I have SIJ Dysfunction- MRI is normal and Physical Therapy didn't help- Is Prolotherapy an option?

  • Prolotherapist Danielle Matias, MMS, PA-C discusses a pretty common scenario of patients who are diagnosed with sacroiliac joint dysfunction but whose MRI is normal and they try some physical therapy but it doesn't resolve the issue. The reason for this is most frequently underlying ligament laxity in the region causing SI joint instability. When ligament laxity/joint instability is found, Prolotherapy is a great option worth exploring because it stimulates ligament repair and tightening.

    Story of my life. Very frustrating to have Doctors think you are trying to bullshit them when you are in pain 24/7. And nothing works.

      @@CaringmedicalProlotherapy thanks. I've been dealing with chronic pain, sij for over 25 years. Osteoarthritis 10 years and crohns for 15. I can not take nsaids because i am allergic to them and it was the only thing that helped for inflammation. I've tried cortisone shots, physical therapy, humira shots, acupuncture, nothing works. Deep tissue massage is the only thing that helps at all but of course insurance won't pay for that and it only lasts 3-4 hours at most. I would give up everything i owned to be pain free but i guess it isn't in the deck i was dealt. 🤬

      I was told 3yrs ago to stop thinking about my pain and push myself harder, by a 27yrs old physio , im 59yrs old and worked hard all my life on my feet and brought up 2 children as a single mother.. Turns out he was looking in the wrong place on my MRI..
      I finally found great physio and have just had another MRI yesterday and having a pain injection in my SI joint on monday.. I diagnosed myself after watching Bob and Brad on YT, and a consultant confirmed it.. Its been a long painful and frustrating road but hopefully things will get sorted soon and I can play with my 5yrs old twin grandsons like i want ... 🙏🙏🙏

    I have this problem and doctors treat you like you’re okay and exaggerating. This has affected every facet of my life. It has made life activities so difficult and a lot of people don’t understand how painful.

    I had this problem, the best way to correct it is to stabilise the hip by getting it level and then doing exercise around the muscles to streaghten them the ligaments start to grow stronger it takes around 6 to 8 weeks working slowly every day. I have never felt so much better when playing football. I have had this problem for over 10 years and always suffered doing sport. The rubber bands are the best for straightening all the weak muscles/ in balances in your body.

      I had many rounds of prolotherapy in si joint area plus chiro adjustments. Took long time, but got pretty close to normal again. I can hike long distances, 30 km. I also used to wear a sacroiliac support belt to help hold di area in alignment.

    My story, after a fall I have been in pain for a year. I tried physiotherapy, chiropractor, Xray.. tend machines, hot patches, exercises, medications, and it is frustrating.
    What is giving me relief right now is a SI belt , thanks to that belt I am sleeping again, and IMS needles.

      @@gfriedman99 Probably for the same reason I haven't gotten it, cost. Insurance doesn't pay for it, they consider it experimental.

      The same thing happened to me after a fall on my right hip...I've done it all including seeing a chiro for 4 years. It's helped a bit but really helped a lot was getting an SI belt.

    Wow, I’m 61 and injured by vehicle 45 years ago, then again in 2020. It’s a miracle I can still walk. Pain and stability are almost gone.
    Hips are failing. Tailbone.
    🇨🇦 lost in Canada and doctors not helping.

    I have suffered from this for over 10 years. It was undiagnosed they could not figure out what was wrong, I seen a new specialist and they told me they believe its s.i joint dysfunction. He had told me I will live with the pain for the rest of my life. I seen a surgeon and they told me no surgeon in Canada would touch it. My mri didn't show much other then an extruded disk so its been a very confusing and frustrating decade for me.
    This video instantly made me finally understand the Injury more but also made me confused as to why mine is said to be permanent

      @@lesliecrawford783 I own a few different styles, I find it's great for short relief wile standing but sitting or sleeping it does not help me.
      Over the decade I have tried numerous things but only ever find very short relief. I have been prescribed numerous medications and have tried leaning myself off of them because I am still quite young to be on so much medication. I have tried creams and exercises but it seems know matter what I try I am in some sort of pain and just pushing through the day.
      If my joint cracks I get the most relief but the pain come back within a few minutes to a few hours. I personally believe the fight for sleep is the worst part.

      I suggest you to find a pain management dept and explain to the dr and they will help you out with SJ injections. The same happened to me I always told my dr of a weird hip pain very intense and by my self I went to see a rheumatologist bcz i have arthritis and they told me I was in the wrong dr bcz stronger pain are treated with a pain management dr. First time I went to see a pain dr he knew right away from where it was n did a few movements on my bad side n he knew. After he recommended me to get SJI n I did and feel better but my issue is very strong pain n I go see him to get injections every 6 months, if u still feel pain let him know to increase the pain meds injections. It’s don’t in a hospital with a lot of nurses n in n out. Hope it help you

    I have extremely loose ligaments as I was a contortionist and professional dancer when I was young and I am now elderly and have had several back surgeries and all they did was relieve a little bit of leg pain temporarily which didn't last long and left me with severe spinal stenosis-can only stand for 5 minutes. So true that loose ligaments make you very unstable and prone to injuries and probably why I've had so many in my life time. Wish you were closer I would come for treatment.

