Deeply appreciate your presentation that gave a clear understanding of the problem. I have gone through ITA and it was not very serious. CT,MRI ,Holter, Doppler tests were taken. Specialist ruled out any adverse situation. Now now to avoid recurrence of ITA and full scale stroke. Age 82Male taking medicines for hypertension, diabetic but in reasonably good health
Good video, thank you. (Note: no mention of MRI. Our facility orders MRI/MRA before they even get a room in the ED. Which , in my opinion, is unnecessary and the wait period is too long for MRI to be available.)

But I want to communicate that for such types of Incidents or pt coming in such emergencies should be seen by seniors, who can decide to take bold decisions for pts good recovery. But usually all juniors are at the front line, they have no courage to take rt decisions or call seniors, understandings the gravity of situations n after effects, even pt reached in golden pd of with in 4hrs. Resulting pt to suffer with many debilitating conditions for very long time to suffer, including their wellwishers who suffer by loosing their jobs, time,finances. It is better if pt survives, recovers in good health conditions or may leave forever. Surviving in pathetic conditions is a very bad result, reflecting how life of human beings is respected, or all elderly pts has no place in this self centered world. Our health system needs innovative changes for better results. Thanks
My sister aged 60 yrs was arthritis patients suffered stroke with severe headache and partial paralysis of left hand only .About 3 months back she had a severe bleeding from ankle probably due to varicose veins rupture .She was given TPA and discharged within 5 days.She died of cardiac arrest within a month She was lean and nonHT / Diabetic .
what i've learned from this video: 1- acute management of stroke is to give tpa ( tissue plasminogen activator) if the patient has ischemic stroke , diabetic and presenting within 3 hrs 2- if the patient has ishcemic stroke, non diabetic and present within 4.5 hours according to their neurological deficit the patient is given tpa but tpa is not given if the patient is present after 4.5 hrs 3- heparin is not used in the management of acute stroke 4- if the patient presented after 3-4 hrs , he's given aspirin 325mgm 5- blood pressure control in acute management of ischemic stroke is very important 6- chronic management of stroke includes no tpa 7- if there's risk of dvt, give prophylaxis low dose low molecular weight 8- if i's the first stroke, give aspirin 8- if it's a repeated stroke, give aspirin with diapertamol 9- if the patient has allergy from aspirin , give clopidogrel never combine aspirin with clopidogrel

Recent guidelines, including ESO, recomended DAPT (so Aspirin AND Clopidogrel) for some time (between 21 to 90 days, depending on study). However you should still consider it according to the patient. Anyway, in emergency treatment it is not so important, by and large you should rather focus on thrombolysis and thrombectomy when possible, just like the author said. :)
