit more sting with some. Only one time did i have to change the pod because it felt like it was literally in a nerve near my belly button. A few inches above have been some of the best sites so far. I dose when I start eating 99% of the time. I also found that breaking up say 4 units into 2 2-unit doses, 3 minutes apart, makes a big difference for me. Less chance of going low to soon, and also tunneling/leaking at the site. If anyone has questions feel free to ask.

      I would like to try Fiosp at some point too. My insurance didn't want to cover it a while back, so left it go. I have seen a few people in the FB OP5 group mention it wasn't as effective as Lyumjev either. A few people said the mixing Lyumjev helped with the stinging, but not as "rapid" acting.

    Hi, Neither insulin is approved for Tandem pumps since their algorithm is based on the Novolog and Humolog insulin profiles. How often do you change you infusion site? Apparently, Fiasp and Lyumjev cause pump issues after 2 days. Those of us on Medicare are mandated to site changes every 3 days unless your Endo goes through hoops to get every 2 day changes approved. Hasn't happened for me for 10 years. I've been on a pump since 1993. Medtronic > Animas>Tandem. I really enjoy your channel and it's straight talk. Keep producing these quality videos :)

I still change my OP5 every 3 days and have not had any issues, but that's just me - I've certainly heard of people experiencing issues just as you've described. And I forgot that it's not approved for use in the Tandem pump - come to think of it I'm not sure it's approved for use in the OP5 either, but doctor's seem to ignore that from what I've seen.     Thank you for another great video! I have a couple of questions only related to Fiasp and it would be great if you could answer them:
    1. Did you have to change your bolus factors when switching to an ultra fast acting insulin?
    2. Did you had a higher risk of running into lows especially for meals that are rather slowly absorbed? On the contrary, I read that fast acting insulin doesnt last that long, so is there a higher risk for a late spike like 2-3 hours after the meal?
    3. Although you prefer Lyumjev was there still some noticeable difference between Fiasp and Humalog? If there were no Lyumjev, would you still switch to Fiasp instead of Humalog?

    Thank you for letting us know about Lyumjev as I have not heard of it before. I didn't tolerate Fiasp, but will ask my endo about this new insulin with a Tandem TSlim 2.

    I've had the exact same experience with Lyumjev vs Fiasp, and I've switched (usually because insurance forces me to) between insulins enough times to feel confident in evaluating their effectiveness in my body. With Lyumjev my basal rate is 1.1 per hour; with Fiasp I was having to go all the way to 1.5 and I was still having run-away highs. Fiasp also had occlusion issues for me that Lyumjev does not.
    The downside of Lyumjev for me is some stinging with, more importantly, skin issues at the site (like bumps and bruising). The result is that sites tend to "go bad" much more quickly than with the non "ultra-rapid" insulins. Still, I've tried them all and Lyumjev is definitely my choice. Also a heads up for anyone having trouble with insurance, Lilly has savings cards for Lyumjev that will apply whether you have insurance or not. My insurance won't cover Lyumjev, so I use the savings to skip my insurance entirely and pay out of pocket an amount that is no higher than my insurance co-pays. I find myself paying out of pocket for more and more medical expenses, even though I have an expensive monthly premium marketplace insurance plan. Go figure

    I’ve been using Fiasp for multiple years in a variety of pumps. My experience mirrors herrjohnny and glad to see someone else has the same experience with it. My endo is a bit resistant to having me try Lyumjev and this video makes me want to discuss again with her.

    Hi Andrea! Thank you so much for the extremely helpful videos. You have said that you are happy with the control that you are achieving with the Omnipod 5 and I’m just wondering when you say good control, what exactly does that mean? In other words where are you seeing your average blood sugar? Are you hovering around 110 most of the time? Are you able to get your blood sugar to run under 100 some of the time? My experience with the Omnipod 5 was that I definitely ran in the low 100s and I really couldn’t get the pump to consistently bring me under 100.

    I love, love Lyumjev insulin. Was on MDI for three years and had to use humalog. No Lyumjev cartridges. They did have fiasp but wasn’t covered under my insurance. Finally went back to using Lyumjev pens. Recently went on a tandem pump. Using Lyumjev even though not fda approved. Very happy. Took awhile to get settings straight. Use a lot less than when on toujeo for long acting. I swear never worked well. For me. Like the tandem so far. Don’t have to fish out as can bolus etc on my phone. Omnipod cost prohibitive on Medicare. Also had lots of site problems with the pod.

    Hi Andrea, I take Lyumjev and Abasaglar in UK, no punp. I find occasional stinging only depending on the injection site, it feels like the higher in my tummy the more likely it is to sting. But it’s fairly minor, not really a big deal

    Thanks Andrea, I ❤ your videos - I'm on fiasp since several years and after trying multiple pumps, I sort of gotten an acceptable situation with roche pump combined with diabeloop's dblg1. HbA1c now at 5.7 and it looks /feels fine for most of the time, but i still need manual boluses as the Algorithm does not calculate the proper insulin on board with fiasp. I cannot modify the activity duration of the insulin. But I will give Lyumjev a try.
    I hope one day we'll see an open source version of the DBLG1.

