The US healthcare system is a financial scam, designed for profit, not care. Unlike the rest of the world, the entire medical and healthcare system in the United States is for profit. And when there's money to be made, you can almost be sure that someone will abuse the system.
Health insurers implement secretive practices that stop people from accurately knowing what they're being charged. Companies like TeamHealth (owned by Blackstone) have such an influence over the business of the medical industry, that chances are your doctor or nurse is employed by them. And investment firms like Vanguard and Blackrock own at least a part in almost every single major publicly-traded hospital or healthcare system in the entire nation.
And of course, as you might expect, it's the poorest people in the nation who always suffer as a result of this. People who can't afford insurance, and even beg people not to take them to hospital when injured, because they likely won't be able to afford treatment.
This is an unbelievably important topic. I'd love to hear your thoughts.

We make healthcare trash too that isn't real addressed. Pill bottles, left overs, needle, medical trash,ECT. . No one can talk about it.At my job the public restroom accrued lock boxes for lance and needle disposal.Since the mask lift they still haven't cleared out the boxes to be serviced. We have a lot medical money ,no trash recycle service .
      Something is off.
Medical neglect is different than other fraud or scam. In the same town the food is thrown away if not sold.
Just another version of decline in the worst area, medical and well being. Not good long-term.

My ex-wife worked for an American insurance company. She would tell me stories that amazed us. For example, she would get calls asking what kind of coverage people have. One day an elderly woman called and asked if her husband fell down the stairs and was severely injured, could she call an ambulance for help. My ex-wife looked up her policy (this took a while) and told her that they would be covered for this situation. She then heard the woman yell, "Harold, we can call an ambulance. Hold on." This is just one of numerous stories.

As a nurse I can confirm that insurance companies stand in the way of our patients healthcare on a daily basis. The American system is rotten to the core and corrupt. The Netherlands have taken over the US medical system. There used to be good healthcare, now it is a disaster. "Evidenced based" criteria, hours of gaining prior authorizations, patients having to do with cheaper, less effective medicines. When a very ill patient has to continue to take a less effective medication for 30/60/90 days, all the while remaining ill (and sometimes declining) before insurance will pay for a more effective treatment is criminal.

Went had the Colonoscopy, they removed a non-cancerous growth, then my insurance said they did something that wasn't authorized, after telling me everything was covered, left me with a $3000 bill for the entire cost of the Colonoscopy. A bit taken back, I asked, "So what you're telling me is that I'm going to pay you, one way or another, even if you don't provide any medical coverage for me once I move out of the U.S.?" We wouldn't pay insurance, but our taxes would increase by more than the cost of insurance. Then, under universal coverage, the companies raise their prices, then the government raises our taxes to pay for it. It would still be corrupt. I remember working in the 80s when we still had "fee-for-service" healthcare. When people called to ask what the charge was for a lab test, I would look through a list and tell them. Only one price was listed. By the early aughts, it got a little more difficult to find the prices and my supervisor told me not to give out prices. I investigated a little further and discovered that the price for a CBC or chem panel was listed with 10 different prices depending on the insurance or lack thereof. By 2011 I discovered that dentists were charging MORE for a cash payment than a customer with Delta Dental. That means that Delta was punishing customers who preferred to pay cash rather than sign up with Delta Dental Insurance. In the 80s and 90s, all bills were less if you paid cash. Folks - We are in the Monopoly Stage of Capitalism!

You are correct. I spent the last 18 years of my career in the healthcare industry before retiring. After many years working in different organizations, the ugly picture of why unhealthy people are good for business and why the system creates, exploits, and feeds off of them emerged to me. The hospital industry and pharmaceutical industry are symbiotic. The FDA food recommendations are a creation through intense lobbying by the food industry. In fact they were written by the food industry and the main driver of poor health, obesity, and most of the diseases plaguing the American public. I cannot begin to explain in enough detail in a comment here to do it justice. It would take hours or days. But what can be said is US healthcare is an avoidable human tragedy that does not have to be.