      I'm not usually on the floor (though it's more comfortable to sleep on then any bed) lol but yep, my goal is to be able to stand the whole time, pain free, while fixing dinner or making plates again. Not to mention being able to do work outside.
      Goals are good to have for sure.

    I've fallen hard on my butt and had this SI Joint distinction. A chiropractor adjusted me and told me to move gently for a month which I didn't do. He gave me a Serola si belt which I eventually used and it made the difference. I had to use it a while until symptoms went away. Occasionally I need it for short stints. Strengthening and weight loss prevent any low back issues.

    You have no idea how much I need to take a trip from Australia to have this treatment for my endometriosis. My nerves are going crazy at times. I really want to try this before having another surgery 🙂

      @ Niki Tobias I'm sorry you've got Endometrial pain. I don't know Medical care is in Australia but I would like to suggest you check out the pain relief machine called Alpha Stim™. There's2 different machines but the Alpha Stim M ™ is more versatile & can do more. You might not need a prescription to get it in your country. (& it may be less expensive or covered). At least it will buy you time. There aren't many NUCCA Chiropractic Doctors in Australia but if you can get to one, I strongly urge you to at least meet with one & talk to them too.

      @@lorriefinley3129 Thank you so much. I will look into it. I find if I stimulate my nervous system it triggers my nervous system. To be honest the only thing that works for me when I have a flare is to take a mild dose of codeine before I sleep one or two nights 😕

    Very informative video! I was also curious, when you do comprehensive prolotherapy of all the ligaments of the SI joint, which ligaments are those exactly? Is it just the iliolumbar, sacrospinous, sacrotuberous, and then all posterior sacroiliac ligaments? How about ligaments like the interosseous sacroiliac ligament? Do you bother with any sacrococcygeal ligaments? Lastly, does each ligament get a single injection per procedure or do multiple injections span the area of each ligament? Trying to learn more about the comprehensive approach

    Sounds like me But my pain is better today. Before today its been SO painful all the time when standing or walking. Especially in my front quad. Could barely hang up 2 shirts before my leg was so bad.
    Barely lifted a bed and this happened.

    I've been suffering from SI joint pain for three years and exercises/stretching do not work.
    I can feel pain touching the SI joint with my thumb (it's always the same spot, the pain just stays there), does this mean that ligaments are involved? Thank you

    The more videos i watch on these back and stomach videos the more i want to pack my husband on a plane and come here. I still have to call you folks. Im just traumatized and so is my hubby from lack of care and treatment here in Michigan. The drs think hes lying and hes not! 2 videos now that have pretty much diagnosed my very ill and pain riddled husband. We have lost hope and faith this will ever get fixed. I need to get him here

    Hi I had a.low back injury, something popped while leaning over. I was on opioids for 2 weeks. I didn't have nerve pain down the leg, pain was.local and radiated to front of pelvis. However it has reaggravated and I get pain down my left leg, not like nerve pain, its like a painful pulling,.like when overstretched, but it happens when I extend the leg to walk and left side feels tight. There is a local pain on palpation like pushing on a bruise near sacral bones, not in sacroiliac joint, more close to spine below s1... could this be caused by what you are talking about in this video?

    those prp and prolotherapy injections are great but majority of insurance doesnt pay for them and they are expensive

    Can you explain where exactly the prolotherapy injection should go? Is the picture in the video accurately showing where the injection should go? My daughter had Ozone injections done and got some relief but not full relief of pain.

    I've suffered with this since i had my son 11 years ago. I was so excited when i got accepted to a pain clinic (i was 25 and truly thought pain clinic meant clinic that TREATS your pain) I was put on opiates and offered to have my nerve endings burned off or some fusion surgery. Later i was told that the pain was in my head due to depression.
    If only I'd have known there were places like this that actually want to help you....

      Or pain mgt doesnt treat " acute pain" like pain from a surgery or a injury less than 3 months old and if you take rx from a Dr after surgery you must report it WITHIN 48HRS to pain mgt guess wht If I had surgery calling pain mgt isn't at the top of my list them you will see let's call her staci a N.P she writes a script you are responding well you are on rz 6 months magically 1 day staci is poof gone now you see? She doesn't tell you who she is doesn't know who you are or ANYTHING about you but TELLS you tht you must be under 90megs per day of morphine ( thts the old 2013 FDA recommendeds)) they changes the level of megs per day in 2023 but this clueless provider makes blanket statements about threatens you about taking away your rx

    A woman who practised fascia therapy on me ( I wasn't seeing her for fascia therapy!) severly injured me 5 months ago. She totally destabilised by right SI joint and my right leg. I'm still trying to get over this and spent a fortune on doctors (who just ignore the SI joint) osteopaths . I've finally found a real fascia pulsologist but she is away on a long holiday ! Yes seems ligaments were injured, also psoas. I lost mobility in my right foot so could not drive and this problem has returned a little last couple of days; freaky.

