As I mentioned before, I didn’t have hot flashes (probably because I eat soy daily) so I didn’t take HRT not realizing all my joint pain was due to a decline in estrogen. And now at 65, I finally started HRT for osteoporosis. I wished I start earlier but better late than never. Glad you’re feeling better. Thanks for all you do.
If I am 65 is it too late for MRT? The worst symptom for me has been poor sleep.I can usually fall asleep but wake up often throughout the night, often as part of a hot flash/night sweat.Once I wake up I cant seem to shut my mind off and go back to sleep. This has been going on for years but the inability to fall back to sleep is definitely getting worse.
It’s not necessarily too late for menopausal hormone therapy (MHT), but it depends on your overall health and how long it’s been since menopause. Poor sleep, especially when tied to night sweats and hot flashes, can be a strong reason to explore options like MHT, as it’s one of the most effective treatments for those symptoms. That said, starting MHT after 60 can carry some risks, like cardiovascular concerns, so it’s really important to have a conversation with your doctor. They can evaluate your overall health and help you decide if MHT or other treatments, like sleep aids, cognitive-behavioral therapy for insomnia, or non-hormonal options for hot flashes, might be a better fit. It sounds like sleep is a big challenge for you, and getting it back on track could make a huge difference in your quality of life. Definitely worth exploring further!
When it comes to starting MHT, transdermal estradiol (I like Climara) paired with micronized progesterone (needed if you still have a uterus) is a fantastic option for many women. This could help organize your discussion. drmarbas.mykajabi.com/Perimenopause-and-Menopause-Evaluation-Checklist
I still have trouble staying asleep. I wake up at least twice if not more. And l seem to wake up at about 3am. Everything else is good. How can l increase sleep. Thank you
@@DrLaurieMarbas l keep hearing we need more sleep. But the question is how. I go to sleep alright but l am waking every hour and a half or so. With a set time of 3am to stay awake. I average between 4 5 and every now and then l will get 6 hours. I will look forward to seeing if there is something l can try. Thank you
I’ve experienced all of this joint pain, muscle fatigue, muscle soreness and weakness, and still experiencing it after being postmenopausal for 14 years. I am now 64 yrs old with a BMI of 19. I noticed if I put on any weight, the added pressure on my joints is not good. Unfortunately I do not think hypothyroidism which I have helps as well as having had severe endometriosis i. The past and having to be put on Lupron twice to prepare myself for surgery for the endometriosis which temporarily places you into menopause. I think having had to take that drug also during my prime time with muscle and bone building did not help. Do you think there would be any benefit for taking estrogen at this point? I do have osteopenia in my hips and osteoporosis 2.5 in my lower spine. I do get over 50 g to 75 grams of fiber a day on my Whole Foods plant-based diet.
It sounds like you’ve been through a lot, and it’s impressive how much effort you’re putting into staying healthy with your diet and lifestyle. As for starting estrogen at this stage, there might still be some benefits, especially for bone health, since it can help slow down osteoporosis progression and reduce fracture risk. That said, starting hormone therapy later in life can come with some risks, so it’s really important to talk it over with your doctor. They’ll consider your history, like hypothyroidism, Lupron use, and your current bone density, to see if it’s a good fit. There are also other options, like osteoporosis-specific medications, that might help.
@@DrLaurieMarbas thank you as always for your kind and thorough answers. I did want to let you know I started on a very low dose of estradiol vaginal cream. I will let you know if there’s any benefit to my muscle soreness and poor muscle recovery. I will keep on truckin since I know what I’m doing is for the best. I’m sure after many years of not doing the right thing has taken its toll and I understand that, but I will persist and again thank you for being the biggest and brightest light for this community. I wish you all of God‘s blessings today and into the new year. I wish you and your family a very merry Christmas and a happy and healthy new year!❤

@@DrLaurieMarbas I actually feel better following Dr. Brook Goldner's green smoothie recipes.

Happy holidays!
Thank you Dr. Marbas. I am going to talk to my doctor about Mht at my next visit. Is there a particular kind that is good to start with?

Dr. Marbas, do you check hormone levels? I read to aim for an estradiol level between 60-100 for osteoporosis. Thanks.
Hi, I do not check hormone levels as the guidelines say start low and increase until symptom resolution.