I know I'm about a year late on this, but there is an angle you're missing that is used to also prop up the current system: The Veteran's Administration. Those who served in the Military, like myself, can opt in to the VA medical system. The US then handled all medical bills with a sort of balance sheet that goes by your current income. There are a number of levels, but at the lower end it'll cover everything save for a percentage of your medications. But notice that the funding for the VA is voted on by Congress, it's part of funding bills and what-not. And very VERY often, Congress is voting to cut the funding for the VA, which puts the VA is a very awkward position of having to cut costs while treating a massive amount of people. This means cut corners, reduced testing, etc etc etc. But then, the VA system is turned around and used by those very same politicians as evidence that a universal health care system doesn't work. They've created a bad example, and trot it out anytime someone talks about major changes to health care as an example.

Hi, medical coder here. This goes so much deeper than most people realize. The facilities and the physicians (and other non-physician professionals, NPPs) bill separately from each other. The code sets are ICD-10-PCS (facility), CPT (professional), HCPCS level II (other services and supplies), and ICD-10-CM (diagnoses). These all have their own EXTREMELY COMPLICATED guidelines, and then CMS and the commercial insurance payers have their own additional rules about how they will accept the coding. Facility claims are reimbursed through HCC risk adjustment and professional claims are reimbursed by fee-for-service. However, the professionals do not simply receive all that money. A lot of them are paid based on RVUs, while others are salaried. Sometimes physicians have bonus incentives. There are still bills that need to be paid after those payments are received, even if they work within hospitals. The rules for preauthorization, documentation, coding, and billing are so complex that the overhead for revenue cycle management is outrageous. The coding and documentation requirements have to be searched for on complex government and insurance payer provider portals, and then there are pay walls to see the actual guidelines like AHA Coding Clinic and CPT Assistant articles. The insurance companies update their medical and payment policies monthly and the codesets change annually. Then there are CMS updates every day to read through. Reading and interpreting the Medicare LCDs and NCDs alone is not straightforward, let alone the Final Physician Rule and all those other fun publications. Then there are system limitations with the already expensive electronic health record (EHR) and medical billing software systems that still require users to do a ton of manual labor to adhere to those policies. The changes often require system build changes by IT staff, and many times there is no room in the budget for this. Physicians and professional staff are burdened by clinical documentation having become a billing document and this takes away from the time they need to deliver care to their patients. They are not trained on CDI, coding, and billing in medical school. The physicians don't just take out a recorder and dictate a note anymore without running into problems. Physicians end up having to shut down their private practices and move to hospitals because the administrative burden is too high, but then they learn that it does not get easier. Many hospitals and practices have to outsource the revenue cycle work, and this can and often does lead to even more struggles with following the guidelines and adhering to timely filing limits. The hospitals will often go with the cheapest companies, and then end up with millions of dollars in AR. You get what you pay for. The denial rate can shoot up if people are not constantly monitoring it, and the appeals process is incredibly arduous. The insurance companies know that a lot of the organizations do not have the time and resources to successfully appeal the denied claims. Then there is the fact that just because a claim gets paid does not mean it was coded and billed correctly. Way after reimbursement is obtained, audits and payment takebacks can happen if even just one error is identified. The insurance payers make up the rules and then decide that they don't need to pay the claims. The physicians sign the insurance contracts every year without much of any opportunity for recourse for the contract provisions, since the hospitals spend most of their time negotiating in favor of the facility side. The professional side is not nearly as lucrative. Claims that don't get paid are adjusted as write-offs, but this still takes away from the work RVUs for the physicians. Working within the professional revenue cycle is a neverending game of cat and mouse that pretty much negatively impacts everyone involved except the insurance companies. Surgeons will not hesitate to tell you how ridiculous this has become. A lot of the physicians retire or leave because the system is not advocating for them. Everything I have said here will probably not make any sense to people who do not deal with this regularly. It makes me so angry because it is so difficult for experts in this field to get documentation and claims generated correctly and appropriately paid, which means that it is not made accessible or transparent to the patients at all. The Hospital Price Transparency Final Rule is the tiniest baby step.

However, I’m really concerned that if we move to a single payer, the companies currently running the system will find a way to make it more expensive and more broken. There’s no way that they would give up the power they have and no way their people in the government would oppose them. The affordable care act is a good illustration of the inability of the US government to fix this problem.

