    Hey dr. Ryan, thanks so much for the videos and the work, much appreciated! I have a question, what is meant by compromised endodontically TX tooth? Is it just every endo TX tooth or an endo TX tooth with complications (or even more involved large post, little structure remaining)? Thank you in advance for your reply! Stay safe 🙂

      You’re welcome! Good question, any endodontically treated tooth is less desirable as an abutment, but the specific examples you list would be even less desirable.

    Wow!! Absolutely amazed you have refuted the long held misconception that endodontically treated teeth are more "brittle" after Alan Helfer's manuscript from 1972. Good job Dr. Ryan! You are absolutely correct - endodontically treated teeth are generally weaker not because they lack moisture or are somehow drier and more "brittle." It is simply because, as you stated, there is more tooth structure lost following RCT. Very happy to hear you say this.
    Endodontically treated teeth are, however, enjoy similar prognosis for retention, support and function of FPD units as those that have vital pulps. The KEY component that people do not see is the first word in your statement (11:39): COMPROMISED. It is not the endodontics that causes these teeth to be poor choices as abutments. It's the fact that there is nothing left after it has been ravaged by caries, etc. and miracles are expected. Be sure and stress the compromised word a little more ;-) The same holds true for any tooth, vital or non-vital, in terms of survivability and usefulness as an abutment when it is compromised restoratively, periodontally or endodontically.

    Just a question regarding Ante's law: I read on multiple papers that this law is controversial and is not used widely anymore. Thoughts?

      Yes, you are absolutely right that it is controversial! I teach to the board exam and it is routinely asked on there, so that is why I have it included here.

    One question about splinting abutment teeth. I'm a bit confused about whether it carries a poor prognosis or if it is indicated in the cases you mentioned. Would you mind clarifying? The indications do make sense, I think I'm confused about when it has a bad prognosis.

      I’m glad you’re enjoying the videos! This is a great question. Multiple splinted teeth can carry a poor prognosis because #1 it makes hygiene a lot more difficult and #2 when teeth are splinted together, the entire system becomes more rigid so there is a higher chance that one of the connectors fractures. Hope that helps!

    Doctor, Thank you so much for this video. I finally got my questions answered as none of the 4 dentists I have consulted answered any of implied questions (which anyone with engineering degree may have about the teeth replacement options durability and longevity and future prognosis). I may now continue grieving for my bone loss and abscess under my teeth but at least I now understand that Endodontist treated crowned tooth which I thought could support partial denture on my last back molar seems to be what you call "poor treatment" option with not a good long term prognosis. When they consultated me they only gave me the verdict to extract and lose 2 or one teeth, and I am now braving up to a possibility that both back end molar teeth need to be replaced by the only option Implants if I didn't want further bone loss (recession) inevitable for dentures second best temporary option based on my Mom's experience... But now I know only thanks to your video that I have both back molars compromised both Periodontically (not enough bone under the gum specifically to maintain the ideal ratios to be used as an abutment tooth) and Endodontically (old Root Canal and it's weaker and not going to last with high probabilities of greater force being used by back molars). I wish the Doctors who consult patients would explain in simple terms and words the aspects of durability and longevity of the proposed treatments (especially when someone might be hoping to miraculously save their teeth or most of them).

      It is still being taught believe it or not, and it can be asked about on the board exam. You are definitely correct though; it is an oversimplification and not only has it been considered outdated, but in some circles refuted outright.

      Good question. While a 1:2 crown to root ratio is ideal, 2:3 (which is slightly worse) is more realistic due to natural tooth morphology and the average clinical attachment loss in patients.