    That first picture is exactly what my MRI looks like. Just wrecked.
    Tried every type of PT, cried through all of them but one, the pool. Something about being in the water just makes it more tolerable. 💔💔💔

      She mentions a “negative MRI,” which here means no indication of an issue indicated on the MRI.

      @@trick351 Well yeah of course there's an issue because these people are in pain. The point is there is no definitive imaging procedure because weakened SI ligaments do not show up on MRI, ultrasound, or xray. The diagnostic is in clinical history and review by an experienced provider.

    Danielle Matias, very informative video. In your experience, have you ever heard of SI joint pain causing radiating/shooting pain down the right thigh? Almost every time my wife has excruciating shooting pain down her right thigh, one of her SI joints has been stuck (most commonly one side is stuck anteriorly and requires a Chicago Roll posterior adjustment which produces an audible clunk and her shooting pain doesn't occur again. She's been dealing with this pain for almost 2 years and as of yesterday received her first right SI joint steroid injection to see if this helps. She also looks to have a misaligned pubic bone with her right side being more prominent but unsure if this is due to her hip replacement. We've seen 16 doctors over the last 2 years and still no solid answer to what happened to start this issue and what is actually causing it.
    She had her total hip replacement at age 15, after normal recovery, she never had these pain patterns, was able to run, jump, bike, etc. Hip replacement has been evaluated by 3 orthos and they all agree it looks great...actually better than they expected 10 years later. My wife can go days with walking and have no pain but suddenly one of her SI joints will get stuck and cause the pain pattern. She can do the same leg workout 2-3 times and have no issues, but then on the 4th time, she has pain. It is such an interesting, frustrating case that has stumped so many physicians. Hoping you could help!!

    So what happens when you have natural hyper mobility in joints? When my SI or Lumbar area slides out, it can be as bad as not being able to walk, to fully in place and full mobility restored. Unfortunately, sliding out of place happens at bad times, like while out hiking. Pain and instability is so NOT a good thing. Dealing with it since 1980….with no doc helping.

      @ There's Dignard I suggest you try to be seen by a Healthcare Provider who specializes in Ehlers - Danlos Syndrome. They will have suggestions that will help as well as ways to manage other symptoms you most likely have too.

    Wow she explains the LIGAMENTS connected to the muscles. How doe prolotherapy work. I'm sorry Aldo for your pain . I know that this is hard We need to try prolotherapy. But how do they prove that it's the LIGAMENTS? Can acupuncture help with this too? We have so many questions and answers. May God illuminate our minds and I pray I find someone here in Houston.       A ligament injury is shown on the MRI. This is for people who have already had MRIs but nothing abnormal shows up.
      Prolotherapy involves injections and is still an unapproved treatment since it's still in very early stages. There are a lot of videos explaining how it works. You may want to ask your doctor what they know about it and if it's an option for you. Your insurance will probably not cover it so you need to be prepared for the cost.
      Acupuncture is also not really ideal, but for some people it may help. Some research shows that acupuncture comes out the same as simulated acupuncture. You need to talk to your doctor before doing either one though and investigate the provider for cleanliness, sterility, etc.

    Is it possible to have problem with SIJ without pain in joint area but lot of muscle imbalances and spasm and inhibition? Also no pain whit SIJ tests ?

    Story of my life I been suffering for 5 years with this. Chiropractor acupuncture weekly massage soaking in episode salt and magnesium nothing helped. I’m going to ask my PCP about this thanks for sharing.

    Thank you for all this info!!
    I felt pain after using the Torso Twist machine at the gym. I was doing 20 on each side, using 40lbs. I haven't gone to the gym in almost a month and I'm 95% pain free. My psyiatrist advised PT but after watching all the videos about the SI joint, I'm going to try the exercises for a week or 2 and see how I feel.

      I developed SI joint problems following lumbar fusion surgery. I had injections for both SI joints and injections into hip bursa both sides and these did not help at all. I went to therapy but that didn't really help either. I have worked out an exercise routine that is slowly helping. I used a couple of exercises from therapy and got several more from UA-cam videos. You will need to be your own diagnostician and therapist.

    Problems with my MRI but getting occasional injections now with mixed results, PT in therapy pool. Fibro has calmed down mostly so I'm actually making some progress after probably 10 years. The SI problems have been crippling over the last few years. Degenerative disc disease in cervical and lumbar spine, some stenosis, and osteoarthritis in SI joint and knee. Bursitis in right hip, femur wasn't actually completely in the socket for over a year and before that the hip was locked on the other side. I'm looking at surgery sooner or later.

