    I partly agree with you. It may not have been approved by the FDA but we both know very well how finasteride has an economic monopoly on baldness. and where one company has more money it also makes more market and boycotts the market of others. There is not a single video about finasteride where I do not read negative experiences in the comments regarding libido, gynecomastia, etc.. unlike RU58841 which I am sorry to say I have only ever heard good things about.

    Isn’t it sad that the two oldest drugs(fin & min) are still the most effective on the market. Basically scientists could come up with nothing, that could cure balding for decades now. As far as topicals are concerned the only acceptable choice seems to be Fluridil, which has at least a clinical research backing up its safety and efficacy.

    • @isaacwadepharmd3969
      @isaacwadepharmd3969 2 роки тому

      @@kungfu_kenny63 Yes, dizziness and fatigue are reported side effects, although rare. Probably a consequence of systemic absorption since these side effects are more frequently reported with oral minoxidil secondary to low blood pressure I believe. Topical minoxidil solution has a systemic absorption of roughly 1-2%. That means that if you are applying 1 ml of 5% solution twice daily, that is a 100 mg topical dose and as much as 2 mg may be absorbed into your bloodstream each day. For reference, oral minoxidil, which is essentially 100% systemically absorbed, is typically dosed at 10-40 mg per day. Oral minoxidil gets a bad wrap for causing cardiovascular side effects but it is typically only prescribed to very sick people - with kidney disease, heart disease, very high blood pressure, etc. I would guess that young healthy individuals would be less likely to experience side effects from systemic exposure to minoxidil.

    • @isaacwadepharmd3969
      @isaacwadepharmd3969 2 роки тому

      That being said, if you believe you are experiencing systemic side effects from the 5% minoxidil solution, you could try switching to the 5% foam with has half the systemic absorption compared to the solution or decreasing your total exposure to minoxidil by applying 1 ml of the 5% solution only once per day or switching to the 2% solution.

It's over a year and my hair is still falling out I've used literally everything and every day and I still use them I take finasteride 2.5 with Avodart Dutasteride 0.5 and use minoxidil mixed with finasteride, bicalutamide and cyproterone, RU58841(from china),and in the morning I use Stemoxdil mixed with dutasteride and spirolactone, and finasteride then put it on my head after using the microneedling 3ml I also do a daily head massage with a powerful vibrator. I also use ketoconazole shampoo every day. All this and my hair is still falling out and it is now worse than before. Greetings to you, and I hope that you will answer me. Why didn't all of this work for me

      @isaacwadepharmd3969 2 роки тому +8

      Hello! Very sorry to hear about your situation. It sounds like you are doing everything possible to preserve your hair but despite your best efforts your hair is continuing to shed. I imagine that this is extremely frustrating for you. To be completely honest, I wouldn't be able to provide you with a concrete answer as to why your hair is continuing to shed. My advice would be to see a physician (preferably a dermatologist) for a full assessment including bloodwork to rule out other causes of hairloss and to confirm male pattern hairloss as your primary diagnosis. I would also recommend speaking with a pharmacist or doctor about your current medication regimen - I am concerned for your safety here. The regimen that you are describing is excessive and I would be concerned about systemic side effects from all the topical anti androgens that you are using (bicalutamide, cyproterone, RU58841, spironolactone). In addition, it seems redundant to take oral finasteride with dutasteride - since you are most concerned about efficacy, you could probably discontinue the finasteride and have similar reductions in DHT with dutasteride alone. I would also check how you are microneedling (length of the needle, frequency, and the force that you are applying). Excessive microneedling can lead to poor hair outcomes from my understanding. I also don't believe there is much data to support head messages as an effective treatment for hairloss.

    • @isaacwadepharmd3969
      @isaacwadepharmd3969 2 роки тому +7

      Also, try and source your meds from a legitimate pharmacy - otherwise, impurities may harm your hair. But again and most importantly, make sure you see a doctor. If your hairloss is being caused by some other condition, all of the hairloss meds in the world will not save your hair. You would need to treat the underlying condition. And lastly, it's possible that you lost the genetic lottery and are destined to be bald. In that case, I would consider using a hair system or coming to terms with being bald. The potential damage that you could do to your health through these excessive regimens of experimental treatments is really not worth the risk in my opinion. Your physical health is so much more important. Hope this answer is helpful! All the best, Isaac

      @isaacwadepharmd3969 2 роки тому +2

      @@engyousif4383 Thank you my friend! That is the 1 million dollar question. If you look at any clinical study, there's always a group of individuals that do not respond to the treatment. Why is this the case? A lot of the time, we honestly don't know. Everyone has a different set of genetic and environmental factors that influence whether or not a particular drug will work for them. Sometimes we can figure out the reason for this, other times we can't. When it comes to male pattern hairloss, my understanding is that we really don't know why some people don't respond to treatment. All we know is that that the vast majority of men (greater or the same as 90%) will respond to finasteride or finasteride + minoxidil so these are typically the treatments that we recommend.

    I think you need to educate yourself more on post finasteride syndrome. Patients describe a very similar pathway in onset of symptoms. After stopping or during use of the drug, a "crash" happens where their previous symptoms of low libido, ED, insomnia, depression/anxiety, shortness of breathe, muscle wastage, fatigue, etc. all become noticeably worse very fast. People describe being unable to sleep and feeling like the body is in fight or flight mode all the time. The milder form of post finasteride syndrome is just the persistence of common finasteride side effects like low libido, ED, brain fog, and a few others.

So he is correct that we have absolutely zero public human studies on RU 54881 to evaluate, and just a single study on its own is already extremely weak evidence in the grand scheme of things to determine whether or not a drug is safe and effective enough to recommend for human use. Evaluating safety in particular can often require multiple high quality good methodology randomised controlled trials in humans and thousands of people to use the drug in order to try and catch those potentially more rare side effects.

      @@Man-u-flex No. Just to name a few reasons:
      1. You are confusing slightly systematic (a topical serum) for 100% systematic (oral meds). Magnitude always matters.
      2. They function differently
      3. Only fin is currently known for having risks of permanent side effects.

      That human trial on human participants was never released to the public. So we don’t know the results of that study or whether that study had good methodology and was well conducted. Also even one well conducted study on its own isn’t enough to deem a treatment safe and effective enough to recommend to the public. You need at the very least multiple well conducted randomised placebo controlled trials to come to that conclusion.

    It is stupid to discuss if a drug can be responsible of certain side effect if you wont believe that this is a side effect that the drug can cause. You are disregarding the evidence because you believe that the drug can't make this. Of course, it is easy to evaluate a side effect like pain or death, but psychological effects, by its nature, is more difficult, there is no analytics, no measure. Also, many idiots think that hormones and chemistry has little to do with emotion and sex. That's the problem of people ignorant who don't Believe in pfs

      The side effects are barely more than placebo. If you read the actual placebo controlled research trials into it, the sexual side effects will often be like 1.3% of placebo users experiencing sexual side effects and 1.8% of Finasteride users experiencing sexual side effects. Whereas antidepressants can have up to a 50% rate of sexual side effects in men.
