I didn't see the 0.0001 miners fee that you mentioned. Is that just not shown? Or did you mean the 0.005 was the miner's fee and mispoke?

At 1:00 do you mean as a new output? Or is change actually added in the raw txn somehow as an input?
Your wallet automatically generates a new "change" address to receive the 0.5 BTC from the example. So that change / excess amount is split off from the amount you pay to the receiver and redirected to the new address controlled by your wallet. Hope that clears it up.