    Danielle matias, after looking at all fhe caring medical and email since last year and i have been informed. You guys have successfully treated patient who have had if for years later, at the moment it's been 11months for me and i'm still unable to travel as my country borders has been closed due to this pandemic so therefore i would like to double confirm that it is okay to wait and be patient till i can travel for the treatment right? Because it will still probably even more months for me to travel as the borders are being lock down due to this pandemic
    And then once i get treated can return back successfully return to my competitive sports as i am a martial artist and does lots of strength and condition workouts.

      Prolotherapy is an injection of saline water solution into the ligaments that stabilize the SI joint area. It induces tissue growth of the ligament that makes it stronger and tighter and helps keep the joints aligned. That's the objective. Misalignment causes the pain, not something wrong with the actual joint, like arthritis, for example.

      I had prolotherapy for SI joint pain. The Dr told me it was glucose solution. He prodded my back for sore spots and injected it just under the skin - definitely not into the ligaments or joints. It took about 2 minutes. Eased the pain a bit but not for long.

    Ironically, I hyper-extended either the Psoas muscle or tendons in the area of the SI following the same sort of exercise I was doing during physical therapy. In therapy, using a stretch strap gave me a great deal of relief, but that was closely supervised by the physical therapist. So I bought a strap for home use and that's where things went wrong. I didn't feel I was getting enough of a stretch and ended up over-doing it. Now I'm stuck with this tightening in the pelvic area and have to do different exercises each morning to loosen up that area. X-rays and an MRI didn't reveal any damage.

    Thank you for this informative video. I believe this us exactly what I am suffering from since Feb. and it's not getting any better, only worse. Does anyone do this therapy in BC, Canada? I really need answers. I just learned about the trochanter belt. Would that help in your opinion?

      I found the belt made my SI pain worse. But try it. Worse case you take it off and don’t wear it again.

      have you tried cortisteroid injections in the SI joint? a physiatrist can do these or an orthopedic dr. they use ultrasound or xray machine to guide the needle directly into the SI joint and inject with a mix of steroid and different numbing meds. it may take a couple different shots for them to figure out the exact spot for your relief.

    I know upper cervical care can correct a leg length discrepancy, at least temporarily. But does the root cause of the discrepancy more commonly lie in the SI joint, whereby correcting the SI joint will result in longer lasting correction of the leg length discrepancy?

      @ McNature100 Hi! You're referring to NUCCA Chiropractic treatment? As someone that was successfully treated with this after my cervical spine was severely messed up after multiple car accidents. It caused a sideways kyphosis of my spine pulling one leg 3/4 inch shorter. Now, my spine is completely straight & legs are equal in length. This correction is holding now going on 15 years. (Occasionally my cervical vertebrae slips out when I'm under severe emotional or physical stress. I know when this happens because I will "trip" by catching the toe of my left foot. Then I go in for 1 maybe 2 adjustments & I'm good for another 6 months to 2- 3 years!
      I don't know if this answered your question, but imo, with my hyper mobility in my joints, I would expect my SI joint to dislocate much more often. Yet it doesn't anymore since the original adjustment.

    I had a lumbar fusion done in 2017. Immediately after I first awoke I had immediate pain in both thighs, along the outer side. So painful I could barely touch it. It has progressed over the years to be diagnosed as sciatica. The pain medication was not able to take it away, though it became a painful level I have tolerated. Just in the last week I was having a conversation with my pain management Dr about the areas that are so painful. After several months of failing epidural injections he now feels it's my si joints. I am hoping 🙏 that the injections help.

      I developed SI dysfunction following lumbar fusion. Had injections into SI joints and hip bursa but did not help at all. Therapy did not help. I worked out my own exercise routine taking some movements from therapy and many from youtube videos. Am slowly getting better but it takes a while. You will have to be your own therapist.

      Lumbar fusion is the #2 cause of SI Syndrome. I had lumbar fusion in 2020 and did well for almost 2 years. One morning I had difficulty getting out of bed and had a lot of pain on the right side. I couldn't turn over in bed. Then it was both sides and I have suffered with it for over a year now. I had epidurals and physical therapy which did nothing. I would like to try prolotherapy but cannot afford it.

    I was involved in a vehicle accident. Since then I have been having back pain. I went to see a spine surgeon who told me MRI look good, but EMG shows I have abnormal reading from my right leg. I have pain going down my right leg, hard to sit down for long period of time and when I do stand it hurts a lot. At first I thought It was my lumbar but after watching this video I think it is my SI joint. I'm going to go see another spine surgeon who can hopefully tell me what is really wrong with me.

