Can it print on the sides and back without having to remove the cap??

Yes you can definitely do the sides. Happy has the largest sewing field for caps at 290 degree rotation.
For the back you would either use a standard hoop or a third party back of cap hoop.

How do you center it? Do you go just by the red laser dot down on the center line of the cap?
