Scorcese is such a master storyteller. Here we see Travis say he wishes he could be "organizized". Not "organized". "O-R-G-A-N-I-Z-I-Z-E-D". This cuts deep into the impetus of the character. Note that by the end of the movie, he doesn't get that little sign that says organizized! A tragedy that could rival Sisyphus's.

That he never really met Betsy or even took her out on a date. However he always hung outside the place she worked, and when he finally went in and snapped. A sense of rejection, because he never really had the courage to approach her from the beginning. I always feel the last scene with her in the car, was also part of that fantasy and when she got out of the car. He finally learnt to let go of it.

It's not hallucination & can be rationalized. Even Scorcese never confirms if it's just a hallucination. It could be real since New York City in 1970s was so gritty and people with guns are praised to be hero by society (a problem still persist in America until today).
He was just too obsessed with the guy she works with and you can tell it was off-putting to the girl. Why was he concerned about him? It's clear that he wasn't in the same league with him.