    Yes. That’s my issue. I am hyper mobile and it happened using the hip abduction machine at the gym. The. Inner thigh one caused it by pulling the ligaments slightly and I am strong and use heavy weights. This particular machine to others I have used. Not had this issue prior and now My si is out of place ..moves around and is very painful and unstable.

That is very common as the research show ankle ligaments do not heal very well on their own after a sprain. We treat a lot of people for ankle instability and pain after a sprain and in our experience, most patients need a few treatments, depending on the extent of the injury but typically start feeling less pain and more stability within the first couple of treatments.       I have heard of terrible (permanent) damage caused by PROLOTHERAPY.
      (We only hear about the highly successful cures)
      Do in-depth research- look for negative testimonials.
      Stop eating sugar and carbs - especially ALL grain (especially wheat)
      Also No corn or Soy.!
      Eat meat and bone broth -
      Google all the utubes of DARKO VELCEK - his protocol requires commitment but he is a next level doctor - a gift.

    Urgent question
    Can one have an si joint dyfunction or si joint stability? Without any physical examination test sign being shown positive?
    Because i have all the symptoms of an si joint dysfunction.
    I have aching around my si joint, i have aching around glutes medius, and if i sit down for too long aching can increase
    If i increase my daily activities, the aching will increase.
    Even from time to time my do feel clicks/pops in my si joint area
    If i jerk and skip on my right leg, which is my affected side i feel like an almost sprain feeling. When i do "Child's pose" stretches i feel tightness and stretched feeling around that si joint area.
    But my physical therapist did si joint dysfunction tests on me, it showed no sign of positiveness
    So i'm very confusedand frustrated
    I'm planning to have prp prolotherapy but i need to clinically diagnose it
    So can i deteremine i have si joint dysfunction, without relying on the physical examination test since all the symptoms above i mentioned links directly to si joint dysfuntion.
    Or
    Maybe it is si joint ligament laxity?

Perhaps you need to see a different physical medicine specialist or osteopathic doctor as they may be able to do some additional tests that the physical therapist didn't do.       So i just want to ask you a important question rather than bending forward, can bending backwards in twisted way also lead to si joint instability/dysfunction too?

    Danielle, I have been viewing Caring Medical vids for a few months. I've posted comments to Dr. Ross on other UA-cam vids. Prolotherapy has become so very interesting to me. I have a compilation of spinal issues including stenosis and facet joint arthritis throughout my spine . I've been to 4 different Chiropractors duri g the past 10 years. I'm 69 yo, suffered a fractured right femur at age 15 which resulted in a 1 inch shorter right leg. I recently assumed that the growth plate in the bone had been affected, but a recent opinion from a chiropractor i dicated that he felt the bone was not set properly. The treatment in 1966 when this occurred entailed, leg in traction in the hospital for 6 or 7 weeks, then a full body cast for 10 weeks. The shorter right leg was determined when the cast came off. My parents or I thought to ask the doctor how to address that.... like wearing an adjustment to my shoes. After some years later, the shorter leg wreaked Havoc to my hips and spine. Dextroscoliosis to 25 degrees worsening over the years, and arthritis in many joints and spine. Pertaining to the SIJs, I suffer with sacroiliitis determined by my rheumatologist after taking xrays. She was looking for RA. That was ruled out after the blood test for the specific gene (don't remember exactly). She did say that sacroiliitis is ground Zero for gravitating up the spine and contributing to the spinal ailments. After looking more into Prolotherapy, and discussing with my Pain Management doctor, who said Prolotherapy would not really help my conditions, I'm seeking more opinions for treatments to stem the chronic pain and stiffness for many years . Like so many out there with chronic pain, it goes hand in hand with chronic Anxiety and depression. Dr. Ross is spot on with cervical instabilities and the Vagus Nerve. I suffer with many cervical issues: herniated discs, pretty severe DDD, Military Neck, with facet joint arthritis. I can Feel and Hear the bones cricking and cracking ausing so much pressure that gives me headaches to the bridge of my nose, and lots of TMJ issues. I appreciate this info so much. I'm in the Hunt for the right people here in Westchester county NY who an make a difference. I've had epidurals 3x , didnt do much at all. I've had facet joint injections 2x. The first set of 6 helped for about 2 weeks, the seond set was stopped as i became very woozy when the 2nd needle was inserted.

      It's worth it to try Prolo. Check out a doctor in Bryn Mawr Pa. Not as far away as Florida Trurhfully it will cost out of pocket but it can help you live a normal life and have relief.

      @Aldo Matulich Here is what I learned over a period of 20+ years following a twisting, popping injury of my SI joints. My unstable sacrum pulled my neck, jaw out of place. Look at the Sacro Occipital Chiropractic literature. The are minute rhythms between the sacrum and occiput ( and other cranial bones) that pump spinal fluid from the sacrum to the brain. The entire dural meningeal system ( and spinal cord via the filum terminale ) is attached inside the sacral canal. So, a dysfunctional sacrum can affect the entire body and cause unending miseries. The issue with prolotherapy : Exactly what ligaments were injured ? How can these be treated without over tightening un- injured ligaments ? For me, ( 5 prolotherapy sessions) the sacrum became even more unbalanced and I could no longer to a partial self correct. Keep reading , and check out Jerry Hesch videos.

