    Palliative care is provided in a variety of settings:
    Hospitals: Many hospitals have palliative care teams or units that provide specialized care for patients with serious illnesses.
    Hospice: Hospice care is a type of palliative care that is provided in a hospice facility or in the patient's home. Hospice care focuses on providing comfort and support for patients who are in the final stages of a serious illness.
    Home care: Palliative care can also be provided in the patient's home, with a team of healthcare providers coming to the patient's home to provide care and support.
    Long-term care facilities: Patients who are in long-term care facilities, such as nursing homes or assisted living facilities, may receive palliative care as part of their overall care plan.
    Outpatient clinics: Palliative care can also be provided in outpatient clinics, where patients can receive symptom management and support without being admitted to a hospital or other facility.

    One of the last times I was hospitalized with heart and kidney failure I was visited by a palliative care person and I put it to him plainly... "what is palliative care?" he could not answer. I have searched around extensively to find this answer any have yet to find one... what does it mean to me? have I received palliative care and not known it? What specifically is it? I still don't know.

      I'm terminally ill and in pain. I was told the palliative care I will receive is to help control the pain to keep me comfortable and to keep me alive as long as possible with a good quality of life.

    I think the advanced care plan is crucial for families because we can make decisions which reflect what our loved one wants because the truth is when the plan is needed, our emotions are a lot higher and we make decisions based on emotions not what’s best for the patient. Speaking subjectively about dad with end stage dementia
