It's usually not that hard given how different the meanings given by each tone are (if a word even has a valid meaning for each tone). When sentence structure/wording does line up for it to be genuinely confusing, it's really about at the level of occurrence for puns in English. Same idea. There is a lot more casual "misuse" of the right word/sound in colloquial speaking/texting for humorous purposes, though, and of course a lot of traditions/superstitions based on those verbal puns.