    Yes interested to know abt prolotherapy! I was offered a fusion in left SI joint but I’ve gone through 5 back surgeries on discs including 2 spinal fusions & my surgeon said he couldn’t promise success🤷‍♀️ I had an injection into the joint in theatre but it just made my leg numb completely & no pain relief in fact worse! I’ve got sciatic pain constantly from the first level I had problems with over 30 years ago (mine started at 23) it’s at l4/5 & had never gone away. I’m fused half way up my back to my sacrum & im bent over & have dreadful spasms in my mid-upper back. I have all the referred pain from si joint it’s different from sciatic . The sciatic goes from lower back to along top of buttock then into buttock down back of leg then don’t front then front of foot to toes. The SI starts on similar path (but from joint) then goes to groin down femoral nerve then back of lower leg to ankle (like a vice at times ) then under my foot. I can barely walk, I have constant pain, numbness, pins/needles weakness to point of leg giving way- basically a disaster. My skin is thin at lower back from steroid injections & again don’t get much relief. My hip area hurts on both sides & my surgeon used to give injections that felt went into joints - ie hip area, but he retired & im now given less aggressive ones like for bursitis. I’m told I don’t have it but I can’t lie on left side 🤷‍♀️the more deeper injections helped a bit in that area, but no one seems to know what I’m talking abt. My surgeon said the hip area pain was linked to SI joint pain 🤷‍♀️ soz for essay but any suggestions coveted! Take care all- this pain is awful for us all especially when meds are not always given especially when you know it’s not in your head! Blessings anyway 🙏🏻🥰

      Injections are typically steroids and they break down bones especially as you age, have chemo, etc. So they are not such a great solution to replete. This video suggest an injection of our own red blood ( platelets) which the National Institutes of Health has studied and used successfully in frozen shoulders and other spine and hip areas.

      I had a lumbar fusion at the beginning of 2020. No one bothered to tell me that lumbar fusion very often results in SI joint problems. Did well for almost two years, then problems started with my right SI joint. Soon it was both SI joints. I had steroid injections in both SI joints and the right bursa which did nothing. Same with physical therapy. I'm really having a hard time with this and would like to get prolotherapy but insurance will not pay for it. I have two different SI belts and they do provide some relief. Hope you find some relief.

    This is happening to me right now. I have tingling running into my leg, from the SI joint inflammation. I am failing with the osteopath and the doctors don’t seem to recognise this as an issue. I have soreness over the joint and the nerve irritation mirrors sciatica.

If you have SI joint pain and are a good candidate for prolo I guarantee that you will consider it the best money you ever spent on yourself. Even if insurance covered it chances are you would still be out-of-pocket because of the large deductibles most plans have these days. In the scheme of things the cost for prolo is not excessive given the benefit it provides. It's a much better option than surgery as Danielle explains at the end of the video.

    I've got fibromyalgia and Ehlers Danlos and sacroilitis in both hips now I've got osteoarthritis in my hands feet and knees, I can stretch extremely well but my joints feel like they're stuck, locked up or click out of place, and and I'm wearing orthotics because I have two propinated ankles with completely flat feet, can I do anything about any of it without painkillers? I've been at it 21 years and I'm only 42. I feel jerky when I walk. I miss walking for miles 😢 lucky if I can get my shoes on. I get bad paraesthesia in my left hip too.

    is it best to go to a chiro, get si rotated into place then go to the clinic for prolo so it stiffens the sis in alignment?

      That was my thought before treatment. The answer is no you don’t need chiro. As the ligaments strengthen they will pull the joint back into alignment on their own. I felt the Si click back into place a few days after treatment. And it stays that way.

      @@archanasworldofsongs Yes getting chiro treatment more than 3 or 4 times is not a good idea. If the joint is locked up from a severe sprain with a lot of pain then yes a chiro can pop it and unlock it which will bring rapid relief. But it wont fix the chronic instability after that. It will make it worse by loosening the ligaments by high impact chiro manipulation. Only prolo will fix it permanently without surgery.

    How do you know the condition of the ligaments for sure? My pain is severe around SI, pelvic bone does not stay where it should, pain radiates to hip and groin making it very difficult to walk. Pain management wants me to get a new MRI, but I’ve had this issue for years now…with two lumbar MRIs being done previously….and of course they just look at the report and can’t read the images themselves. I just need to know how to confirm everything first before I figure out where to go from here. Physical therapy made things much worse and so did chiropractic.