We are discussing WESTERN health care here and I can tell you without fear of contradiction that the standard of care in the "richer" WESTERN countries is at least as good as in the US and in most cases better. As to cost, the charge made per capita of ALL other countries (not necessarily rich or Western countries) is so low you would not believe it. Taxes are NOT very high and as in my earlier post you are falling for the lies of the BIG GREEDY CORPORATIONS that get absolutely obscenely rich from overcharging for healthcare in the US. As I stated, I've worked in our system and know others who have worked not only in the UK, but other places as well. High taxes and additional policies are required for TIMELY care...that IS substandard care. Cuba for instance..the stuff you see on the news? For govt officials, foreigners, and rich people only. I work with doctors from Cuba, so again, I know the facts. Not having free healthcare is not our issue, as everyone working in the system knows. The uninsured here have many options, and hospitals are REQUIRED by law to treat them. Again, I spent 25 years as a medical social worker, I know what I'm talking about. Until big pharma and the govt are no longer bed buddies, our system will NEVER be fair. Nice try, though.

I believe the problem is not that you have to pay for your treatment in the US ( you have to pay for it through taxation or government debt elsewhere anyway), but the lack of true competition among hospitals, clinics, health providers and insurance companies. The corrupt government maintains the corrupt health insurance systems with no transparency regarding health care prices. Imagine that the hospitals would always publicly have to inform about prices (just like you go to a grocery store) and compete with other health care providers on the open market. Next the insurance companies would pay the publicly known prices competing on the open, fully transparent market, among each other for customers.
I bet the cost of the health care would go down significantly. No corruption, just fair competition.

But now we’ve got to be our own doctors too? It’s just so defeating. We are so powerless. This topic is just one of the many ways our system is rigged, and you can only do so much to avoid being a victim of this corrupt system. You’d basically have to have infinite money and resources. You’d have to know how to do everything on your own. And that’s just impossible. You can know all of the truths about our broken system (which is good bc ignorance is what makes this so easy for them and only makes things worse for us) but there’s only so much you can do. Our insurance system is stupid, but having seen healthcare operate both here and in EU, there's more to consider than just cost. The quality of care, and timeliness of care matter as much if not more than cost. In England, for example, it's normal to wait months for critical appointments for things like cancer or other serious issues. It's also normal to undergo a surgical procedure and never meeting anyone from the surgical team or specialist that worked on you. These kinds of things would be incredibly bizarre in the US, but they are built into the NHIS system. It's so bad, that it actually appears from the outside, that the NHIS is actually hoping a certain percentage of patients actually just die, rather than receive care, so they can save on costs. And have you been in a hospital in England, and then visited a hospital in the US? The differences are astonishing.
So yeah, the insurance scam is stupid, but asking government to solve the issue would most likely be worse.
The real issue is that the system does not provide mechanisms for price transparency. As a patient, you cannot ask what a procedure will cost in advance of the procedure, because it's too difficult to navigate the insurance system. Because of this, there is no motive for organizations to compete which would cause prices to lower. So the real challenge is, how to craft a system that is not government owned or managed, in which health providers need to compete with one another on price, in such a way that patients can then choose from providers that offer affordable prices for care. A system like this should be able to address pricing for most non-emergency care scenarios, where immediate time to care was not a factor. At that point, I could see government programs to establish mechanisms to control costs on emergency care, which wouldn't bankrupt individuals that found themselves having an acute event - maximum out-of-pocket limits, with subsidies going to critical care organizations to cover the difference. Something like that. At that point, if people still feel like they need insurance, it's optional and up to the individual to decide.

They were (are) pushed on children to guilt trip uneducated parents into thinking the situation is severe to peddle unnecessary surgery. Ask yourself how many problematic cavities a 6-9 year old could possibly have when they JUST lost their baby teeth and had fresh, brand new ones grown in, yet fillings were being recommended left and right in my generation.

