Is it just me, or does the wiki/fandom do level 0 dirty? Level 0 isn’t endless, there’s entities, and it is a class 1?
-No food
-No drinkable water
-Extremely loud lights
-No easy way to escape.
-most of the deaths come from this stage!
Also the first encounter with a smiler was in level 0! The main image of the skin stealer, and hound, are both in level 0!
Edit: here’s what level 0 gets wrong with the original creepy pasta.
-Level 0 being endless
-No entities
-There’s more than just a bunch of repeating yellow rooms (sub levels)
-Being unable to see others, even when you enter with them.
-NON EUCLIDIAN nature.
(Ok that’s actually a change that fits well.)

Theory:the smile room (like you said) is some sort of space creature that looks like a giant octopus and on each tentacle there is a mouth.All the mouths are different sizes and the smile room aka octopus can make some sort of portal that leads wherever the smile room wants to go.
But thats just my theory also the smile room is on the tentacles not the octopus himself.

It’s called “The classroom”. It’s a sub level of”the school”. It’s dreaded mainly for its entities, which are:
A- Teachers: they come in different forms (chemistry, maths, biology, etc). They’re very hostile and have different attacks (homework, detention (and sometimes) assault).
B-Bully: they are even more hostile. They have different attacks too, such as punch, kick, roast, etc and are typically scary
C-School shooter: if they see you, it’s already too late. They have a gun and are covered in black. If they come, a school lockdown will proceed.
To enter, you have to wake up at 6:30, carry a backpack and no-clip to it.
To exit, you have to wait for the bell to ring 14 times, carry your backpack and run out asap.
There's a VRChat horror world called The Smile Room! It may be only loosely inspired though, because it doesn't have a door-mouth. Not that I remember, anyway.
Hey I just thought of my own backrooms level an I think you will like it so it is very hard and rare to get too. The one bad thing about it is you need a pencil to get out of. It is basically a white room and anything you draw on the walls becomes real and you can even draw a door to another level any level at all and I think that this is a pretty good level idea. Btw it is called The Doodle Room.
The Smile Room would look at me and frown ngl. Because my life be a hot mess. Entities, take me now. I wont fight. 😅🤣

This does sound like a similar entity from the Backrooms sublevel "Flooded Windows" of the main level 37, those fake doors you find there which are disguised as regular door, when you enter you're inside a mouth. This is still worse though
i had a nightmare like this once before.