      That’s a great question. When you come for your first visit, you receive a cone beam, DMX, and ultrasound. This type of testing allows for Danielle and Dr. Hauser to see exactly where the instability is and its severity. After testing you have a consult with either Danielle or Dr. Hauser and they are able to make a treatment plan!

      Hi Eric small, Thanks for the question. No, there are objective changes that can occur on x-ray and other imaging, as well as changes in tissue samples when it was first studied in animals. Additionally, we see changes on physical exam as tissue integrity improves. Take care.

    When I bend slightly my right psis (dimple) pops out and there is no pain on pressing but prolong standing and sitting causes pain at that area and moving or standing up from sitting causes popping noise sometimes

    I’ve been to 4 physical therapist, 1 chiro, and have been seeing an Orthopedic Dr out of Rush Chicago. MRI shows “arthritis” but nothing else. I’ve had multiple injections to no relief. I have no sciatica, just major tightness and also feels like there’s something wrong structurally to my back. Especially when lifting anything. This has not even been brought up. They’re sending me for an SI injection next but only because they don’t know. It’s been 2 yesrs

      @@mmkat99 What you are describing doesn't sound like SI pain. With SI you can almost always point to the exact spot that hurts. It's going to be a couple inches left or right at the base of the spine where those dimples are. SI pain doesn't feel like a tightness - it's varies from dull ache to sharp pain that can force you to the floor depending on how unstable the joint is.

    What exam can reveal this problem?
    I had THR in 2017. Fell last year on my hips and lower back. The fall caused me pain and discomfort in both hips and lower back. An EMG/NVC showed three Pinched Nerves, my pain is also in my groin and the back of my hips, which I have never had before, even when I had my left THR in 2017. No one can figure out what's wrong and I sleep about 2 to 4 hrs and sometime not at all. I think something happened to my Femoral and Obturator Nerves or the ligaments in the areas.. And yet no one believes me. MRI and Ultrasound shows nothing. I need a CAT Scan and my Orthopedic Surgeon doesn't believe it's necessary.

      Omg I’ve been going through the same thing! I had a discectomy on my L5, S1 joint the end of April this year. I had a budged disc, that herniated , part of that disc broke off and lauded into my sciatic nerve (right side) sending me into emergency surgery! Accompanied with deGenerative disc disease . Surgery was successful .. no more pain on my right side, it’s all on my left.🤦‍♀️

I also have tingling burning sensations in parts of my body, while other parts are cold. She is checking for Plexopathy,Neuropathy and Polyneuropathy and Genetic Testing is being requested as well. Thank you again, so many times I've ask for a reply from someone with s suggestion, and you was the only one.💕

      @@elizabethtorres6069 I myself have peripheral nerve problems for a while and recently had a surgery with this doctor who is a peripheral nerve specialist/surgent. There are only a few doctors who deal with chronic peripheral nerve pains top to bottom. His name is Dr. T Tollestrup and he is in Nevada. There are many of his UA-cam videos explaining peripheral nerve pain and causes and what to do about it. Please check them to see if they are relevant to you. He also does piriformis muscle removal surgery to eliminate sciatica pains among other things. Hope you find some solutions soon.

I was wearing an SI belt during the periods I was getting the PRP injections. I am starting to also incorporate core and glute exercises bur my understanding is the healing of the ligament should reduce the pain significantly?

      @@SuperSlimShaady It sounds to me like you are not a patient of Caring Medical. So the benefits you see depends on how proficient your practitioner is. It also depends on how much you educate yourself about the SI and can advocate for yourself. The problem, as Danielle points out in the video, is that SI pain cannot be diagnosed with MRI. Therefore, the practitioner is very dependent on your explanation of the pain in order to differentiate from other disorders such as ruptured disc and on the location of the injured ligaments. The entire area is actually a mesh of ligaments (as shown in the thumbnail) - not just one ligament. So if your practitioner does not inject the right area or to the correct depth then the injured ligament will not start the healing cascade. All that said, if the injection goes to the correct location, in my case, I felt the joint click into place and stabilize in 2 days on one occasion and in about a week in another. After that there is gradual but noticeable improvement for about a month until the sciatica referred pain stops - if you have that issue. The ligaments continue to tighten over the next 6 to 9 months so there is continued gradual improvement to the point where - if all goes well - you are 100% pain free. In my case, after 6 months, SI pain never entered my mind any more. It was quite astonishing. And I had been living with the pain for 20 years until it got noticeably worse which is what made me try prolo.