    I use Lyumjev in my Tandem T:slim pump. It sometimes stings a little bit when doing a bolus. Maybe in theory it works a minute or two faster than other insulins. But not enough faster to make any difference. You still need to pre bolus and counts carbs accurately and give correction boluses when your CGMS starts going up. I'm sure Lyumjev is better than the older insulins. But we still need insulins that start working instantaneously.

    I always find it so interesting that insurance companies rule one type of insulin over the other. For years I had been on humalog insulin and then suddenly my insurance company decided it only covered novolog insulin. I was told it was the same yet my body sure had a different type of reaction to it. I found my low's reaction were much more of a crash and I just struggled more when it occurred. Since I didn't have a choice I finally raised my low alert alarm so I could try to avoid lows as much as possible. Obviously health insulin companies see a diabetic as coming from the exact same mold as another diabetic and to them its all about cost. Welcome to the mighty USA. I thought about purchasing insulin out of pocket but was told I couldn't afford the price and they were right. I said the government said insulin now was to be highly regulated but was told only if you have insulin and I guess only certain types so one I guess just have to accept things. Anyways thanks for sharing your experience with these two insulins. Always good to know such a things

    Thanks for the video Andrea! I tried Lyumjev in my Tandem pump and had the stinging sensation but worse were my infusion site reactions! Most times leaving a swollen red sore and bruising. And I could barely go 2 days with an infusion site.
    I did like how fast it worked and how my blood sugar seemed more manageable but the sores and the sting were more than I could put up with Iand I went back to Humalog. I tried 1/2 Lyumjev and 1/2 Humalog but that didn't work out so well for me.

      Thank you so much for sharing this! And how interesting you did the half and half trial! I find that fascinating - but am also so so so sorry it didn't work for you. That is incredibly frustrating. I must say I do have some red spots when I take my site off and perhaps that's increased with use of the ultra fast actings (although the OP5 always seems to leave a mark when the tslim didn't really).       Because of my desire for an ulra-fast insulin I am trying Lyumjev in my t:slim for a second time and I will see how well I tolerate the sting and site inflation!
      I love how much control I get of my blood sugar and if I get a rise a bolus brings it down immediately!
      And that's probably worth the sting and irritation!

    This is the first I've heard of these new types of insulin. I definitely want to try one of these new insulins to see if I can get better blood sugar control. I will ask my endocrinoligist to prescribe this type of insulin for me when I'm at my doctor appointment next month.

    I never had a chance to try Lyumjev, but I did try Fiasp in pen form. I’m not sure if it would be different now with my pump, but with the pen I really didn’t notice a difference from the humalog I’d been on. It didn’t really work any faster before meals, didn’t change the way correction doses worked, and it didn’t burn. I’m on the tandem pump and they don’t approve the use of Lyumjev or Fiasp so (at least for now) I won’t be able to try Lyumjev to compare

      Interesting you had that experience with Fiasp - I kind of agree with you. It wasn't as stark a difference to Humalog (to me) as the Lyumjev was. A lot of people are commenting that these fast acting insulins aren't approved for use in the Tandem pump - and I actually am now wondering if it's approved for use in the OP5. This seems to be the type of thing that am aggressive Endo ignores and proceeds with anyway (in my experience) but it's definitely safer to stick to what's approved so totally understand that line of thinking!! Thank you so so much for sharing this all with me and all of us here!!

      @@ShesDiabetic that’s something that might vary from one country to another, but I’m pretty sure fiasp and Lyumjev are approved for OP5. If my pump was off warranty I would be more willing to try, if my endo gave me the go ahead.

      @@ShesDiabetic Let say that the TSLIM is not designed for those ultrafast insuline in the control IQ mode. That mode you can not define how long the insuline is active and is always 5 hours. That is the reason. In Belgium it is your doctor who decide what insuline is the best in your case and so they can decide that the ultra fast acting insuline is even with the tandem pump better for you and so subcribe you that insuline.

    I’m on fiasp and i feel it works so much better than novolog. It didn’t sting for me but im on pens not on a pump.

    I've been using Fiasp for about a year I love it, and no stinging here. Not sure how much you need to take to feel a sting but I've taken 9 units for a meal.

      I am interested in your experience; you didn’t mention if Fiasp were the only insulin you tried of this category.
      I read quite a bit before requesting a rx for Lyumjev instead of Fiasp. My first experience with Lyumjev has been poor; horrible site stinging and pain with no change in improving post-meal hyperglycemia. I am wondering if there is any significance in the fact that Lyumjev is manufactured by Lilly . . . I have been on Novolog for years and perhaps my body just prefers Nordisk products. I will return to Novolog I hope only briefly and then try Fiasp.

    I used Lyumjev for a few months as insulin pen, and it was much better than Novorapid. When I started using Omnipod Dash with lyumjev, it resulted in a painful experience and multiple pump failures (it just stopped providing insulin on a day 2 with three omnipods in a row).
    After calling Omnipod for instructions, they advised that Lyumjev is strongly NOT recommended for their product. Unfortunately Fiasp is not available in Israel, so I had to go back to Novorapid.
    The speed of Lyumjev is fantastic, I wish it could work for me…

    i trried fiasp a while back and it felt to be about the same as humalog/novolog so I just went back to those. (whichever is covered at the time by aetna)