I'm an American and have hated this system my whole life. I went bankrupt WITH what was considered to be "good" health insurance. They simply didn't pay for things that actually worked and only paid for part of the things that didn't. (I have chronic health issues). If you are poor enough or old enough you get coverage through medicare/medicaid, but wouldn't you know I'm stuck in the middle? So I pay taxes for other people to have healthcare while not being covered at all myself.
It is a terrible scam and we Americans (80%) hate it but the corrupt politicians refuse to budge on the issue. We need a complete overhaul of the entire system.
Thanks for the vid. It's nice to hear a voice of reason on this insanity.

The only thing I would say, is that I hope when you say “hospitals” make deals, you mean management, people with business degrees. Almost every healthcare worker, myself included, believes healthcare is way too expensive. For one, I’ve known fellow medics and nurses that go bankrupt when they are sick, because we don’t get any discount for being in this field, and none of the high cost goes to the providers. Paramedics in many parts of the United States are making 40 thousand or less, Some are even volunteer. And nursing salaries have only been rising in the last decade or so, still not amazing though. Even doctors, unless you do elective surgeries and trial new medications, don’t get paid what they are worth. Speaking of emergency medicine in particular.

US is one of two countries in the world where you're allowed to trade prescript drugs that the only use a hospital in US provide is a bed or surgeries. Nurses would likely earn better at selling the medicine at the market than working in a hospital. If only the hospital wasn't the mafia. Of course you can't get a discount only because you're a nurse since you don't need permission to buy a drug from a hospital and the market sells to the hospital who gains return by selling the medicine for a higher cost. Basically lifting one hand over from left to right and then gets lots of money because of a title. They don't have guilt for a patient dying because all surgeries finished are successful even if the result didn't had a good effect and most is in the hand of god. US is weird in this perspective because you aren't responsible of a patient if you're not a nurse or doctor. Side effects of drugs is individual that the patient need a follow up on if the drug had a good effect that all power the doctor has is to not sell the drugs but the patient is able to buy it by importing it from other countries that these authorities sit on seats with victims to a free market.

Wow. I have no idea where you work, but you should probably look around for a different employer or a different area. This is not meant to be mean. I am also a paramedic as well, and my base salary for working a standard 48-96 schedule is 72k. With just a bit of overtime, I average 92k. I also have a friend who is a nurser with one year experience who was making $36/hr as a full time nurse, then 45 as PRN and then 75 as a contract nurse. With shift diff, she is around 95 and with OT, she pulls in a crazy 140/hr. I have actually met several RNs who make similar amounts. They do not get health benefits, but with that level of income, they can pay out of pocket, and most are covered on their spouce's insurance. (Insurance being tied to employment is stupid!) I know that if you look around, you can easily double your current rate of pay. And there is always an RN bridge program if you really want to make bank.

I'm a retired Marine who was combat wounded and as such am pretty deep in the "system". Before Covid its was, I'd say very acceptable. Since Covid, its has diminished several orders of magnitude. Since they bullied and pushed out those who wouldnt submit to a being part of a trial (EUA/my wife is an engineer in the FDA biotech regulatory space and didn't submit); the psychographic landscape is more narrow and the clinics/hospitals only have people who would either take it without investigation or who knuckled over for a job. In any case, the independent thinking lions, and the lionesses are gone. The system is choked up/backlogged, and we are suffering as a result. Id love for you to also make a video on the "free" health care systems. I live in Canada and our universal system comes at the cost of extremely high taxes and doesn't cover dental/eyes. We often get praised around this topic, bur in reality, our system is also broken. Unfortunately, when you remove the profit incentive, many issues get tossed under the rug and end up festering into disasters. Not only we have years long waiting lists to get a family doctor, but our waiting time in hospitals themselves is often over 10 hours. For example, last time I went to a hospital, they ended up telling me to go to another one because they had no doctors in the emergency room.. So I went to the closest one and was told that doctors only get in at 8am (it was 4am). I ended up waiting 12 hours to finally be told that I had to go see a private sector specialist. Also, if you have a busted knee and need surgery, expect to wait about 6 months for an MRI and another 6 months for surgery. I know so many people who decided to go to the US or to just spend the money at a private clinic to get good/fast health care. We really need to find a way to combine the private and public sector together to build something efficient and reliable.
