It's quite literally two 4004s stacked on top of each other into one siren, with additional brackets covering the top and bottom of the throat section.

Friendsville TN is the only other active one we know about. A bunch of the Seabrook ones were analog before getting 2030s.

This has got to be an early 4008 since the 4008 came out in 1988 and the controller is tuned to a high pitch.