      @@thetippingpoint172 Hey. Thanks for your reply. I am from overseas so no I am not a patient of this clinic. My situation is a bit complicated as it does show I have inflamattion in this area. Doctors & specialists have been trying to diagnose me and have assumed it is some sort of inflammatory arthropathy for years. However, I continue to be seronegative with no genetic markers and no morning stiffness. It is until I met a sports doctor who said my left SI joint is unstable and is not locking into place and diagnosed me with SI joint dysfunction. He explained I am tender in my sacral dorsal ligaments and hence why I am getting pain in this area (in my glutes). He said my inflammation is minimal, and is a cause of wear and tear due to this core issue not being resolved. I tried doing physical therapy for a few months but I did not get better. So, it was recommended I do PRP so help strengthen the ligament so my SI joint stabilises and my muscles were not enough to achieve this. It was explained to me that only my left side was dysfunctional, but I am also feeling pain in my right side as it is feeling abnormal loading. I am just hoping that it is a matter of time before I see the effects of PRP. Of course I am slowly continuing with core and glute activation as part of the rehab, but I am really helping to get a large pain reduction from the PRP. It has been two weeks since my last PRP injection, but I have not noticed any pain reduction yet. Does it take time to work? I do know however that it was injected in the right spot as when the local anaesthetic was applied, my pain was gone.

    Am at my wits end to the point of suicide .     I have had pain for decades I have had epidurals at least 7 over years acupuncture chiropractor PT I have pain that travels I also have RSD of L lower leg ankle/foot with limited range of motion my foot was paralyzed for a period of 5-6 months I have severe to mild pain chronically and I use diclofenac for pain which helps but if I didn’t have that I would be in a lot of pain I wish someone could fix it I like to paddle board and horse back ride and swim and bicycle I garden a lot too how can I stop this pain

      Hi @SuperSlimShaady - The pain relief normally continues for a few weeks after your first treatment. Between the second and fourth weeks, the initial stabilization induced by the Prolotherapy may feel like it has lessened because the initial growth of ligament tissue may not be fully complete, therefore some of the original pain may return during this “window period” of healing. Patients normally need about 3-6 treatments in 4-6 week intervals.

    isometric exercises are best for strengthening and thickening tendons and ligaments. Take glycine and vitamin c and work towards holding an position/exercise for 5 minutes.

      SI joint dysfunction is from either wear and tear over time or the result of a mechanical injury and is not the same thing as sacroilitis that you see in autoimmune conditions like spondylarthritis or ankylosing spondylitis. In those autoimmune conditions a lot of times the joint will fuse itself due to the chronic inflammation causing your body to lay down more bone matrix in that joint space.
      Easy to mix things up.

      Great question - this is something we see in a lot of out patients. At our office we use a DMX, this allows for us to see the instability when the body is in motion. An MRI (and other scans) could be normal, when in reality the symptoms and instability show during movement, which is why we use DMX.       I've been looking into an "SI belt". If you look it up, it's supposed to take pressure off the ligaments when you wear it. I haven't got one yet, but I'm real curious if it would help.

      I had pain in SI JOINT due to injury while working. Both sides of the pain were severe, especially the left side. However, treatment was performed with back disc diagnosis and abnormal muscle syndrome diagnosis, but there was no significant effect. However, I was also diagnosed with a ruptured hip joint and underwent surgery. SIJOINT pain has not been resolved and returned to work. Then, he was injured again and his hip arthroplasty ruptured on both sides. After that happened, I underwent three more hip surgeries on both sides. This time, SI JOINT pain continued on both sides, but the pain on the right side was more severe. In particular, the pain of Lillumbar ligament, PSIS, and Scrotuberous ligament persisted on both sides. If you put your weight on one side, you will experience pain that comes vertically from the PSIS to the rice bone, and pain in the horizontal direction of the PSIS when you extend your upper body left and right or change direction. I am not good at English because I am Korean. There is a limit to accurately expressing the pattern of my pain.However, it matches the pain patterns shown in the video. In particular, wearing the SIJOINT belt relieves pain on the SIJOINT side, but pain on the Lillumbar ligament and Scrotuberous ligament side remains. The posture in which the pain worsens causes pain throughout daily life, such as sitting, standing, walking, and lying down. Pain such as quadratus lump muscle, iliac muscle, grain, etc. occurs on both sides.

      @@absolute4001 Bro you need to realize 2 things. First, SI joint ligament problems are not visible on xrays or mri. They are good for broken bones and torn ligaments but just a weak ligaments do not show up on xray. Second, every doctor is different. They are trained differently. And they don't know everything - its impossible. You don't get a carpenter to fix your plumbing. You need to see a doctor that understands the problem you have. Listen again to this video. This practioner is highly skilled in using prolotherapy to strengthen ligaments. What she says in the video is exactly correct in every regard. Listen again and pay attention. How did you get injured?

      @@deannewise3320 The more treatments you get the stronger the repair. If your doctor only gives you one treatment and that treatment consists of one shot then chances are it wont last unless you are cautious and lucky not to re-injure the area. It sounds like your doctor is not giving comprehensive treatments if he says they usually don't last a year. It's a different approach but I would rather have 3 treatments and the area is strong enough to withstand re-injury instead of one treatment followed by injury then another treatment followed by injury then another treatment and so on....
